Inflammatory bowel disease

Infammatory bowel disease IBD is unspecified form of bowel inflammation represented by two types - Ulcerative Colitis and Crohn's Disease. UC is illness that typically affects intestine mucous membrane from rectum up to proximal parts of gastrointestinal tract. Affection in UC is limited only to colon. CD is characterised by inflammation of whole intestinal wall with common manifestation in ileocecal region but entire colon and small intestine can be affected. Patients with UC and CD suffer by extraintestinal manifestations similar for both diseases. Illness therapy is currently focused on biological treatment. People with IBD are not excluded from common life activities like school attendance, sports, etc. even they have complications or some sort of handicap. These patients can meet each other and seek advice within IBD associations all over the world.
The options of nutritional support in inflamatory bowel disease

In my bachelor's work I describe the idiopathic inflammatory bowel diseases. The theoretical part is devoted to the history, epidemiology, etiopathogenesis, basic characteristics of the disease and the current treatment options. One of the serious consequences of the inflammatory bowel diseases is malnutrition, which then affects the treatment, worsens the course of the disease and reduces the quality of life of patients. The cause of malnutrition in these patients is both inflammatory activity during a relapse, the side effects of drug therapy and reduced food intake due to the numerous diarrhea and abdominal pain. In the practical part I monitored the patients during my practice in gastroenterology clinic. The aim of my work was to determine what attention is paid to the state of nutrition of patients with severe course of Crohn's disease and ulcerative colitis, which were indicated by the biological treatment. Monitoring of nutritional status, nutritional support and correction of malnutrition is an integral part of the treatment, during which a nutritional therapist plays an important role. Human gastrointestinal mycobiome in health and disease

This Bachelor thesis is a complex review of information about mycobiome of human digestive tract. It summarizes the known incidence, diversity and abundance of fungi in different parts of the gastrointestinal tract together with factors that determine composition of its communities. It also describes plentiful interactions with immune system, other microbes and between fungal species themselves. Then there is a brief remark about methodology that is in use for this subject together with its pros and cons. Thesis sums up and discusses current knowledge about the healthy mycobiome, highlighting cases, when it plays role in the formation of disbalance often resulting in an outbreak of a disease. The paper considers ecology and detailed interactions with the environment for Candida species, the dominant gastrointestinal fungus. Intestinal mycobiota has been lately purposed as an important factor in the development of certain local diseases, which include inflammatory bowel disease, ulcerative colitis and Crohn's disease, or irritable bowel syndrome. Moreover other seemingly unrelated diseases have been associated with a change in intestinal mycobiome in the recent years. Quality of life in patients with inflammatory bowel diseases

Quality of life in patients with inflammatory bowel disease (IBD) depends on activity, therapy and complications of this chronic disease. Further more it also depends on psychosocial and economic aspects. Nurse plays an important role in education and psychological care for these patients. Aim of this work, based on questionnaire and its statistical processing is to evaluate quality of life of IBD patients (according to age, education and social background) and monitor its dynamics from the time of diagnosis and during treatment of the disease. 1. -6-
The cooperation of a nurse with nutritional therapist during the care of a patient with an ulcerative colitis.

The aim of this bachelor thesis is to examine the cooperation of a nurse and nutritional therapist while taking care of the client with diagnosed ulcerative colitis. The paper is divided into a theoretical and practical part. In the theoretical part the author deals with the anatomy and physiology of the colon, the characteristics of ulcerative colitis, its etiology and pathogenesis, clinical picture, complications, diagnosis, treatment and special measures regime. Here, it discusses nursing care for clients with ulcerative colitis, preparation of a client for an endoscopic examination and client´s informative education. The final chapter of the theoretical part focuses on the multidisciplinary team, workplace relationships and after that nurse and/or nutritional therapist competences. The qualitative research was carried out for the empirical part of the thesis. The selected method of data collection was semi-structured interviews with nurses, nutritional therapists and patients with ulcerative colitis. These clients were deliberately selected providing that they have been currently or lately hospitalized in the inpatients departments due to the deterioration in health concerning ulcerative colitis. Interview questions designed for nurses regarded the availability of a nutrition therapist for their department, co-operation with this specialist, educational materials available along with the way the nurse educates the patient and nurses' knowledge of the issue. Nutrition therapist questions focused on their collaboration with nurses in inpatient departments, information transmission, clients´ education and their feedback collection. At the interviews with clients we determined dieting and raising awareness of the diet, the source of the acquired information and the interest of nurses to improve the nutrition of each client while hospitalized. On the basis of respondents' answers, we successfully worked out responses to research questions. Research question no. 1 how nurses collaborate with nutritional therapists in the care for patients with ulcerative colitis in the inpatient department- emerged that nurses in these departments did not come in contact with nutritional therapists at all, so they could not cooperate so that the exchange of essential information would take place only by reading through the client's medical record. Furthermore, from nurses' and clients' statements, the way a nurse primarily educates a client with ulcerative colitis in terms of nutrition is commonly just done by giving out printed information brochures or leaflets available at the department. The prime source of nutrition information for clients is usually their physician while diagnosing. Clients can learn more thanks to nurses, nutrition therapists and they also search for information themselves. Four out of five respondents claimed they still do not have enough relevant information about their diet. Attention must be given to ensure clients get to reliable information sources in order not to be a subject to the misleading information presented on the Internet and therefore for instance stick to improper diets. Education provided by nurse and nurses' knowledge in this field can thus have a direct impact on client´s health as much as the collaboration with a nutritional therapist, which could help the nurse to test the client's knowledge of nutrition therapy education. The acquired knowledge can demonstrate the importance of multidisciplinary collaboration not only in the care of clients with diagnosed ulcerative colitis but also the significance of nurse´s education, which is often needful support and safety for the client during his hospitalization.
Quality of life of patients with inflammatory bowel disease (IBD)

Inflammatory bowel disease is often discussed topic among the medics in the field of diagnostics, treatment and prognosis, however what is usually ignored is quality of life. The bachelor thesis aims at revealing the quality of life of patients with idiopathic bowel disease. The theoretical part introduces inflammatory bowel disease with its principal types of Crohn´s disease and ulcerative colitis. The empiric part is focused on IBD patients´ quality of life. The research part of the thesis is based on standardized questionnaire Short Form 36, aimed at quality of life of patients in South Bohemia. For reaching the goal, three main hypotheses were selected: H1: Patients with inflammatory bowel disease (IBD) have mental problems. H2: Patients with inflammatory bowel disease (IBD) have physical problems. H3: Patients with inflammatory bowel disease (IBD) have social problems. The questionnaire construction was conducted in Word Excel programme using graphs and basic statistics. A positive output from the respondents was very surprising because the most of the respondents answered the questions concerning their quality of life positively. It is possible to state that this situation may be caused by the latest available treatment, timely diagnosis and also by a certain effort within our society to deal with chronically ill patients with kindness and respect. This survey convinced me that also people with chronical diseases can have good quality of life. The topic of this bachelor thesis was highly interesting for me; my original presumptions were different, therefore the results were surprisingly pleasing. Creating this bachelor thesis was very helpful for me. This bachelor thesis can be used as informative material for nurses in hospitals or for students of non-medical health care branches. Furthermore, this thesis can be used for further study.
Clinical and genetic predictors of drug dependency in inflammatory bowel disease

IN ENGLISH Drug dependency in inflammatory bowel disease (IBD), Crohn's disease (CD) and ulcerative colitis (UC), is a specific disease phenotype which determines disease prognosis and hence may be used as a prognostic marker for treatment management. Drug dependency in IBD has been well described in corticosteroid treatment and recently also in infliximab (IFX) therapy. The aims of this thesis were: 1) to assess the occurrence of IFX dependency in paediatric and adult patients with CD; further to search for clinical and genetic predictors of IFX outcome and to evaluate the impact of IFX dependency on surgical rate; 2) to assess in CD patients the outcome of the first course of 5-ASA monotherapy with emphasis on 5-ASA dependency and to define clinical predictors of 5-ASA treatment outcome. We found that 66% of children and 29% of adults with CD became IFX dependent. The high frequency in paediatrics is in agreement with previously published studies, while the finding in adult patients indicates a lower rate of IFX dependency in the only study to date. Perianal disease and no bowel surgery prior to IFX start were predicative of IFX dependency in paediatric patients. Patients with chronic bowel disease and their lifestyles

The thesis deals with the lifestyle of patients with chronic intestinal disease. The theoretical part contains topics of theoretical knowledge essential to the orientation in the issue. It describes the principles of a healthy lifestyle, the issue of chronic intestinal disease, especially Crohn's disease and ulcerative colitis as a disease of clinical manifestations, from diagnosis to treatment. An important chapter of the theoretical part of the work are the impacts of IBD on lifestyle at chronically ill patients. The empirical part deals with the qualitative research. For datas collecting was chosen and used the method of questioning with narrative interview technique. Methodology of narrative interview was expanded to include specific methods of collection datas, the life curve method. The content of each interpretation is the final report that summarizes the observed facts associated with answers to research questions. Significant elements of the interviews are recorded in tables. The aim of the thesis was to find out in what areas and how the selected people with chronic intestinal diseases have changed their lifestyle. To provide information to the public about the limitations of patients with chronic intestinal diseases that directly affect their everyday lives. Finally to assist society tolerance to constraints of patients with chronic intestinal disease.
Education of patients with ulcerative colitis

My bachelor thesis focused on " Education of patients with ulcerative colitis " has been split in theoretical part and practical part. The theoretical part has been focused on characteristicsof ulcerative colitis and education. This part has been split in nine chapters. The chapters describe anatomy and physiology of colon, definition of the disease, its causes, symptoms and complications. Furthermore, the chapters have been focused on examination methods and treatment of ulcerative colitis and nursing care. The last chapter of the theoretical part has been devoted to education. There were two aims defined for the bachelor thesis; to describe methods of educating the clients suffering from ulcerative colitis and to determine efficiency of education for a patient suffering from ulcerative colitis (in the course of 3 months after the first education). Based on the specified aims, three investigation questions were asked. What is the effect of the suggested education on health status of a client suffering from ulcerative colitis? Does observance of diet measures affect relapse of the disease? How do reasonable physical strain and psychical stress affect the disease? The practical part of the thesis has been processed through qualitative research. The research was made with respondents with the diagnoses of ulcerative colitis and with nurses who work at gastroenterologic ward in the hospital in České Budějovice, a.s. The research was split in three parts. The first part included interviewing of eight respondents with the diagnoses of ulcerative colitis. The interview was semi-structured. This included eighteen questions focused on the progress of the disease, education of the respondent, diet measures and influence of physical strain and psychical stress on the disease. Then, each respondent was educated through the education. The second part included interviewing of three nurses from gastroenterologic ward in the hospital in České Budějovice, a.s. The interview was semi-structured, too. It included sixteen questions which are very similar to allow better comparison with the patients. The third part was focused on check of knowledge and efficiency of former education of three respondents. This was a structured interview. It included five questions. These were focused on present status of the disease, evaluation of observance and efficiency of the low-residue diet and the basis of mental hygiene. The interviews were taken in calm home environment. Results of the research showed that the respondents have lack of information most frequently in the field of physical strain and psychical stress and often in the field of low-residue diet. However, the respondents are mainly well-educated about the definition of the disease and about treatment. In my opinion, my bachelor thesis is helpful because it has proven that correct education may have positive effect on the progress of the disease. The thesis might be also used as education material and it might help improve education of clients with the diagnoses of ulcerative colitis.
