The intervention of occupational therapy in patients with impaired hand function after surgery

This bachelor's thesis deals with the surgical treatment of patients with hand impaired function and subsequent intervention of occupational therapy, and the patients who underwent this treatment. Thesis is divided into two parts-theoretical and practical. The theoretical part describes the function of the hand and the possibilities for its disorders. Subsequently, thesis describes the principles of surgical treatment of various diseases and hand injuries that impair its function. Another part relates to intervention of occupational therapist in patients who underwent surgical treatment of hand. The practical part contains two casuistries, methodology and processed results and discussion. The first casuistry deals with patient with carpal tunnel syndrome solved by surgery. The second casuistry deals with a patient with finger's flexor injury on the left upper limp. During processing has been found that occupational therapy is integral to rehabilitation after hand surgery. Occupational therapy helps to improve the function of the hand grip and the coordination of movement and it is important for gaining the greatest possible self- sufficiency of patients after surgical treatment of hand. Use of the Modified Frenchay scale in stroke patients with spastic paresis

The objective of this thesis is to describe the Modified Frenchay scale and its use in stroke patients with spastic paresis. The thesis is divided into two parts - theoretical and practical. In the theoretical part, the issue of spastic paresis in stroke patients is specified; and in addition to this the methods of examination and assessment of spasticity are presented and described in detail. A large part of the thesis deals with the history of Frenchay arm test and its modification - Modified Frenchay scale. The theoretical part also contains a description of the phases and types of grasp, some of which can be observed during the assessment with Modified Frenchay scale. The practical part consists of a questionnaire mapping the situation in the institutions in the Czech republic, that use the Modified Frenchay scale in their practice, and of two case reports of patients after stroke. Examinations and evaluations of spasticity, including Modified Frenchay scale, which are described in the theoretical part, are applied in these case reports. The practical part also contains an evaluation of inter-rater reliability of the Modified Frenchay scale. Grasp evaluation in patients after stroke

Title of diploma thesis: Grasps evaluation in patients after a stroke Objective: The main goal of the pilot study was to determine minimal clinically important difference in selected tests focusing on improvement in the grasps of patients in the chronic phase post stroke. The main hypothesis was set to prove that the differences between the initial and final assessments based on the Modified Frenchay Scale characterizes patients' improvement more accurately than the Score for visual evaluation of functional task of the hand. Methods: The research group consisted of 40 participants (26 men and 14 women) all in the post stroke chronic phase who were selected based on the criteria for inclusion in the research. Data was collected at the Department of Rehabilitation Medicine and the First Faculty of Medicine. Using video documentation of the Modified Frenchay Scale used during both the initial and final assessment, the test itself was then analysed. A 3rd subtest was also used to evaluate the Score for visual evaluation of functional task of the hand. The Bachelor thesis is focused on description of individual points of the 0-10 scale of the modified version of the Frenchay arm test in patients after cerebrovascular stroke (CVA). The thesis is divided into two parts - theoretical and practical. The theoretical part describes the issue of CVA in detail. It includes statistics, types of strokes and clinical symptoms. Further, in the theoretical part, tests for upper limb examination are mentioned. The Modified Frenchay Scale and the Frenchay Arm Test are described in more detail, two other tests are mentioned only marginally. The theoretical part also mentions the types and phases of grip that we can observe in the Modified Frenchay Scale and the list of common daily activities. The practical part of the bachelor thesis contains three case reports of patients after CVA. Patients in the bachelor thesis represent the acute, subacute and chronic phase of CVA. The case reports include descriptions of each task of the Modified Frenchay Scale and their scoring on the scale 0-10. Attachments include a manual with suggested descriptions for the individual points of the 0-10 scale of the modified version of the Frenchay arm test.

Use of Canistherapy for Stimulation of Persons with Rare Diseases

The aim of this thesis was to assess the influence of canistherapeutic intervention on the level of fine motor skills in patients with rare diseases, namely mitochondrial encephalocardiomyopathy caused by TMEM70 gene mutation and Leigh syndrome. Canistherapeutic intervention in these patients is based on activation of motoric functions during programme reasonably designed according to the needs of specific patients. The assistance of the dog is used to achieve this activation. The dog acts as an important motivational factor and the direct contact with it contributes to development of sensomotoric skills of these patients. The thesis summarizes general information about rare diseases and focuses on one of the subgroups of mitochondrial disorders. It presents knowledge about the Leigh syndrome and mitochondrial encephalocardiomyopathy caused by TMEM70 gene mutation. Subsequently the thesis presents information about canistherapy and fine motor skills. The thesis further contains case reports of patients, data from research units and their evaluation. The aim of the thesis was to find out whether patients with mitochondrial disease show improvement of quality and effectivity of selected grasp forms and thus faster and more precise execution assigned tasks focusing on object manipulations.
