Souhrn Téma disertační práce jsem zvolila pro dlouholetý zájem o problematiku fototerapie. Fototerapie je léčebné využití světla, především ultrafialového záření. Mechanismy účinku ultrafialového záření na kůži lze označit jako fotobiologické a jejich výzkum nadále probíhá. Na obecné úrovni představuje fototerapie aplikaci energie do kůže, což zahrnuje dodání fotonů do chromoforů, absorpci fotonů v chromoforech a biologickou reakci absorbované energie fotonů. Ta vede v kůži ke vzniku tepla a k různým biochemickým účinkům. Dříve se hlavní role v léčebném účinku přisuzovala antimitotickému a antiproliferativnímu působení UV záření. V současnosti je zřejmé, že se kromě nich uplatňují mechanismy imunomodulační až imunosupresivní. S přibývajícími poznatky o imunitním systému kůže se odhalují další možné mechanismy působení ultrafialového světla. Pravidla a zvyklosti provádění fototerapie se na jednotlivých pracovištích liší a je žádoucí protokoly fototerapie usměrňovat tak, aby se dosahovalo optimálního poměru úspěšnosti léčby k její bezpečnosti. Také z těchto důvodů se realizovala epidemiologická studie Fototerapie v České republice. Vyplynulo z ní, že fototerapie představuje v České republice dostupnou a relativně často užívanou léčbu, umožňující ještě širší využití. Vybavení pracovišť je velmi dobré,... I choose the topic of phototherapy because of my longlasting interest in this field. Phototherapy is defined as the therapeutic use of light, primarily ultraviolet light. The mode of action on the skin is described as photobiological one and it has been continuously investigated. At its most basic level phototherapy represents the application of energy to skin, which means the delivery of photons to chromophores, the absorbtion of photons in chromophores and the biologic reaction of photon's absorbed energy. This causes the formation of heat and various biochemical effects in the skin. Previously the main role of UV light in phototherapy was thought to be the antimitotic and antiproliferative. Nowadays it is clear that beside them the mode of action is rather immunomodulatory and immunosuppressive. Further possible mechanism of the UV light are detected with increasing investigation of the immune system of skin. The rules and usages of phototherapy in practice at individual dermatological departments and offices differ and there is a need to improve and/or coordinate the protocols in order to reach the optimal risk/benfit ratio.
