The awareness of health professionals in the field of gynecology about prevention of cervical cancer

The bachelor thesis deals with the topic of the awareness of health professionals in the field of gynecology about the prevention of cervical carcinoma. The thesis summarizes basic information about primary and secondary prevention, risk factors and clinical symptoms of cervical carcinoma. It also includes information on the diagnosis and subsequent treatment of patients with cervical carcinoma. The main aim of the study was to identify which group of health professionals would have the greatest knowledge of cervical cancer prevention. A total of four research hypotheses were established, of which three were confirmed and only one was disproved. A quantitative type of research was chosen to address them. Data collection was carried out by means of a questionnaire survey conducted at the Obstetrics and Gynecology of the First Faculty of Medicine and General Teaching Hospital in Prague. A questionnaire of twenty-six questions was distributed in paper form among local doctors, midwives and nurses. The final size of the research sample was one hundred respondents. Is women's vaccination against HPV in the Czech Republic cost-effective? Bachelor's Thesis Author of the Thesis: Martina Lušková Abstract This thesis approaches the cost-effectiveness of women's vaccination against human papillomaviruses (HPV) in the Czech Republic. HPV is a pathogen responsible for the majority of diagnosed cervical carcinomas. The aim is to assess the current reimbursement setting of HPV vaccination compared to the designed change. In order to increase the vaccination coverage and subsequently decrease the future treatment costs, two strategies were proposed. A homogenous multistate Markov model is developed to model the transition among states representing the stages of progression of the cervical carcinoma. Transition analysis is performed based on the data collected for the purpose of healthcare reimbursement under public health insurance. The incremental cost-effectiveness ratio suggests that increased immunization coverage (from 65.8% to 80%) fuelled by the campaign promoting vaccination seems to be cost-effective assuming the threshold of 1.2 million CZK per one quality-adjusted life year. Risks and limits of laparoscopy in the treatment of gynecological cancers

The thesis evaluates the results of experimental protocol involving the fertility sparing treatment procedure in early stage cervical carcinoma (LAP I protocol). Sentinel lymph node detection and experimental extirpation of afferent channels using laparoscopy and its technical aspects were analysed in prospective group of 85 women. The oncologic results and early/late morbidity show that established surgical procedures can be considered safe with minimal morbidity, provided that the indication criteria are met. The second part analyses the results of 148 women with no further pregnancy plans suffering from cervical tumors less than 2 cm in size with invasion less than half of the stroma (LAP II protocol). The oncological results in our defined group are very good and comparable to 'standard' procedure of modified radical hysterectomy type B or C with lower morbidity. In the separate section the thesis analyses the possibilities of laparoscopy in endometrial cancer treatment including the potentials of use of sentinel lymph node detection and technical aspects of laparoscopy in obese women. Currently the biggest controversy is the use of laparoscopy in malignant ovarian tumors.
