Prevention and therapy of spasticity in patients after cerabral apoplexy

Spasticity is a manifestation of many neurological diseases, for example an infantile cerebral palsy, a traumatic injury of brain and spinal cord, a multiple sclerosis and a stroke. A genesis of the spasticity is not yet completely clarified and many definitions try to explain it. The spasticity can deepen a disability of patients even with a minimal paresis. The spasticity has a different clinical picture at various diseases of the central nervous system, but even at different patients with the same diagnosis, it also varies in the course of time at one and the same patient. The biggest problem in the treatment of the spasticity is the fact that the treatment, efficient at one patient, can completely fail at the other. A goal of this thesis was to give an overview of current options in a treatment of the spasticity. Therapeutic approaches to reduce the spasticity are usually a part of special kinesio-therapeutic methodologies, which are for example an anti-spastic positioning, a volitionary relaxation, passive movements, an anti-spastic placing (Bobath´s conception), relaxation techniques (PNF), an application of splints, a long-term effect of the heat, an icing and others. On the contrary, it is intensified by pain, pressure sores, a sub-luxation position most often of the shoulder joint, a psychical stress, urinary tract infections etc. In the theoretical part I focused on a control of the muscle tone, pathophysiology, clinical symptomatology, diagnostics and especially prevention and the treatment of the spasticity. In the practical part I investigated an effectiveness of an anti-spastic therapy through the qualitative research. For the data collection it was used an anamnesis, observations (kinesiological analysis), case reports and a secondary data analysis. The research was made at two probands with a brain damage on the vascular basis. I was finding out an effectiveness of selected therapies for individual probands {--} and at the same time its suitable combination. I focused on the area of the upper extremity. At the first patient there was a success, by a suitable combination of individual methods, to positively affect the spasticity and an active momentum of the upper extremity. At the second patient in a chronic stage of spasticity it was managed to influence it always only for a short term and results varied considerably in the course of time. The treatment of the spasticity must be preceded by a thorough knowledge of pathophysiological mechanisms, a neurophysiology and above all a complex kinesiological analysis. The therapeutic approach should be always individual, to respond appropriately to changes in the clinical picture and to use suitable combinations of therapeutic approaches. If the treatment of the spasticity is successful, it positively affects the motor deficit and reduces the patient's disability.

Posibilities of Physiotherapeutic interventions in children with cerebral palsy

Physical therapy plays an important part in the process of treatment of this disorder. Which in this case for life. The objective of my thesis was to sum up theoretical knowledge and information on CCP and on physical therapy of children suffering from CCP. The first task was to set up an overview of options and alternatives of physical therapy for children with CCP. The other task was to work out case reports of two children with the CCP diagnosis. Then a therapy plan, based on the knowledge from the theoretical part and on the patient´s examination, was made up. A combination of therapy techniques was proposed (the Reflection Method of Locomotion {--} a method developed by M.D. Vaclav Vojta, Proprioceptive Neuromuscular Facilitation (PNF), Basal programmes and sub programmes) suitable for a client with regard to his current state, his needs and abilities. The results showed that combination of methods in therapy is beneficial for a positive influence of motoric skills of a child, improvement of motion in particular joints and for a positive mental condition of a patient This thesis might be used as a set of helpful information for physiotherapists in clinical practice, who deal with CCP, and also as a guide for parents of children with the diagnosis of children cerebral palsy.

Cross-Platform Applications

The aim of this thesis is cross-platform programming. After a general theoretical introduction to the cross-platform programming it gives attention to Mono Project. It gives the overview of available integrated development environments and other tools that can be used during development and it also mentions its limitations. The thesis then compares several available toolkits for creating the graphical user interface of applications. After that it presents a set of recommendations for cross-platform programmers in Mono.

We present a new class of self-adaptive higher-order finite element methods (hp-FEM) which are free of analytical error estimates and thus work equally well for virtually all PDE problems ranging from simple linear elliptic equations to complex time-dependent nonlinear multiphysics coupled problems. The methodology was used to solve various types of problems. In this paper we use a nonlinear combustion problem for illustration.

In this survey, we provide a discussion of the various dimensions of the relative position of women on labor markets and the techniques that have been used in the empirical economics literature to understand the sources of the observed gender differences. We also give a brief overview of the available data and analyses in the Czech Republic and present new gender wage gap decompositions for the Czech enterprise sector in 2004.

Biology and control of Matricaria recutita

Thesis deals with the biology of the Matricaria recutita, its usefulness, by extension, harm, and especially the regulation of its occurrence. The aim of this work is to contribute to improving knowledge of this species. The thesis presents a comprehensive overview of reproduction, importance, harm, relationship to cultural crops and control weeds with a focus on biology and control of Matricaria recutita. They proposed appropriate measures to control and use of plants in practice.

Safety assessment of Temelin NPP

History of peaceful use of nuclear energy, despite of all necessary emphasis on operation safety, is accompanied by a series of accidents, some of which left indelible trace in minds of wide public and so led to changes in the comprehension of nuclear safety and in the access to its preservation. Because the consequences of possible nuclear power plant accident have always serious social impacts, it is most desirable to define all possibly risks, to quantify probability of their formation and in this way to gain the possibility to prevent them effectively and efficiently. This work is dedicated to approve or to disapprove that the KARS method is practically applicable for evaluation of nuclear power station operation safety. The work objective is to evaluate events and risks associated with the operation of a particular nuclear facility by the KARS method and critically evaluate obtained results and the used method as such. The work is divided into theoretical and practical part. Theoretical part includes basic information necessary for correct understanding of the chosen data processing procedure and subsequent interpretation of the gained results. A brief overview of basic concepts relating to the nuclear safety, the safety culture and the emergency preparedness, including their definitions is given in this section of the work. For the evaluation of obtained results it is useful to make familiar with basic concepts of the NPP safety evaluation, PSA and the stress tests. Obviously, it is necessary to describe the KARS method and to define the concept of quality of human performance with regard to the obtained results. The risk analysis by the KARS method was carried out in the four consecutive steps: 1. Data collection for the risk analysis. 2. Data processing by the use of the KARS method. 3. Interpretation of the obtained results. 4. Comparison of the obtained results with the available data. The particular risks for the construction of the risk correlation table were based on the analysis of activities of individual members of the emergency and technical support center on the basis of managing documentation for each function. In the emergency instructions these risks are either named or the action to manage them are determined. Subsequently these risks are further more specify according to the operational documentation used for solving abnormal and emergency conditions or solving of severe accidents. Determination of the risk correlation seems to be subjective and requires a considerable degree of professional insight into the analyzed issues. The risk analysis was processed according to the procedure that was laid down by Ing. Stefan Pacinda, Ph.D. Each risk taken into consideration in this analysis is briefly described within the interpretation of obtained results. The coefficients of activity with the biggest value were found for these risks: human error, earthquake, plane crash, terrorist threat, errors in operating and managing documentation and fire. The coefficients of passivity with the biggest value were found for these risks: human failure, serious damage to health, ecological disaster, components malfunctions, radiation accident, nuclear material integrity damage and control system malfunctions. Comparison of the risks that were postulated in the stress tests and the risks that were considered in this analysis indicate that the ETE NPP emergency response was prepared to be able to cope these risks. Results of this analysis showed that the essential element for the safe operation of nuclear power plants is qualified, well selected and systematically trained staff. In light of the experience acquired during elaboration of this work I have concluded that the KARS method can be used for nuclear safety evaluation only in a limited way.

Possibilities and Limits of School Website Usage for Students and Teachers at Primary Schools

This thesis deals with the issue of primary school websites and the potentials of their usage by students and teachers. Recently, websites became very common means of communication and educational platform. The aim of the thesis is to determine the potentials and limits of using websites in primary education. The thesis is divided into three main chapters and it has four appendices. The introductory chapter introduces the aims and the concept of the thesis. The part dedicated to the current state of the problem describes the development of Internet usage by children in the Czech Republic and other countries. This chapter deals with statistic data from the survey carried by the Czech Statistical Office and from a National Educational Institute project Škola21, in which the leaderships of various schools answered questions about the ways of using the school website as a part of self-evaluation. In this chapter, there is also a division of the target groups of school web users, short overview that demonstrates the development of school website in the last ten years, and a description of the parts that it, in some cases, contains. Special part is dedicated to evaluation tools of school website and other resources that can help determine the criteria of a high quality website and thus its potentials and limits. The second chapter, which is the main part of the thesis, is a research of school website and Internet usage in general by students and teachers and presents the basic research questions that were being answered. The main research question that is later divided into sub-questions is "How do students and pedagogical employees use the Internet and school website?" The main tool for collecting the answers were questionnaires designed according to a pre-research. The questionnaires collected the answers about the usage of the school website from students and teachers. The answers are statistically elaborated and the results are being compared by classification of fist and second degree. Here, the ways of students' and teachers' work with the website are compared and relations that can designate the potentials of the school website are looked for. The answers were further compared according to the respondents' school location, frequency of the school website visits, Internet access and other criteria. The results of the research were further discussed in interviews with groups of students and teachers. These interviews were focused on discovering the reasons for the respondents' answers. The thesis summarizes the results of the research and puts them into context with previously published work on this topic. Using these findings, the third chapter defines and explains recommended content of a school website and thus, sets the basic limits and potentials of its usage. On the basis of the information from the overview of current studies on this topic and using the research data, the thesis suggests a school website evaluation tool, which can answer the question about how the potentials of a school website are utilized. Computational algorithms and procedures of creating the evaluation tool are described in the research part of the thesis as well as the results of its testing on two primary schools in the Czech Republic.

Rights of insured persons and patients by legislation

Even though the health care services in the Czech Republic are of high international standards and accessible to majority of the population, some drawbacks may be noticed in terms of the approach of health care providers to patients and patient rights. Until 2011, the Act No. 20/1966 Coll., on Public Health Care, had regulated the health care provision and patient rights in a complex way. Gradually, it became clear that the main part of regulations did not consider the patient-specific needs adequately. Legislators' efforts to bring the patient rights in conformity with the principles of the Constitution, Charter of Fundamental Rights and Freedoms, and the Convention of Human Rights and Biomedicine have led to the Act No. 372/2011 Coll., on Healthcare Services, which has replaced the Act on Public Health. The new foundation of patient rights guarantees that the failure to fulfil may be rectified as only the rights defined by law can be effectively enforced. The prerequisite for a patient to exercise and demand his/her rights to be fulfilled is their knowledge. This dissertation aims to provide a concise overview of the progression of patient rights and their origins within the international and national context, to outline some rights of an insured person and a health care user more in-depth, to introduce the protection of patient rights, and to verify the knowledge of selected rights among the patients in the South Bohemian region. Based on my own research which took place in outpatient and inpatient facilities in the South Bohemian region by the questionnaire form I wanted to find how are patiens informed about theirs rights and whether they are actively interested in them. I chose two hypothesis. The first hypothesis was verified claims than the patiens over the age of 50 have more knowlidges about their rights than the patiens to the age of 50. From the results of statistical data exploration it shows implies that the patiens over the age of 50 have really more knowlidges than the patiens of the second age category, therefore the hypothesis was confirmed. The second hypothesis should confirm whether the patiens are passive at getting informations about the patiens rights. The survey shows that the patiens are active and they are interested in finding out informations. The second hypothesis didn´t confirmed. My purpose wasn´t describe in detail all patient rights. I wanted compile basic patient rights and rights of an insured person and a health care user which can be used to more patient know-how, for use of health professionals utilization or to teaching students of the Social and health faculty of South Bohemian University.
