Title: Possibilities of functional Evaluation of Patients after acute cerebral Palsy in Czech Republic Objectives: The main objective of this study was to describe the problems of functional assessment of patiens after stroke. Another objektive was to determine and compare the properties ofthe most commonly used functional measures designed for stroke,in our country. Methods: This work is theoretical - research work, which is the theoretical part of the period from September 2012 to June 2013 when retrieving data from literature sources using advanced targeted keywords and thein hotkey in databases based on EBM. The practical part of the study consisted of a questionnaire, which was using the questionnaire survey gathered data needed to evaluace the clinical utility, in the period from May 2013 to July 2013. After obtaining the necessary data from both theoretical and practical part of this data was evaluated and analyzed according to the standard metodology for the evaluation of functional measures for spinal patiens (Chapman, J.R. et al, 2007) and the results of the individual measures were mutually compared. Utilizing of electrical stimulation for decreasing of spasticity in stroke patient

1 Abstract Electrical stimulation by coupled impulses according to Hufschmidt was created with the primary goal of reducing spasticity. However, there are currently few studies that confirm or refute the effect of this type of electrical stimulation in reducing spasticity. The aim of this diploma thesis was to determine the effect of electrostimulation according to Hufschmidt on the reduction of biceps brachii spasticity in patients after stroke. In addition, the effect of electrical stimulation on increasing the active range of movement of the antagonist of the examined muscle and on the active function of the spastic limb was investigated. For these purposes, a non-randomized controlled pilot study was created, which involved a total of 16 probands, 8 - in the therapeutic group, 8 - in the control group. Five-step clinical assessment in spastic paresis according to Gracies were used for the examination. In the therapeutic group, electrostimulation took place 3 times a week, a total of 8 times. The intragroup difference was evaluated using the Friedman and Wilcoxon paired tests, the intergroup difference was subsequently evaluated using the Mann-Whitney U test. Title: Case study of the physiotherapy treatment of a patient after ischemic stroke with right-sided hemiparesis Objectives: The aim of the bachelor thesis is to process a theoretical part dealing with a stroke and a practical part, which includes the case report of the patient after ischemic stroke with right-sided hemiparesis Summary: The bachelor thesis is to process a theoretical part dealing with ischemic stroke and a special part, which contains the methodology of work and case report of the patient with this diagnosis. In the theoretical part, I dealt with the characteristics of ischemic stroke, its cause, risk factors, symptoms, consequences and treatment. The patient's case report includes the patient's initial examination, 8 therapy sessions and the final control examination including an overall final assesment. The usage of virtual reality in therapy of patients with neglect syndrome with consideration of a cybersickness risk

Title: The usage of virtual reality in therapy of patients with neglect syndrome with consideration of a cybersickness risk. Objective: To find out if stroke patients with neglect syndrome tend to have higher risk of gaining cybersickness after using virtual reality compared to stroke patients without neglect syndrome and healthy population. Also, to find out if the severity of neglect syndrome has correlation with the severity of gained cybersickness. Afterwards to evaluate the trend of making games in virtual reality for treating neglect syndrome with consideration of a cybersickness risk. Methods: The experiment participants played game in virtual reality developed for treating neglect syndrome for ten minutes. During the game participants were seated and were playing using head movements. A HTC VIVE console was used to deliver the virtual reality game. Participants were divided into 3 groups: the group Zdraví (n = 12) without neurological impairment, the group CMP (n = 6) for stroke patients without neglect syndrome and the group Neglect (n = 10) for stroke patients with neglect syndrome (KF-NAP ≥ 1). Analysis of gene polymorphism of CYP 2C19 patients treated with clopidogrel

Clopidogrel is an antithrombotic medicament from the group of thienopyrids, used for the prevention of blood cloths by the patients with acute coronary syndrome, patients after an ischemic stroke, myocardial infarction or by the patients with proven lower limb ischemia. Clopidogrel is a "prodrug", which means that it needs to be metabolised into an active metabolite before it stars working. The cytochrome P450 2C19 plays the main role in the bioactivation of clopidogrel. Individual enzymes of cytochrome complex are coded by genes, which are polymorphic. The function of P450 2C19 can differ by individuals, from the reason of gene polymorphism, therefore the bioactivation of clopidogrel can differ as well. There are few alleles of cytochrome P450 CYP2C19, which are connected with different therapeutic reactions on the submission of clopidogrel. Individuals with the allele CYP2C19*17 have an increased concentration of active metabolite and are at a higher risk of bleeding complications. The carriers of CYP2C19*2 a CYP2C19*3 alleles have a decreased metabolisation of clopidogrel into an active metabolite, which leads to insufficient antiaggregate effect and to increased appearance of cardiovascular complications. The allele CYP2C19*1 is connected with a fully functioning metabolism. In the group of 512 patients, we detected 191 carriers of the genotype *1/*1, 79 carriers of the genotype *2/*1, 12 carriers of the genotype *2/*2, 1 carrier of the genotype *3/*1, 43 carriers of the genotype *17/*17, 146 carriers of the genotype *17/*1 and 40 carriers of the genotype *17/*2. The stratification of the patients according to genotype CYP2C19 has a significant meaning for adjusting the right treatment strategy and it is a prevention of stroke and the risk of bleeding.
Complications of systemic thrombolysis in patients with stroke

Strokes are one of the most serious diseases that often lead to permanent disability and in some cases even to death. Currently, widely available treatment by intravenous thrombolysis is recommended. The objectives of this diploma thesis are to analyse complications that may arise for patients with iCMP treated by intravenous system thrombolysis, to define the role of a nurse in ICU in terms of taking care of this group of patients, and to map all the factors influencing the result of the treatment. In order to obtain the data necessary, combined research method has been applied. The quantitative part was retrospectively evaluated based on SITS Registry data. The research part comprised of patients, to whom IVT was applied between January 2014 and December 2015. The subjects of the analysis were treatment complications, gender of patients treated by IVT, risk factors present, time data, and heaviness of the deficit before the treatment and after. Furthermore, we observed where is the patient released to after IVT and how many patients are able to stay in home environments three months after the treatment. Name: CASE STUDY OF A PHYSIOTHERAPY TREATMENT OF A PATIENT AFTER A STROKE Aims: The aim of this thesis is to gain and collect theoretical information about theme "stroke" and the development of the case study of a patient with this diagnosis. Methods: The thesis is devided into two main parts - theoretical and practical. Theoretical part is devoted to a knowledge about stroke gained during university studies, and to information from specialized literature. This part summarizes basic findings about diagnosis, treatment and rehabilitation. Practical part comes out of exact case study of a patient after suffering ischemic stroke with cerebellar syndrome. Case study was prepared on a bassis of practice from 22nd of January till 16th of February 2018 at Vršovická zdravotní a.s. ADL manual for people after stroke

This bachelor thesis is aiming at creating an ADL (Activity of Daily Living) manual for people who suffered a stroke. Theoretical part is mainly focused on results of stroke that patients have to live with. It represents what occupational therapy is about and its irreplaceable role in the process of the patient's return to active life. It shows wide range of rehabilitation process and patient's difficulties with availability of complete materials with its issue. There are materials describing etiology, causes and results of stroke on the market, but not so many publications describing procedure how to deal with it and how to reach the highest level of patient's independence in normal life when it once happened. Practical part is represented by my own manual called ,,ADL manual for people after a stroke". Manual is designed for ergotherapist to provide people after stroke with useful tutorials and instructions right in given part of ADL in which people have troubles. Summary sheets were made for each category of ADL with description of the most common difficulties. There are particular tutorials for each method of ADL and suggestions of compensatory aids with description how can each aid facilitate given activities.
