Influence of accreditation on patient

Nowadays the accreditation of hospitals is very current topic. During the last years there has been a change in the way of life of society. Within the provided services, the number of patients seeking higher quality medical facilities with specialist care rises. The basic lead for patient is the information about accreditation of the medical facility. The accreditation of the hospital is guarantee for patient that procedures used in the hospital comply with the prescribed standards. Standardization of care provides a unified approach to the patient and leads to safer and higher quality care. Thesis objectives Objective 1: To find out how the selected hospitals introduce "Program of improving the quality and safety of the services provided in a hospital accredited (JCI)" Objective 2: To explore the impact of accreditation (JCI) on patient satisfaction and quality of feedback. Objective 3: To define the benefits of accreditation at the hospital with JCI against the facility without JCI from the patient's perspective. Research questions 1) How does patient perceive the level of care, its accessibility and continuity in the accredited facility? 2) How does patient perceive the complex care of team of trained personnel (doctors, nurses, educational nurses, nutritional therapists)? 3) How does patient evaluate the awareness about all care related to his/her health during hospitalization? 4) What difference in care is there for patient at the hospital with accreditation from the other facility without it? Methodology To obtain the necessary information, a qualitative research was carried out and the methods of questioning with the technique of semi-structured interview on a pre-prepared set of questions were used. A total of 6 patients hospitalized in Hospital Na Homolce were interviewed. The hospital management was informed about the research in writing. Results and Conclusion The interviews revealed that the patients were provided by nursing care according to general or functional method during the hospitalisation in JCI accredited hospital. The care was available according to the patients? needs in terms of time and organisation. In one case a bed wasn?t arranged. The situation was resolved by agreement of staff with the patient. An important part of patient admission to the accredited hospital was staff presentation to patients, which was done, except for one patient. Furthermore, all patients agreed that a physician in the department had attended them every day. The cooperation of the multidisciplinary team worked well according to the patients, they particularly appreciated the work of a psychologist. Patients saw differences in the care in the security measures and in terms of availability, the interviews revealed that two respondents had experienced a long waiting time to scheduled surgery at a hospital without accreditation. The patients evaluated positively the level of provided nursing care. The results of the survey show that most of patients were aware of the differences in provided care. There is surely an influence of education of the nurses in the communication with patients, when was found out that it is needed to develop the communication more. The output of this thesis is a proposal of practical seminar for nurses, which should contain basic information about an effective communication with the patient, particularly aimed to feedback.

Artificial neural networks and their application in the assessment of insurance risks

The present work studies applicability of artificial neural networks in the assessment of insurance risk. Structure and function of multilayer perceptrons, Kohonen maps, Fuzzy ART networks, and Fuzzy ARTMAP networks are described. Concept of insurance risk is defined and the ratemaking by generalized linear models is introduced. Neural networks' methods for reduction of input space's dimension, knowledge extraction, and visualization are summed up. Data describing traffic accidents are acquainted and results achieved on them are presented. The work successfully demonstrates theoretically and experimentally that multilayer perceptrons approximate better than generalized linear models. Modification of multilayer perceptron estimated distribution function of total claim. Analysis performed by Kohonen map and by subsequent visualization detected two significant clusters. Analysis by Fuzzy ART network is presented as a part of new algorithm for reduction of input space's dimension. New algorithm inspired by Fuzzy ARTMAP network discovered a group of accidents where the claim is above average. This group is delimited by interpretable rules. Attached CD contains scripts for Matlab and MySQL that was used for mentioned analyses.

Initial stages of fatigue failure of polycrystalline materials

Geometry of persistent slip markings (PSMs) formed during cyclic loading of polycrystalline stainless steels and superalloy was studied using atomic force microscopy (AFM) and high-resolution scanning electron microscopy (SEM-FEG). Internal dislocation structure was investigated using transmission electron microscopy (TEM). Persistent slip bands with dislocation structure corresponding to the cyclic strain localization were identified and correlated with PSMs observed on the specimen surface.

Differentiation And Efficiency Of Public Expenditures On Prevention And Fight Against Crime

This Bachelor thesis is concerned with the description and analysis of criminality in the Czech Republic and with related public expenditures. In the first part is an outline of common progress of criminology, individual schools and their approaches to influence crime. It brings closer view of economics dealing with the solutions to crime rates and with calculations of the social and individual crime costs. Situational analysis of crime in the Czech Republic, its sources, background and structure of supervisory authorities follows. One chapter is devoted to the female crime. The second part analyzes the correlation between the crime rates, public expenditures on crime prevention and crime repression from national and regional perspectives. The main issue of this work is the efficiency of public expenditures on the prevention and regression of the crime in the Czech Republic.

Influence of handball on the shoulder girdle state

Title: Influence of handball on the shoulder girdle state. Goal of the project: Examine and evaluate of functional condition of shoulder joints of young handball players on various player positions between input and output investigation. Suggestion of strengthening and stretching exercises for the area of shoulder girdle and finding if the exercises has some influence on the shoulder point as well as on the player performance. Method: We made comparison of input and output test results by the method of "kvasiexperimental" qualitative study. A modified questionnaire was given to each player before input investigation. Based on the questionnaire we learned deviations from the function, eventually the pain in the area of shoulder joint. After input investigation strengthening and stretching exercises of the shoulder girdle for 13 handball players on the various player positions in the youth team of Dukla Prague were suggested. Results: Results of the investigation of shoulder girdles in the group of handball players did not prove large number of subjective symptoms in the sense of pain and limitation. The Value of CSR for Czech Consumers

Nowadays consumers purchasing behavior is influenced by new factors such as the social and environmental implication of companies. This is why Corporate Social Responsibility (CSR) is a growing trend which companies need to look after carefully. However implementing an efficient CSR strategy is a complex process for corporations; especially since the core concept of CSR remain quite blurry. The goal of this study is to analyze the perception of consumers towards CSR to find out about the value creation that CSR produce for consumers and its impact on their purchasing behavior. This paper will ultimately help companies to implement their CSR strategy more efficiently. This study aims to contribute by conducting an in-depth analysis of consumers attitudes and behavior towards CSR. If most of researchers are using a quantitative approach this study means to deal with the issue with a qualitative perspective. Indeed twelve semi-structured interviews will support the findings. On top of those practical and physical interviews some theoretical knowledge will be added to the construction of the argument especially to bring a framework that shows the importance of all types of value creation (functional emotional and social). The findings of the thesis emphasize the facts already proven by other researchers; value creation is fundamental to make consumers care about CSR. However the study will show how much skepticism towards CSR can impact negatively consumers purchasing behavior. The research will help companies implementing more successful CSR strategy and develop new solutions to reach customers and influence their purchasing behavior through the creation of value for them.

Civil emergency planning in Czech republic and its developement and role within North Atlantic Treaty Organization

The presented Thesis deals with Civil Emergency Planning (CEP) in the Czech Republic and its evolution and role within the Alliance. The theoretical section of the main body focuses on CEP at NATO and Czech national levels. In relation to NATO, this Thesis explains the concept of CEP, its origin and evolution within the Alliance. It describes the course of CEP development as NATO strategy concepts, priorities and CEP aims changed over the time due to changes in political situation. The important point to understand the whole issue is the depiction of tasks to be met by CEP within the Alliance, and the Ministerial Guidance being the primary CEP related document. Five basic tasks applicable from the very first edition of the Ministerial Guidance till now have been described in detail. An integral part of the NATO CEP is the visualisation of the organisational structure which is necessary for understanding the practical portion. The other section of the main body focuses on CEP as designed for the Czech Republic. At this level, the CEP is a methodological tool for management of incident and crisis planning guaranteeing their interaction and preventing duplicities with defence planning. It defines CEP´s areas of interest and it depicts legislative and institutional support. The practical portion is mainly focusing on meeting basic tasks by individual responsible entities working within four basic planning groups. To get the best possible view of tasks fulfilled based on the Ministerial Guidance, the following method has been chosen: main tasks defined by the Ministerial Guidance have been divided into three main parts in compliance with the task categories of the Ministerial Guidance. The tasks are coped with within the four planning groups. The planning groups are then assigned to ministries that are the responsible entities in terms of the Czech Republic. The result is a clear picture showing main tasks of public administration central bodies, and how these tasks are met within the Alliance plus their translation into the terms and conditions of the Czech Republic. The system also shows the history of each task depicting its conclusion or processing in which case it is transferred into the period of the next Ministerial, now Political, Guidance. The discussion analyses the issue of task fulfilment and importance of financial support to the CEP. The discussion concludes highlighting the necessity of interconnection between the national and NATO levels. This is ensured via permanent representatives in CEPC and representatives in individual planning groups. An example of some member nations shows, how the communication between CEP representatives works (or does not work) at various levels. The end of this Thesis evaluates legislative and institutional CEP interlinking of the Czech Republic and NATO. Unlike the institutional interlink, the legislative interlink is insufficient. The system of interlinking underlines the important role of the permanent representative in CEPC and representatives in planning groups. The main part of conclusion is dedicated to the facts resulting from outcomes and discussion giving clue whether the aims of this Thesis have been reached. The system, how the Czech CEP is working, has been found very efficient even in comparison with long-term members like Great Britain or France. Despite its clear quality and efficiency, it would be desirable to integrate the CEP into the legal system of the Czech Republic and thus to achieve its self-contained legal form. This change would not impact the work itself, but it could add to CEP´s political importance. At the end, a positive response is given to the research question evaluating whether the current status of CEP functioning in the Czech Republic is covering the needs of NATO.
