Supervisor: MUDr. Hana Krejčí, Ph.D. Prague, 2019, 66p. The thesis deals with the application of the profession of nutritional therapist/specialist in the care of pregnant and nursing women. In the first half of the theoretical part, there are described individual differences in particular nutritional issues, between the period of pregnancy and the period of breastfeeding. In the second half of the theoretical part, the issue of nutrition health literacy and the characteristics of the professional relationship between the gynecologist and the nutritionist are discussed. The aim of the thesis was to design a real concept of how to include the profession of nutritional therapist/specialist in routine care for pregnant and breastfeeding women so that the current possibilities of specialized outpatient departments are taken into account. Change of a diet during pregnancy with diagnosis of gestational diabetes and without this diagnosis

The topic of this bachelor thesis is diet or changing diet of pregnant women monitored for gestational diabetes mellitus (hereinafter referred to as "GDM") compared to a control group of healthy women. The aim of this work was to verify and assess the impact of risk factors on the development of GDM and to compare the development of pregnancy from the point of view of weight of pregnant women and eating habits of pregnant women with and without GDM. To evaluate these goals, I conducted a questionnaire survey and critically assessed the results. My research has confirmed that during pregnancy, the eating habits of pregnant women have changed due to GDM, which has a very likely impact on the different weight gain of both groups (with or without GDM diagnosis). A large percentage of respondents admit that they had a problem with just limiting the sweets. On the other hand, the majority of respondents stated that they had more or less followed the recommended diet. Based on my research I can confirm direct relationship between GDM and some risk factors, especially the most frequent risk factors such as higher age, BMI above 25 and low physical activity. The risk of type 2 diabetes in women treated for gestational diabetes

This bachelor's thesis deals with the subject of gestational diabetes mellitus (GDM), the most common metabolic disorder in pregnancy. GDM is a carbohydrate metabolism disorder that develops during pregnancy and disappears after delivery. GDM is associated with a lot of complications and risks for both mother and child. For this reason, it is very important to make an early diagnosis of GDM and start timely treatment, which includes lifestyle changes, regular physical activity, or drug therapy. The theoretical part summarizes current information on GDM. In the practical part, follow-up OGTT data obtained 3 - 6 months after delivery were analysed. The aim of the thesis was to assess OGTT results through a statistical analysis of data of women treated for GDM diagnosed in 2016 and 2017. Retrospective processing of medical records was done. It was found that 75 patients (48.1%) had positive follow-up OGTT results after delivery, out of which 70 patients (93.3%) had elevated fasting blood glucose. 7 patients (9.3%) had increased glycaemia at 120 minutes after the test. Introduction: Diploma thesis deals with issues of gestational a pregestational diabetes and the influence od diabetic diet during pregnancy on eating habits. Goals: The theoretical part is focused on general summary of diabetes during pregnancy - its characteristic, epidemiology, risk factors, complications, treatment including glycemic self- monitoring, observation during and after the childbirth. The last part is focused on education, as the important part of the therapy. The emphasis is put on the practical part, which analyse eating habits including measures regime of women dealing with gestational a pregestational diabetes, before and after the pregnancy. Crucial task was the comparison of choice of the food, frequency of monitored food and the regularity of eating. Methods: Research took place through the anomyous questionnaires, obtained on Gynecological-obstetrical clinic of General hospital in Prague. The Master's thesis entitled "Gestational Diabetes Diet" deals with the issue of eating habits and a lifestyle of women diagnosed with gestational diabetes mellitus during pregnancy. The theoretical part of the thesis describes the definition, causes and a mechanism of origin, consequences for mother and fetus and also deals with the diagnostic procedures and all possibilities of treatment of this disease. In this part, the diagnostic method analyzed by using the oGTT stress test and a diabetic diet - that together with other preventive measures (mainly physical aktivity) is the first choice in the treatment of pregnancy diabetes - are described in more details. The practical part of this master's thesis contains the results of a questionnaire survey in two groups of pregnant women - with already diagnosed gestational diabetes and before its diagnosis in the oGTT test. It also contains the results of the analysis of women with GDM diets. 7 Abstract The aim of my bachelor thesis, which deals with eating habits relating to gestational diabetes, is to discover potential effects of diabetic diet on eating habits of patients with diagnosed gestational diabetes mellitus. The main hypothesis is related directly to this aim. Other subsidiary aims and hypotheses reflect not only concrete changes of eating habits and constraints in comparison with previous habits but also diagnosis, education and treatment of gestational diabetes mellitus in concrete medical institution where the research took place. My work is divided into two main parts, theoretical and practical. Theoretical part aims at gathering available information about gestational diabetes - emergence of disease, manifestation, perils and complications, diagnosis and dietary treatment. There are briefly mentioned general rules and principles of healthy eating. The research focused on the gravid women to find out their interest of gravidity nutritional needs, where do they find the information about this topic and how are they satisfied with their quality. The research confirmed my premises that women are interested about this topic and their main source of information is internet (93% of respondents use it). They are mainly satisfied with the accessibility of information but their satisfaction of understandability and contents is worse. The influence of pharmacology and reduction diet on metabolism of adipose tissue in obese patients with diabetes type 2

Insulin resistance, obesity, type 2 diabetes mellitus and their complications are linked together under definition called Metabolic or Reaven Syndrome. The presence of Metabolic Syndrome increases the risk of atherosclerosis and consequently cardiovascular morbidity and mortality. Metabolic Syndrome belongs among civilization diseases and due to changes in our life styles becomes major health and social-economic issue. The insulin resistance is considered primary pathology that may induce other components of Metabolic Syndrome. Research conducted within last decade brought significant results and showed how important is the adipose tissue in the development of insulin resistance especially in the context of discovery of endocrine function of adipose tissue. The existence of nuclear receptors PPAR (peroxisome proliferator activated receptor) could explain the interaction between particular tissues and hormonal factors which may induce insulin resistance. When the PPAR receptors are bound by lipophilic ligands they directly affect transcriptional processes in cell nucleus and induce expression of genes involved in metabolism of lipids and carbohydrates, in the regulation of inflammation, in tumor growth regulation, in immune response, in cell differentiation and etc. In current medical practice...
