Awareness and Education of Parents within Postoperative Care for a child with Inserted Ventriculoperitoneal Shunt

This bachelor's thesis focuses on the topic of parental awareness and education regarding postoperative care for a child with an established ventriculoperitoneal shunt. The implementation of this shunt is a common surgical intervention to treat hydrocephalus. Objective: The aim of the thesis was to provide a comprehensive overview of relevant knowledge concerning the care of a child with a ventriculoperitoneal shunt. Another objective was to determine how parents of children with the shunt subjectively evaluate their own level of information, and how they perceive the approach of the medical staff, including mutual cooperation. It also sought to identify possible improvements in educational materials related to the issue. The method chosen for processing this work was quantitative research through a non- standardized questionnaire containing 29 questions. The questionnaires were distributed among parents of children hospitalized in the pediatric section of the neurosurgical clinic for children and adults at the 2nd Faculty of Medicine, Charles University, and Motol University Hospital. Nursing process in a patient with hydrocephalus

The present bachelor thesis is devoted to the nursing process and education in a patient with hydrocephalus. The aim of the thesis was to determine to what extent the disease affects the patient's quality of life, focusing on the period before and after the insertion of a ventriculo-peritoneal shunt. Furthermore, how the patient felt during the hospitalization, whether he was satisfied with the attitude of the staff, whether he was properly educated before and after surgery or before discharge to home care. The theoretical part of the thesis is devoted to a detailed description of the disease, its causes in neonatal, childhood and adulthood, symptoms of patients of all ages and treatment options. The empirical part of the thesis is based on quantitative research investigation, analysis of the obtained data and comparison of the results of the thesis with similar works. The research method chosen was a quantitative survey using a non-standardized questionnaire. The questionnaire survey was carried out at the Neurosurgical Clinic of Motol University Hospital and the 2nd Medical Faculty of Charles University, after prior approval of the deputy for nursing care and with the consent of the management of the clinic. The incidence of postoperative infectious complications in neurosurgical ICU

This thesis examines in detail the nature of postoperative infectious complications in patients who underwent brain tumor surgery. It deals with the incidence of these complications and their severity, depending on various factors and suggests the most appropriate nursing procedures used in the care of a patient in whom these complications occurred. For the structure of the work is divided into two parts - theoretical and empirical. The theoretical part deals with the issues examined in terms of the medical field Neurosurgery (closer diagnoses, conditions, operations and other procedures, and complications to occur after these operations or expected performance), analyzes a nursing practice and patient care on neurosurgical ICU, and finally closely examines infections and nosocomial diseases, including a description of the most common types, their agents, diagnosis and treatment. The second, empirical part focuses on the problems from a practical point of view. Pineal lesions: clinical presentation, hormone secretion, sleep quality and effect of surgical treatment

Introduction: Pineal region is a deep-seated part of the brain surrounded by highly eloquent structures. Differential diagnosis of space-occupying lesions in this region encompasses pineal gland cysts, pineal gland tumours, metastases, germ cell tumours, meningiomas, gliomas, hemangioblastomas and neuroectodermal tumours. In this thesis, I focused mainly on patients with pineal cysts, which is a benign affection of the human pineal gland on the borderline between pathology and normality. The clinical management of patients with a pineal cyst remains controversial, especially when patients present with non-specific symptoms. A melatonin secretion in patients with a pineal cyst before and after a pineal cyst resection has not been studied yet and the effect of surgery on human metabolism is unknown. Materials and Methods: We performed a prospective study between 2000 and 2016. All patients with a pineal cyst larger than 7 mm were included. Epidemiological data, presenting symptoms, surgical results and radiographic and clinical follow-up were documented. Rehabilitation of Pediatric Patients after Epilepsy Surgery with Postoperative Motor Deficits.

In its general section, the thesis 'Rehabilitation of Pediatric Patients after Epilepsy Surgery with Postoperative Motor Deficits' discusses epilepsy, treatment of intractable epilepsy and epilepsy surgery. Furthermore, a chapter on possible complications of epilepsy surgery in relation to the locomotor apparatus is included in the general section. The special section deals with rehabilitation in neurology, selected physiotherapy concepts and the relationship between epilepsy and movement activity. The aim of this thesis is to assess the effectiveness of the chosen rehabilitation procedure in five pediatric patients with post-operative motor deficiency. The Gross Motor Function Measure - 88, the Barthel Index and the Box and Blocks Test were used for objective motor assessment.
Rehabilitation of Pediatric Patients After Neurosurgical Removal of Epileptogenic Zone

In its theoretical part this bachelors thesis "Rehabilitation of Pediatric Patients after Neurosurgical Removal of Epileptogenic Zone" concludes knowledge about epilepsy and its treatment, epilepsy surgery, medical rehabilitation used for neurological diseases and relation between epilepsy and sport. The research of literature concerning these topics was done. The second part of thesis consists of case report, which summarizes the process of postoperative rehabilitation in pediatric patient after hemispherectomy done at the Department of Neurosurgery, 2nd Faculty of Medicine and Motol University Hospital. Thesis evaluates the effect of rehabilitation procedures after the resection of the epileptogenic zone. Tests used for objective evaluation of motor functions were Gross Motor Function Measure - 88, Barthel Index and Box and Blocks Test.

Case study of physiotherapeutic care of a patient with the diagnosis of a postoperative left-sided hemiparesis

This bachelor thesis consists of two parts: theoretical and practical. The first part describes basic anatomy of the ventricular system and the blood supply of the brain. Furthermore it deals with theoretical knowledge of cerebral vascular malformations, its types and possible treatment. The main subject of this part defines upper motor neurone syndrome and regarding rehabilitation methods. Theoretical part of the thesis is based on information from professional literature. The practical part follows up a case study of a patient with postoperative left-sided hemiparesis developed after resection of cavernous malformation of right ventricle trigone. This part contains a methodology of the work, input data and kinesiology analysis, short-term and long-term physiotherapeutical plan and then a description of specific therapeutic procedures. In the end there is a comparison with input and output kinesiology analysis data and the evaluation of possible therapeutical effect. All of the data needed for this part of the thesis were gained during a continous practical session at Rehabilitation Centre in Beroun from 18th January to 12th February 2016.

Comprehensive nursing care for a patient after a neurosurgical operation in selected diseases

Theoretical approaches: Neurosurgery is a wide and sophisticated field and given the variety of neurological issues, this thesis focuses on the brain (vascular issues and tumors). Goals: This thesis has two goals. 1. To single out the specifics of caring for patients after brain surgery (of selected conditions). 2. To find out, whether the nurses know the specifics of caring for a patient after brain surgery. Hypotheses and research questions: H1: Nurses with tertiary education have greater theoretical knowledge of caring for patients after brain surgery than the nurses without tertiary education. H2: Nurses after getting the job of caring for patients after brain surgery didn't have an instructor.H3: Nurses after getting the job of caring for patients after brain surgery, most commonly struggle with an external ventricular drainage system. H4: Nurses most commonly gain the required knowledge to care for patients after brain surgery from literature. Research question 1: What is the level of knowledge of novice nurses about the specifics of caring for a patient with a brain condition? Methodology: The research was split in two parts. For the quantitative part of the research, the questionnaire method was selected. The questionnaire had 22 questions, 17 of them closed, 4 of them semi-open, 1 was an open question. The researchers handed out 300 questionnaires and received 246 (82%) of them back. 49 questionnaires had to be discarded as invalid. The final research sample consisted of 197 (100%) filled-out questionnaires. For the qualitative part of the research, the method of semi-structured interviews with open questions was selected. There were a total of 8 interviews conducted. Results: The goal of quantitative part of the research was to confirm or disprove the 4 hypotheses. H1: Nurses with tertiary education have greater theoretical knowledge of caring for patients after brain surgery than the nurses without tertiary education. Based on the data, we can see that the knowledge of nurses with tertiary education is not different from the knowledge of nurses without tertiary education. H1 was disproved. H2: Nurses after getting the job of caring for patients after brain surgery didn't have an instructor. Based on the questionnaire data, nurses did have an instructor. H2 was disproved. H3: Nurses after getting the job of caring for patients after brain surgery, most commonly struggle with an external ventricular drainage system. Based on the research data, an external ventricular drainage system is indeed the most commonly quoted problematic procedure when caring for a patient after brain surgery. H3 was confirmed. H4: Nurses most commonly gain the required knowledge to care for patients after brain surgery from literature. The research shows that this is not the case. Nurses most commonly learn the specifics of caring for patients after brain surgery from their instructors. H4 was disproved. For the qualitative part of the research, the method of semi-structured interviews with open questions was selected. The research question was: What is the level of knowledge of novice nurses about the specifics of caring for a patient with a brain condition? The interviews have shown that the knowledge of novice nurses about the specifics of caring for patients with a brain condition is on an appropriate level they know the specifics of caring for patients after brain surgery. Conclusion:The conducted original research as well as secondary literature has led us to several questions and recommendations: Is the nurse who has just started at the neurosurgery division properly being instructed about the specifics of her new job and is the adaptation process being conducted in the proper fashion? The results of this thesis suggest that a "Caring for patients in neurosurgery" manuscript could be written for general care nurses just starting at neurosurgery. This option was mentioned by some of the respondents in the questionnaire research
