Validity of predictive equations for determining resting energy expenditure

This bachelor thesis deals with the validation of predictive equations to determine resting metabolic rate. The objective of this bachelor thesis was to compare individual predictive equations for determining resting metabolic rate with self-measurement using indirect caloriemetry and to establish a own procedure for predicting resting metabolic rate. The evaluation of each predictive equation was based on statistical analyses. Based on statistical analyses, it was found that the available predictive equations underestimate resting metabolism by an average of 20 % of kilocalories per day compared to the value measured by indirect calorimetry. Machine learning was used to determine the actual procedure for predicting resting metabolic rate, which was then presented using the user interface. The following testing showed that the neural network for predicting resting metabolic rate provides more accurate results compared to the available predictive equations.
Resting energy expenditure and nutritional intake in patients with pancreatic cancer before elective pancreatectomy

Aims: The aim of this work was to determine whether a increased resting energy expenditure contributes to the nutritional risk for patients with pancreatic cancer. What the variability of resting energy expenditure is and whether it predicts weight loss. Another aim was to verify that lower protein-energy intake also has an impact on weight loss. Methods: Data for the diploma thesis were obtained from the project "Pancreatic Cancer: Metabolic Derangements Associated With Insulin Resistance", which takes place at the Department of Clinical Physiology of Metabolism at 2nd Department of internal medicine in the University Hospital Královské Vinohrady. The project includes 40 - 50 patients with pancreatic tumor resection per year. Data were collected from the beginning of December 2019 until the end of April 2020. The project is funded by the grant AZV NV19-01-00101. 10 consecutive patients (4 women and 6 men) with pancreatic cancer who had a complete dataset for analysis were selected for the research set for the diploma thesis. Nutritional Status in Patients with Huntington's Disease and Nutritional Support

Huntington's disease is a dominantly-inherited autosomal neurodegenerative disease manifested by disorders of motility, cognitive function, behaviour, and weight loss, which is conditioned multifactorially. The aim of the study was to determine whether there are eating disorders in Huntington's disease, as well as its etiology and severity. Neurological scaling, anthropometric examinations, evaluation of three-day diet records, measurements with a manual dynamometer, bioimpedance analyses, indirect colorimetry and predictions of energy expenditure were performed on 10 patients. Algorithms were applied for the diagnosis of sarcopenia and malnutrition. Unwanted weight loss was observed in all patients and 4 out of 10 showed malnutrition. No difference was found between the values of measured resting metabolism and calculated according to the predictive equation. However, it has been shown that strict nutritional recommendations based on this data can be misleading for some patients with HN, as real energy consumption can be significantly higher. Use of indirect calorimetry in nutritional therapy

Introduction: Indirect calorimetry (IC) is a very accurate and non-invasive method which is used in clinical practice to measure the resting metabolic rate. This method is based on the assumption that oxygen consumption, carbon dioxide production and nitrogen metabolite waste are in definite relation to energy consumption. Based on the measurement of resting metabolic rate, a nutritional therapist can individually determine daily energy intake of a respective individual, the optimal amount of specific nutrients and thus prevents excessive or inadequate intake of energy and nutrients. Objective: The main aim of this thesis is to compare for group of overweight patients the results of the measurement of the resting metabolic rate (RMR), values of the resting metabolic rate calculated according to the Harris-Benedict (HB) equation and the actual energy intake calculated from the diet report obtained from the patients. In addition, the body composition of the examined group with a focus on the amount of muscle tissue is also marginally evaluated. Changes in resting energy expenditure during pregnancy.

Background The aim of this work was to determine changes in resting energy expenditure in three periods of pregnancy using indirect calorimetry and assess correlations between the obtained resting energy expenditure values and anthropometric parameters. Methods To determine resting energy expenditure was used indirect calorimetry, which measured oxygen consumption and carbon dioxide production, from which this value was calculated. Measurements were performed in seven healthy pregnant women. Measurements took place over three periods. The first examination took place from the 17th to the 27th week of pregnancy, the second from the 28th to the 35th week of pregnancy and the third from the 36th to the 38th week of pregnancy. Results We found that resting energy expenditure values increased with pregnancy length (REE1 = 1670 ± 148 kcal/day; REE2 = 1717 ± 230 kcal/day; REE3 = 1984 ± 209 kcal/day). Also, correlations have been found with age, body weight, fat free mass and fat mass, body surface area, body mass index, and other parameters. Resting energy expenditure during lacatation.

Background The aim of diploma thesis was to evaluate resting energy expenditure (REE) and substrate oxidation in Czech lactating women and their relation to measured anthropometric parameters. Methods The indirect calorimetry was used to asses REE. Subsequently, the oxidation of substrates was calculated. At the same time were measured anthropometric parameters of breastfeeding women. Then the correlation was investigated. The total amount of examination were three times. First was in the time three weeks after birth, the second one was three months postpartum and the last was six months after birth. Results The mean values of REE in lactating women were 1577±93 kcal/day in the time of three weeks after birth. Three months after birth it was 1622±140 kcal/day. Six months after birth it was 1545±80 kcal/day. Significant positive correlation was proved between REE and triceps skinfold thickness (r = 0,98; P < 0,05) and also chest circumference (r = 0,99; P < 0,05). Both were three weeks after birth. Breastfeeding women had a value of lipid oxidation 60 % of substrate oxidation three weeks postpartum, which decreased to 46 % three months postpartum due to sharp increase (from 8 % to 18 %) of carbohydrate oxidation value. Indirect calorimetry in patients with home parenteral nutrition

Introduction: Establishing the energy expenditure of patients with home parenteral nutrition (HPN) is a key component of nutritional care, just as important as being aware of how HPN affects energy expenditure. This helps prevent complications associated with excessive or insufficient nutrition. Certain studies have aimed at these issues have been carried out around the world, but little attention has been devoted to this subject in the Czech Republic as of yet. Several methods of ascertaining energy expenditure exist, with indirect calorimetry being considered the gold standard, however, in practice, various predictive equations are predominantly used instead. Aims: To compare the difference between the base metabolic rate (BMR) and the resting energy expenditure (REE) in patients with home parenteral nutrition. When measuring REE, to also establish the extent of the thermic effect of parenteral nutrition. Assessment of the Resting Energy Expenditure in Obese Individuals

Introduction: Weight loss programs are based on inducing a negative energy balance by decreasing energy intake while increasing energy expenditure. Energy needs of obese patients are difficult to determine. The value of resting energy expenditure, as the largest component of the energy needs, is usually calculated to asses an adequate energy intake for obese patient. In clinical practice, predictive equations are most commonly used for this purpose, because of their simplicity, although many clinical studies have shown that in obese individuals they may generate errors large enough to impact the outcome of treatment. It leads to the prescription of an inadequate diet that does not meet the criterion of long-term sustainability. Objective: The aim of the thesis is to compare the values of resting energy expenditure (REE) measured by indirect calorimetry with values calculated with Harris-Benedict predictive equation in a selected group of obese individuals. Contributes to its overall severity are extrapulmonary manifestations, especially cachexia and loss of lean tissue mass. Chronic inflammation of the respiratory airways and increased respiratory effort cause the hypermetabolic state to some patients with COPD. This thesis investigates the impact of increased metabolism on body composition in patients with COPD. Our study included 50 COPD patients (38 men, 12 women) who were examined by bioelectrical impedance. In this study, we compared the parameters of body composition of men with resting energy expenditure REE > 130% (hereinafter Men over 130%) (n = 9) and the group of men with REE < 130 % (hereinafter Men below 130 %) (n = 29).
