Rehabilitation treatment at the spinal unit. Theoretical basis and practical experience

After spinal cord injury occur a lot of complication, with resulting from neurological picture. These can be central, peripheral or mix paresis in some cases even plegie. Often an autonomic nervous system can be demaged, causing complication practice in nearly all body systems. Rehabilitation attend belongs to basic care of paretic patients and is closely connected with basic with basic nursing methods from which the physiotherapy of these patients arise. This survey gives a summary of procedures and methods of physiotherapy , which help the patient to prevent and better respond to the complication treatment e.g. on respiratory and movement apparatus. Complex physiotherapeutic treatment concentrates mainly on prevention of complications. The physiotherapist must anticipate formation of complications and adapt the comlexity of the treatment. Possibilities of physical therapy in patients with chronic form of respiratory diseases at the pulmonary disease sanatorium

The aim of the thesis is to prove that active techniques of respiratory physiotherapy together with aerosol inhalations contribute effectively to treatment of acute and chronic respiratory problems and also to prove that this therapy increases patient's motivation to realize the importance and effectiveness of respiratory exercises . Benefits of above-mentioned therapy influence patient's professional life. Quality of life has also impact on family and social relationships of respective patient. Bachleor thesis "Possibilities of the Respiratory Physiotherapy in Spinal Cord Unit Patients" deals with respiratory problems of the patient with spinal cord injury. It debates problems of the patient in general, introduces the classification of spinal cord injuries and also informs about a clinical manifestation of the injuries. It includes a comparsion of respiratory kinesiology in healthy man to a man with spinal cord injury. It also informs about common respiratory complications. The work mentions the assessment of the patient. It also shows the most frequent techniques of respiratory physiotherapy which either prevent or suppress respiratory complications. The influence of colostomy on the musculoskeletal system

Introduction. This work is a pilot study which might highlighte pathology of musculoskeletal system at the patients with colostomy. It suppose to evaluate quality of life and the importance of physiotherapy intervention. Methodology. The group includes 13 patients (6 men and 7 women) with left colostomy. Median age was 58 years. The median time from the creation of the stoma at the time of investigation was 33 months. Patients were collected anamnestic data, kinesiological analysis, focused on testing stand, scars, coccyx, muscular tonus of 6 muscle groups, the diagnosis of visceralsomatic relationship and dynamic neuromuscular system. In conclusion, questionnaire methods were used for quality of life and pain. Patients were instructed in two months of therapy, followed by control investigation. Results. The most significant abnormalities were observed in the dynamic neuromuscular system insufficiency, in expression of visceral patterns of colon, but also for the stomach and duodenum. Abdominal wall has hypotonia, the other groups (except for m. piriformis), however, show a left-sided hypertonia. Significant is also the coccyx pain and limited mobility of scars with its palpable sensitivity. Physiotherapy at the department of chronic resuscitation and intensive care

The principal aim of the thesis titled "Physiotherapy at Chronic Intensive Care Unit" (CICU) is to describe and analyze the not much known area of physiotherapy at the CICU and the various methods of its utilization. The thesis is structured in order to deal with the system at CICU, the typology of patients, physiotherapeutic methods and the ventilator modes. The patient should be taken care of by the multidisciplinary team which should cooperate in order to achieve a common goal - consecutive weaning followed by the liberating from the ventilator's support. The chapters about ergotherapy and psychological effects complete the overview of the topic. Magnetic resonance imaging of the respiratory and postural function of the diaphragm

Objective: The goal of the thesis was to objectify changes of the diaphragm motion and shape in a different tasks using dynamic MRI in volunteers with pathological structural finding of the spine and the control group. Methods: Two groups of volunteers were made, those without pathological finding (n=16) and with structural pathological finding of the spine (n=17). We analyzed diaphragm function in three different tasks in all the volunteers during 1) primary breathing, 2) breathing in the course of lower extremities flexion against resistance and 3) holding the breath in the course of lower extremities flexion against resistance. Diaphragm activity was analyzed using static a dynamic parameters. For the first time ever was used postural and respiratory trace, which were separated from dynamic MRI recording. The thesis analyses frequency and amplitude of the respiratory and postural activity (independent on breathing) and their relationship. Furthermore the thesis evaluates breathe in and breathe out rotation, diaphragm excursion magnitude, inclination of the diaphragm, its height in thorax and also diaphragm motion within its different portion. Results: Diaphragm respiratory and postural changes are slower, bigger in size and better balanced in volunteers group without pathological spinal finding....

Thesis "Care of ventilator-dependent patienl wilh spinal cord injury in Czech republic" deals with problems of high cervica\ spinal cord les ion wilh diaphragm para\ysis. Generally introduces clinical manifestation of the spinal cord injury, classifícation of spinal cord lesions and statistical data regarding the incidence, prevalence and etiology. In more detail focuses on respiratory problems. Contains survey of most conunon respiratory complications in ventilator-dependent quadriplegic patients and infonn about the possibililies of respiralory physiotherapy. The practical part provides the first description of the Czech population of pentaplegics.
