A Case Study of Physiotherapy Treatment on a Patient After LiverTransplantation

Název: Případová studie fyzioterapeutické léčby pacienta po transplantaci jater Autor: Saria Al Saseh Cíl: Cílem této práce je podrobně zkoumat teoretické aspekty související s postupy transplantace jater a s nimi spojeným rehabilitačním procesem. Druhá část zahrnuje podrobnou analýzu případu, která popisuje fyzioterapii, která byla podána pacientovi v raném pooperačním období po transplantaci jater. Klinické zjištění: Případová studie je založena na 43letém pacientovi po transplantaci jater kvůli cirhóze způsobené Wilsonovou chorobou. Vyšetření ukázalo snížení nezávislosti, svalové síly, rozsahu pohybu, změn ve vzorci dýchání a měkkých tkání. Metody: Metoda použitá v této případové studii je založena na terapeutických technikách naučených na FTVS Univerzity Karlovy. Výsledky: Pacient během rehabilitačního procesu prokázal významné zlepšení. Tento pokrok umožnil pacientovi dosáhnout nezávislosti a zlepšit kvalitu života. 1 Abstract Author: Zuzana Charvátová Title: Case study of physiotherapy treatment of a patient with Wilson disease Objectives: Aim of the theoretical part is to summarize recent knowledge about the Wilson's disease and its possible physiotherapeutic impact. Aim of the practical part is to present the case study of the patient, who I met during my bachelor's practice in Rehabilitation Hospital of Beroun. Methods: Data for the theoretical part was obtained by systemic research of Czech and international bibliography available both in printed and electronic form. Presented case study was compiled using examining and therapeutic methods and techniques which are taught during the physiotherapy Bc. programme at FTVS UK. Results: Comparison of initial and final kinesiological examination revealed, that the physiotherapeutic treatment described in the practical part of the study contributed to the improvement of patient's condition. Author: Jana Hrmova Title: Case study of physiotherapy treatment of a patient after liver transplantation Objectives: The main aim of this thesis is to summarise the theoretical knowledge about liver transplants, with the use of evidence from a patient that had undergone a liver transplant. The thesis is written from a physiotherapeutical point of view. Methods: The thesis consists of a theoretical part and a practical part. In the general part, theoretical information about liver diseases, Wilson's disease, liver transplant and physiotherapeutic treatment are discussed. All of this information is processed based on an academic literature and other peer reviewed sources. The practical part consists of a case study with a patient after undergoing a liver transplant, and a report of their rehabilitation, which was conducted as a part of a framework of a continuous physiotherapeutic internship at the Institute of clinical and experimental medicine in Prague from 2nd to 27th January 2017. Results: The patient was in a coma during the course of treatment which significantly slowed down the progress of the therapies. Chelating polymers for the therapy of Wilson's disease

Wilson's disease is a hereditary disorder of copper metabolism, which causes copper accumulation in organism, especially in the liver, kidneys and brain. Current treatment is based on using low-molecular weight copper chelators and high doses of zinc salts. Unfortunately, they can induce some severe side effects due to systemic action. The aim of this thesis is to improve the treatment of Wilson's disease by using of polymeric drug delivery systems. The size of polymer particles in tens of microns should provide non-resorbability of the drug after oral administration. Synthetic microparticles of poly(glycidyl methacrylate-co- ethylene dimethacrylate), natural microcrystalline cellulose and cross-linked chitosan were used as polymer matrices. N,N-di(2-pyridylmethyl)amine, triethylenetetraamine and 8-hydroxyquinoline were selected as specific copper chelators, which can complex copper cations with high efficiency. The principle of the proposed treatment is that the polymeric carrier-bound chelator complex copper directly from the food in digestive tract of the organism. Because of non-resorbability, the entire complex should be eliminated from the body together with stools. This virtually eliminates systemic side effects. There is described a electromigration separation method for the analysis of low concentrations of copper with the possibility to detect a Wilson disease in my thesis. There were developed a methodology and propitious electrolyte systems, which can pre-concentrate copper prior to its on-line analysis. The developed electrolyte system for analysis had the following composition: LE1 (1.10-2 M CH3COOH, 2.10-2 M CH3COONH4, 2.10-4 M SPADNS), LE2 (1.10-2 M CH3COONH4, 1.10-2 M NH4OH, 1.10-4 M BKP, 5.10-5 M PAR, 5.10-3 M C6H17N3O7) and DE (7.10-5 M Cu(CH3COO)2, 1.10-3 M CH3COONH4 a 1.10-2 M CH3COOH). With a suggested electrolyte was achieved almost seventyfold accumulation of copper during 42 minutes, which lowered the concentration detection limit on the level convenient for the detection of Wilson disease. Complete analysis took about 98 minutes. There was accomplished a conductometric detection of copper in my thesis, however the developed electrolyte system can also analyze a copper by photometric detection, which is more sensitive.
Education Programmes for Wilson´s Disease Patients

The Bachelor thesis called ``Education Programmes for Wilson´s Disease Patients`` describes the disease itself, its cause, education programmes provided by nurses, the methods and functions of the education programmes, the impact of the disease on patient´s everyday life and significant changes in his lifestyle as a whole. The thesis has two aims. 1. To find out if educated patients follow the diet recommended to them. 2. To find out how much the Wilson´s disease affects patients´everyday lives. Two hypotheses were stated: The patients are supposed to follow the diet recommmended. The patients´ lifestyle is changed while keeping the therapeutic regime. The hypotheses were verified by a quantitative research technique. The data were collected by a questionnaire which was responded by adult patients of professional departments, 4th Department of Medicine - Department of Gastroenterology and Hepatology, First Faculty of Medicine, Charles University in Prague and General University Hospital in Prague, Department of Neurology, First Faculty of Medicine, Charles University in Prague and General University Hospital in Praque and Inpatient and Outpatient Departments in Strakonice. The survey has shown that educated patients follow the diet recommended and the therapeutic regime affects significantly their everyday lives. The hypotheses stated at the beginning were proved. One interesting fact was that the neurology outpatients believe more than the others that their lifestyles are affected by the therapeutic regime. On the other hand most of them think their lives do not differ much from healthy population. The findings of the thesis can improve educational programmes for these patients and their families provided by nurses.
