Molecular Markers with Impact on Kidney Graft Survival and Glomerulopathies Progression

The progression of chronic glomerulopathy and graft rejection is affected by a number of proinflammatory cytokines, whose role in the pathogenesis of damage is poorly understood. The aim of this dissertation was to identify reliable risk markers of renal dysfunction progression and thereby contribute to a more effective patient treatment. Human native kidney biopsies with histologically confirmed diagnosis of glomerulopathy or kidney graft biopsies were analysed. Intrarenal gene expressions were measured by RT-qPCR. Single nucleotide polymorphisms were detected by methods based on PCR-RFLP. Immunohistochemical staining was used to identify and quantify the mononuclear cell infiltration. Gene expression of TGF-β1, HGF, BMP7, MCP-1, RANTES and mononuclear cell infiltration were associated with poor renal function and proteinuria at the time of IgA nephropathy diagnosis. Progression of IgA nephropathy during the 2-year follow-up was shown to be dependent on the degree of chronic vasculopathy and TGF-1 expression in the kidney. Patients with graft dysfunction and enhanced intrarenal expression of TGF-1, MCP-1 had significantly shorter graft survival. Higher mRNA expression of IL-10, TGF- 1, IL-6, MCP-1, RANTES and TNF- was observed in patients with graft dysfunction presented at the time of biopsy....

Need of seniors

The aim of this bachelor work is to find out the difference in the satisfaction of needs amongst in-house senior patients and external, or living at home seniors. The theory part explains the term of needs, it divides them into lower and higher needs. It analyses physical indications of old age and the relationship between medical and social old age. It mentions hospitalism and syndrome of being immobilised. The practical part presents quantitavie research using questionnaires methods. The analysed sample was made up of seniors in "Oddělení následné péče" in Frýdlant and seniors living in "Domov U Spasitele" also in Frýdlant. All findings are present using graphs and charts. The summary and recommendation for nursing practis can be found in the discussion part.

Immunotherapy of ovarian carcinoma with dendritic cells

V ANGLICKÉM JAZYCE Immunotherapy of ovarian carcinoma with dendritic cells Anticancer immunotherapy is a therapeutical strategy aimed at elicitation and maintenance of immune responses against cancer cells. In this study we have focused on immunotherapy of ovarian cancer, because it is one of the most common gynaecological tumors with poor prognosis and high mortality. Our immunotherapy protocol involves preparing dendritic cells (DC) from monocytes isolated from patient's peripheral blood, which are subsequently pulsed with irradiated cells of established ovarian cancer cell line. These immature pulsed DC are maturated and subsequently co-cultivated with autologous T lymphocytes. The aim of this study was to demonstrate, that DC are able to elicit specific immune response after addition of suitable mature agens in combination with apoptotic ovarian tumor cells. Our observations indicate that 24 hours are sufficient for induction of tumor cells apoptosis. Additionally, we have shown that DC successfully ingested most of the apoptotic tumor cells after 4 hours of co-incubation. Furthermore, we have found out that ingestion of apoptotic cells by dendritic cells, which are stimulated with polyI:C, inhibits maturation of DC and consequently also production of cytokines IL-12p70, IL-6 and TNF-α. The influence od educational to maintenance of secondary prevention rules of myocardial infarction

This bachelor thesis deals with the effects of educational plans on the compliance with the principles of the secondary prevention of myocardial infarction. The theoretical background covers atherosclerosis, acute myocardial infarction, the secondary prevention of myocardial infarction and the educational process in nursing. Prior to elaborating the practical part of the thesis we had established two objectives. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. A qualitative form of research had been chosen for this thesis, semi-standardized interview in particular. The analysis of the data gained was subsequently carried out. Two objectives had been established in the thesis. Objective 1: To find out whether the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Through the analysis of the obtained data we found out that the patients educated without an educational plan are as knowledgeable as the patients educated through an educational plan. Objective 2: To compare whether the patients educated without an educational plan comply with the principles of the secondary prevention of myocardial infarction in the same way as the patients with an educational plan. Having compared the results we found out that the patients with an educational plan comply with the principles of the secondary prevention more consistently than the patients educated without an educational plan. The objectives of the thesis were met.

The influence of targeted treatment on stereognosis and somatognosis in patients with chronic vertebrogenic algic syndrome

This diploma thesis called "The impact of individual physiotherapy on stereognosis and somatognosis in patients with chronic back pain" contains basic knowledge about stereognostic and somatognostic function, it summarizes significant information about pain and the development of its chronic form, and last but not least it states possible causes of back pain disorders and its functional factors. One special part of the thesis describes non-standardized tests used to examine 21 probands who had been diagnosed chronic back pain and particularly it compares the results of these two examinations, the first of which was carried out before the beginning of physiotherapy and the second one carried out after 14 days of physiotherapeutic treatment. The aim of this thesis was to evaluate the impact of physiotherapy on stereognosis and somatognosis of patients with chronic back pain. All types of medical examination were tested without visual check. As a part of the medical examination a short form of McGill ́s questionnaire and a visual analogue scale indicating the intensity of pain were to be filled in by the probands to explicate their subjective feelings. The aim of the thesis " A Physiotherapist's View of Overactive Bladder function in Multiple Sclerosis Patients" and the influence of physiotherapy on the symptoms of the overactive bladder. This thesis deals with exert influence on symptoms of an overactive bladder by physiotherapy. Therapy is based on the fact that guarding reflexes that maintains continence at the level of spine and subcortical (micturition centre in pons) are activated by the contraction of the pelvic floor muscles. Nonparametric estimations in survival analysis

This work introduces nonparametric models which are used in time to event data analysis. It is focused on applying these methods in medicine where it is called survival analysis. The basic techniques and problems, which can appear in survival analysis, are presented and explained here. The Kaplan -- Meier estimator of survival function is discussed in the main part. This is the most frequented method used for estimating the survival function in patients who have undergone a specific treatment. The Kaplan -- Meier estimator is also a common device in the statistical packets. In addition to estimation of survival function, the estimation of hazard function and cumulative hazard function is presented. The hazard function shows the intensity of an individual experiencing the particular event in a short time period. Special problems occur when analyzing time to event data. A distinctive feature, often present in such data, is known as censoring. That is the situation when the individual does not experience the event of interest at the time of study. The thesis covers also an empiric part, where the results of an analysis of patients with the larynx carcinoma diagnosis are shown. These patients were treated in a hospital located in České Budějovice. This analysis is based on a theory presented in the previous chapters.
