Injuries of knee joint and consequential physiotherapy after crossed ligaments plastic

This work deals with questions of injuries of the fibrous apparatus of the knee joint, especially with the injury of ligamentum cruciatum anterius (LCA). The theoretical part summarizes the basic information relating to the problems of injuries of the fibrous apparatus of the knee. In the following, it offers an abstract of recent methods of physiotherapy used while dealing with patients with LCA plastic. The evaluation of the research, which compares the efficiency of the physiotherapy between two patients with LCA plastic (one active sportsman and the other a non sportsman), discovered that the therapy was more effective in the sportsman case, especially in term of faster growth of muscles.

Knowledgeability of clients about postoperative care at orthopaedic department

Clients suffering from musculoskeletal system problems are hospitalised at the orthopaedic department. Most clients must undergo a surgery here. Surgery is a stressful situation for many clients. They are in foreign environment and do not know what to expect, they are afraid. A nurse at an orthopaedic department has a very important function. She accompanies the client before, during and after the surgery. She aims to provide her clients with enough information to calm them and to make their stay in a medical facility pleasant. Quantitative research performed by means od anonymous questionnaires was used to collect data. We addressed nurses from traumatological wards in the following hospitals: Nemocnice Písek, a.s. and Ústřední vojenská nemocnice Prague. We also contacted clients hospitalized in orthopedic and traumatological ward in the two identified hospitals. We collected identification data and information about the general awareness of postoperative care and postoperative complications. Three objectives were determined in the bachelor essay. Determination of the level of clients´ awareness, the means used by nurses to provide information about postoperative care and about postoperative complications. And whether nurses provide information through the nursing process. Results of the research were fulfilled. Three hypothese were determined. The first and the second hypothese were to confirm that clients are sufficiently informed about the postoperative care and postoperative copmlications and that nurses provide clients with the nursing care through the nursing process. These hypotheses were confirmed. Nurses inform clients about the postoperative care and they provide nursing care through the nursing process. The third hypothesis was supposed to confirm that nurses utilize all means to inform clients about the postoperative care. This hypothesis was not confirmed; nurses inform clients only orally. The research showed that clients address thein inquiries to both nurses and doctors. Clients´ awareness of postoperative care was sufficient and there is no need to increase it, however, the awareness of postoperative complications is weaker and clients would like to obtain more information from doctors. In spite of that, most of them say that postoperative complications cannot be prevented by better awareness anyway. Postoperative care is a part of the recovery period after a surgery {--} knee ligament athroplasty. Sufficient awareness of clients and timely rehabilitation is an integral part of that. Therefore, it is necessary to provide clients with sufficient amount of information. The results of the bachelor assay will be submitted to nurses in orthopedic wards in the two identified hospitals, i.e. Nemocnice Písek, a.s. and Ústřední Vojenská Nemocnice Prague, where the research was performed; the results could also be used as a source of information in workshops not only for nurses working in orthopedic wards, but for everyne who is interested in this topic. The results could also enhance the postoperative care within the nursing prosess.

Physiotherapy for patients with LCA rupture during surgical and conservative therapy

The bachelor thesis deals with the issue of an anterior cruciate ligament (ligamnetum cruciatum anterius ? LCA) trauma. The anterior cruciate ligament is an important stabilizer of the knee joint. During the LCA lesion serious biomechanical changes occur. The LCA trauma most frequently happens due to forced hyperextension or rotation of the knee joint. Nowadays this injury is quite frequent, in the sports medicine it occurs in the incidence of 1 / 1750 inhabitants. The therapy of the injured LCA has been very efficient recently mainly due to modern approaches both in the medicine and the following rehabilitation. The thesis contains the analysis of the anatomy and biomechanics of the knee joint, the next part describes LCA trauma mechanisms, methods of its examination and possibilities of the successive therapy. One part of the thesis is focused on the comparison of the conservative and the surgical therapy, there are described the circumstances under which it is decided which one of these therapies will be chosen. It is necessary to mention that the most important criterions for the proper therapy choice are the individual needs of the patient. The basic aim of the thesis is the comparison of the efficiency of therapeutic procedures for particular patients and the assembly of the short-term and the long-term treatment plans. The practical part of the thesis is processed by the qualitative research method. Three probands after the surgical therapy and two probands after the conservative therapy were examined. The observation and the therapy for surgically treated patients were carried out between the second and the sixth post-operative week, the examination for conservatively treated patients was carried out for six weeks. Patients were predominantly athletes in the age of 18 ? 24 years. They passed the initial and the final kineziologic examinations and the rehabilitation therapy; the treatment efficiency was rated by the Lysholm score. The initial Lysholm score for surgically treated patients varied between 32 and 34 points and after the therapy it increased to 48 ? 75 points. For conservatively treated patients the initial score varied between 73 and 76 points and the final score was 76 ? 81 points. There is one more important detail in the thesis ? the anthropometry results. The quadriceps femoris perimeter increase was around 1,5cm and the triceps surae perimeter increase was around 1cm. The final examination showed that the majority of the patients is missing minimally 1cm in the quadriceps femoris perimeter compared to the healthy leg. The similar indicator has been found also for the muscular strength testing. The examination was focused mainly on the knee joint extensors and flexors testing. All patients proved the increase of the muscular strength; surgically treated patients reached muscular strength maximally 4+. The short-time rehabilitation plan is initially based on the edema and pain elimination. Subsequent rehabilitation step is the training of the walking with the help of French canes and the knee joint motion range recovery and its stabilization. The aim of the long-term rehabilitation plan is the gradual return to social and sport activities. The basic factor of the long-term rehabilitation plan is good muscular training. In the conclusion it is important to mention that it is not possible to state which therapy (surgical or conservative) is more suitable or efficient, during the decision making process it is necessary to consider individual needs of a particular patient. The thesis may be utilized in the clinical practice of physiotherapists for the improvement of the care of the patient who is diagnosed with rpt. LCA.

Influence of surgical technique in ACL rupture on selected rheological parameters of the knee

Cíl práce: Zhodnocení a porovnání změn reologických vlastností kolenního kloubu u ruptury předního zkříženého vazu před a po jejím chirurgickém ošetření a v průběhu rekonvalescence. Metoda: Experimentálního in vivo měření pasivního odporu kolenního kloubu při vynuceném pohybu do flexe a extenze metodou bioreometrie. Zhodnocení specifických reologických vlastností, které vystihují momentální stav kolenního kloubu. Vyhodnocení naměřených dat pomocí grafického znázornění sumárních reologických charakteristik - bioreogramu. Výsledky: Výsledky odhalují průběh rekonvalescence u pacientů po plastice předního zkříženého vazu. Tento průběh zachycuje specifické změny reologických charakteristik kolenního kloubu během tří měsíců, které byly patrné při vyhodnocování bioreogramů. Byly zjištěny změny tvaru křivek, které vypovídají o změnách tuhosti, disipované energie a tím i "účinnosti" kolenního kloubu během léčebného procesu. Výsledky naznačily možnost budoucího hodnocení poruch kolene právě metodou bioreometrie. Postural stability in subjects with anterior cruciate ligament injury

6 Abstract Title: Postural stability in subjects with anterior cruciate ligament injury. Objectives: The aim of this thesis was to find out if the postural stability is differed in subjects with anterior cruciate ligament injury and in the control group after the "4 steps - one leg stance" test had been performed. Methods: This study compared a group with anterior cruciate ligament injury and a control group on the basis of the "4 steps - one leg stance" test. Methods of comparison and analysis were used. Results: The test and all measurements revealed differences in postural stability between subjects with anterior cruciate ligament injury and the control group while they were standing both on operated and non-operated limb. The use of taping and kinesiotaping in physiotherapy

This thesis is focused on the possibilities of using taping and kinesiotaping at physiotherapy. Application of the methods was provided in three different diagnoses. These were instability of the knee joint after plastic operation LCA, hallux valgus and backpain in the third trimester of pregnancy. The thesis is divided into the theoretical and practical part. The theoretical part describes the methods taping and kinesiotaping and it refers to the problems of the individual diagnoses. The practical part uses the qualitative research where three case reports are processed. The case reports contain introductory kinesiological analysis, progress of the therapy, checking kinesiological analysis and the therapy's evaluation. In the thesis taping and kinesiotaping are used individually despite the fact that these methods are just supplementary to the therapy. It is done in this way so as to be possible to evaluate the efficiency of these methods in selected diagnoses. Physiotherapy after plastic of anterior cruciate ligament

Possibilities of rehabilitation in children with cleft defects

The cleft defects of orofacial complex belong among the most frequent congenital defects. These defects are affecting functions of orofacial complex and then complications of food intake, breathing, phonation and mimics can occur. On that ground the treatment needs complex care. The members of multidisciplinar team are plastic surgeon, orthodontist, paediatrician, geneticist, anaesthetist, anthropologist, phoniatrician, speech therapist and psychologist. The aim of the work is to describe physiotherapeutical techniques, methods or concepts, which can reduce, clear off or prevent disorders of orofacial complex functions since newborns. Currently it can support physiological motory development of children. The assimilation of physiotherapy into care complex could improve therapy for children with cleft defects.
