Education of diabetic patient with diabetic food after amputation

This bachelor thesis is focused on education of patients with diabetic foot syndrome with limb shortening amputation and prevention of higher amputation. The theoretical part deals with the disease diabetes mellitus, its complications, diabetic foot syndrome and education on prevention of amputation. This disease is currently prevalent and its incidence is increasing. The aim of the study was to find out who and by what method most often educates patients with diabetic foot syndrome about prevention of amputation, in what way patients perceive shortcomings on the part of education and to map their knowledge about prevention of diabetic foot. A quantitative method of investigation was used for this study and data was collected using a self-administered questionnaire which was filled by a total of 30 respondents. The research sample consisted of 30 respondents (100 %). The results show that 24 people (80 %) are educated about amputation prevention by both a nurse and a doctor and 6 people (20 %) by a doctor only. The form of education was oral and written for 21 respondents (70 %) and oral only, in the form of an interview for 9 people (30 %). The biggest barriers to education were lack of time for 13 respondents (43 %), inappropriate environment for 9 respondents (30 %), lack of self-will to listen to the educator for 7 people (23 %), and lack of empathy from the educator for 3 people (10 %). 10 respondents (33 %) described their knowledge of prevention as good, 19 respondents (63 %) described their knowledge as normal and 1 respondent (3 %) described their knowledge as poor. The contribution of this bachelor's thesis can be pointing out the shortcomings in education and trying to eliminate them. This thesis contains an overview of recommendations that should be followed by patients with diabetic foot syndrome, so it can also serve as an educational material for nurses and diabetic patients.
Software image processing of infrared thermograms of the lower limbs of diabetic patients

This work deals with the automatic processing of the thermal images of the soles of the lower limbs. It contains general research in the topic of diabetic foot syndrome and general description of diabetes mellitus. It also deals with the theoretical area of the infrared radiation used in thermovision techniques and image processing techniques. In the second part, there is practically described the proposition of the software and then also its realization and commissioning. Created software contains two main possibilities for cropping the searching sole, the first one is completely manual detection and the second one is automatic with manual correction. In this work, there are also described all the procedures for reaching of the temperature analyzes. Last part of this work is dedicated to statical valorization of the information, which was got by the created software and which is conceived generally and serves to functional valorization of the created software.
Case report of orthotic and prosthetic care of a patient with a diagnosis of transtibial amputation of the tibia

vi Abstract Title: A case report of orthotic and prosthetic care of a patient with a diagnosis of transtibial amputation of the tibia Aims: The aim of this bachelor thesis is to discuss in theoretical terms the individual aspects of the case report of a patient that led to transtibial amputation of the right leg. In the practical part, a description of the production of the primary equipment is presented. Methods: In the theoretical part, the professional framework of the problem is presented based on a literature search, knowledge from professional seminars and my practical experience. The practical part describes the course of the production of the first equipment, from the collection of the measuring materials, through the production of the stump bed, tests and adjustments, to the handover of the aid. This is followed by the experiment of fitting an individual with a calceotic aid to complement the compensatory aid. The practical part is documented verbally and photographically as it took place during my professional practice. Results: Too early production of a provisional bed cannot reflect the rapid volumetric changes of the stump and leads to the need to produce a new bed in a very short time frame. Mikrocirculation in the diabetic foot syndrome

Abtrakt (EN) In spite of the fact that the diabetic foot syndrome is not considered a marginal complication of the patients with diabetes mellitus any more, its prevalence and incidence is not decreasing. The fact that early diagnosis and treatment or even prevention of foot ulcers is associated with a decrease in healthcare costs and, more importantly, with reducing patients' suffering, applies also in this case. The dissertation work originated in the Diabetologic Center of the Medical department I., University Hospital and Medical Faculty in Pilsen, Charles University in Prague, which has been dealing with diagnostic and therapeutic procedures in patients with diabetic foot syndrome for over 20 years. The examinations for diagnosis of the main etiologic factors, i.e. peripheral neuropathy and impaired perfusion, belong to already routine ordinary procedures here. My task was to move the diagnostics of this complication to earlier stages, even before the development of the ulcers, through diagnosing an impairment in skin microcirculation. Title: Case study of ortho-prosthetic care of a patient with a diagnosis of Charcot osteoarthropathy. Objectives: The first aim of this bachelor's thesis is to summarize theoretical knowledge about the issue of diabetes and its imminent risks for the patient and the possibility of using ortho- prosthetic aids. The second aim of the work is to process a case study of ortho-prosthetic care for a patient diagnosed with Charcot's osteoarthropathy. Methods: The bachelor thesis has two parts - theoretical and practical. The theoretical part deals with the issue of diabetes mellitus and amputation of the lower limbs, as an imminent risk of this disease and prevention of amputations. The practical part consists of a case study of orthotic-prosthetic care for a patient with Charcot's osteoarthropathy who also suffers from Diabetes type 2. Title: Case Study of Physiotherapeutic Treatement of a Patient with the Diagnosis of Unilateral Above - Knee Amputation. Objectives: The aim of this work is a summary of theoretical knowledge about amputations of lower limbs and processing of case study of patient with the diagnosis of unilateral above - knee amputation. Methods: This bachelor thesis consist of two parts. The general part includes the literature review focused on the issue of amputations of lower limbs in relation to diabetes mellitus and its therapeutics and prosthetic options. Special part consists of case study of physiotherapeutic treatment of a patient with the diagnosis of unilateral above - knee amputation. This part describes anamnesis, complex kinesiological examination, the course of each therapeutic unit and a final evaluation of therapeutic intervention. Results: This case study was created during two weeks at the turn of January and February 2019 in Ústřední vojenská nemocnice Praha. Effect of therapeutic physical program of patients with 2nd type diabetes mellitus

Title: Effect of therapeutic physical program in patients with type 2 diabetes mellitus Objectives: The aim of my work was to examine the effect of 12 weeks therapeutic physical program on distribution of plantar pressure, muscle strength in plantar flexion, ankle range of motion and physical abilitity by patients with type 2 diabetes mellitus and diabetic foot syndrome. Methods: It was a quantitative research, randomized controlled trials. It was chosen 10 patients with type 2 diabetes mellitus and with diabetic foot syndrome. The patients were devided into 2 groups. In each group were four males and one female, aged from 54 to 73 years old. The first group included exercise patients and the second group nonexercise patients. The intervention took place 5 - 7 times a week and contained two types of training units. The first had 11 exercises, the second self mobilization of leg joint and walking. Basic anthropometric figures - height, weight, girth of waist and hipline (tailoring meter,weighing machine), ankle range of motion (goniometer), muscle strenght in plantar flexion (dynamometer), pressure distribution in stand (MobileMat), physical ability (Senior Fitness Test) were measured and compared in two groups after 12 weeks. It also contains summary of basic information about amputations, rehabilitation after lower limb amputation and prosthetic possibilities. The second part is practical and it is a detailed case report of physiotherapy treatment of patient after transfemoral amputation. It contains detailed complex kinesiology examination, itemization of individual therapeutic units and final evaluation of therapy effect.
