Relationship between trunk anthorpometric parametres and back pain

The aim of this study was to compare the anthropometric parameters of the trunk in 50 chronic low back (LBP) patients with a control group of 50 age and sex matching healthy subjects. No significant inter - group difference was identified between the fossa jugularis-processus xiphoideus distance and processus xiphoideus-pubic symphysis distance. A larger waist circumference and waist-to-height ratio, smaller respiratory amplitude and a higher incidence of diastasis recti were observed in the LBP group compared to the control group. However, the increased frequency of diastasis recti in the LBP group was not significant. The results of the study do not support the hypothesis that LBP patients present with different anthropometric parameters of the trunk possibly resulting from the suboptimal postural development during first years of the life. An association between chronic LBP, increased waist circumference, and limited respiratory amplitude has been confirmed.
Correlation between evaluation of functional postural tests and ability to expand abdominal wall in low back pain population.

The theoretical part: The theoretical part of thesis shortly describes anatomy and kinesiology of lumbar spine. The second chapter contains the causes of LBP and alerts to red flags. Muscle stabilization system of spine and intraabdominal pressure are themes of other chapters. Techniques of intraabdominal pressure measurement and questionnaires of quality of life were described at the end of the theoretical part. Methodology: 36 patients (22 female, mean age = 41.86, 16 male, mean age = 40.07) with low back pain were evaluated in three positions. Every proband filled Oswestry Disability Index (ODI) before the examination. Trunk orthosis DNS Brace evaluated 8 seconds intraabdominal pressure in three tests - breathing stereotype test, diaphragm test and hip flexion test. DNS instructor evaluated the same three tests by aspection and palpation. The first half of the probands was evaluated first with DNS Brace, the second half of probands was evaluated first with DNS instructor. Results: A statistically significant correlation (r=0,479, p=0,003) between evaluations DNS Brace and DNS instructor was demonstrated in one out three DNS tests - diaphragm test. Translation and pilot validation of three healthcare questionnaires

Self-reported questionnaires are method of choice for evaluation of relevant domains in low back pain population. Multiple questionnaires exist measuring significant number of potentially meaningful domains. The decision which ones to use should be based on an analysis of clinimetric properties reported in literature. An effort should also be made to translate existing outcome measures to Czech language. This thesis consists of research on methods and parameters designed to evaluate quality and usefulness of measuring methods and example of analysis of clinimetric properties is provided. The experimental section is devoted to reporting the translation process and field testing of three healthcare questionnaires concerning disability (the Patient Specific Functional Scale), self-efficacy (the Pain Self-efficacy Questionnaire) and global perceived effect (the Global Perceived Effect Scale). The questionnaires are comprehensible and ready to be used in clinical practice. There still needs to be a cross-cultural equivalence study performed for them to be eligible for research purposes and international collaboration. In the preliminary analysis, the scores also showed behavior corresponding to the properties reported in the literature.
The use of the OhmTrak device in self-treatment of patients with back pain

Introduction: Impaired trunk stabilization is a frequent cause of chronic low back pain (CLBP). Trunk stabilization training can reduce back pain. Aim: To determine the effect of the OhmTrak (OT) trunk stabilization training device following Dynamic Neuromuscular Stabilization (DNS) principles in patients with CLBP. Methods: A single-blind randomized clinical trial. The investigator did not know to which group the participant belonged. 20 patients with CLBP were randomly divided into 2 groups. 10 patients included in the experimental group underwent six-week DNS training focusing on optimal trunk stabilization. The participants had a treatment session with a physiotherapist once a week. Experimental group patients were educated in self- treatment with the OT device which they performed for 15 minutes 5 times a week. The control group patients (n=10) underwent the same rehabilitation program of the same duration and frequency including the self-treatment. Control group participants performed self-treatment without the OT device. Before the start and at the end of the 6 weeks therapy program the ability to activate the abdominal wall was assessed using DNS Brace device in all participants from both groups. Possibilities of physiotherapy at lumbar spine pain at the people with sedentary employment.

This bachelor thesis deals with the possibilities of physiotherapy for people with sedentary jobs who suffer with pain in lumbar spine region. The lumbar spine region is one of the most loaded part of the spine, because there is a transfer of movement from lower limbs to the torso. The sedentary lifestyle and lack of the movement are very serious problems which affect the modern society and bring many potential risks, which have on the human body. The aim of this work was to describe the negative factors causing lumbar spine pain during sedentary work, to define shortened and weakened muscle groups related to lumbar spine pain and to create an exercise session which will relieve the low back pain in sedentary work. The practical part was performed in the form of qualitative research. The research group consisted of 4 people with sedentary jobs, who also have pain in the lumbar spine area. At the first visit, I acquainted the probands with the schedule and content of therapy and then I took an anamnesis and performed an initial kinesiological analysis. The exercise session was consisted of drills to activate the correct breathing pattern, movements from deep stabilization system, exercises from dynamic neuromuscular stabilization, the Moses method, the McKenzie method and relaxation techniques. The therapy was performed for 3 months. At the end I processed a final kinesiological analysis. The results of the research showed that the biggest changes occurred in the testing of shortened muscles, especially the flexors of the hip joint, and the pain in the lumbar spine was relieved.

Objective evaluation of postural function of abdominal muscles and intra-abdominal pressure Abstract This dissertation deals with an important mechanism of trunk stabilization, the increase of intra-abdominal pressure. Intra-abdominal pressure is created by the coordinated contraction of the diaphragm, along with the abdominal and pelvic floor muscles. Intra-abdominal pressure works in all directions, stabilizing the trunk while reduceing axillary compression of the spine. Intra-abdominal pressure is increased during activities that require demands on the stabilization of the spine. Intra-abdominal pressure and trunk muscle activity can be measured in several different invasive and non-invasive ways. In rehabilitation medicine, anorectal or gastroesophageal probes are most often used to measure intra-abdominal pressure and measurements are often supplemented by evaluation of muscle activation using electromyography. However, these measurements are costly, time consuming and inconvenient for patients. The most common tool for evaluating trunk stabilization in rehabilitation practice is digital palpation of abdominal wall tension. However, this evaluation method is subjective. Title: Use of the Pressure Biofeedback Unit to assess m. transversus abdominis activity - a pilot study Aims: The aim of this Master's thesis is to create an extended rating scale to assess the stabilizing ability of the transverse abdominis muscle measured by the Pressure Biofeedback Unit through PRONE test. Another aim of this thesis is to determine whether the extention of the original rating scale designed to evaluate the stabilizing ability of the transverse abdominis muscle measured by the Pressure Biofeedback Unit in the PRONE test will increase sensitivity of this evaluation. Methods: This thesis is a quantitative study, i tis a quasi-experimantal study. A deliberately selected group of young active men n = 40 with a mean age of 25,63  2,61 who suffer from chronic low back pain without a neurological finding for at least 3 months was used for this study. The influence of rounded shoulder posture on the intensity of low back pain

Title: The influence of rounded shoulder posture on the intensity of low back pain. Objective: The objective of this paper is to determine whether there is a connection between low back pain and a forward shoulder posture. Methods: 23 probands divided into two groups according to pre-established criteria participated in this study. The control group consisted of 10 probands without a forward shoulder posture and without low back pain. The second experimental group consisted of 13 probands with a forward shoulder posture and with low back pain lasting at least three months. The experimental group took part in a four-week therapeutic programme aimed at correcting the forward position of the shoulder joints. This programme consisted of therapy used by Dr. Rašev - the application of a hot towel roll and elastic resistance exercises with Thera-Band. The forward shoulder posture was assessed visually and with a digital caliper. The numerical pain rating scale (NRS) was used to determine whether the low back pain changed when the forward shoulder posture was corrected. Muscle activity in the lumbar spine was monitored by surface electromyography.
