Spinal trauma in terms of pre-hospital care

This bachelor thesis topic is the pre-hospital care about the patient with spinal trauma. The aim of the thesis is to evaluate used algorithms of paramedics during the first contact. This thesis contains of theoretical and research part. Theoretical part is focused on anatomical and physiological relationships which are neccesary to understand because of complexity of spinal trauma. Detail view on anatomy of spine and spinal cord is introduced same as the mutual topographical relationships. Following that general pathophysiology of spinal trauma is briefly pointed out same as the most common causes of this injury. For complete overview thesis also contains basic diagnostical procedures and spinal trauma classification. For the pre-hospital care remains the most important part precise collecting of patient history and evaluation of situation on site. Imagine methods and clinical examination is the most important steps for final diagnostics in the hospital. But this thesis major focus is targeted on the pre-hospital care. Because of that it also mentions cooperation between integrated rescue system members and their imobilisation and transportation equipment. Research part has two goals. First one is to find out methods and algorithm used by paramedics. Second goal is to analyze their equipment. For this was used qualitative research study. Data were gathered by semistructured questionnaire and its outcomes were analyzed by method of coding. Representative sample was picked with method of snowball among paramedics of Rescue servis Vysočina. After data evaluation was found that participating paramedics are all well educated in the recent rescue guidelines for spinal trauma. Small surprise was that most of them prefer to use manual extraction methods to extraction equipment which seems to be too complicated for fast use in the field. The most prefered equipment was cervical collar and vacuum splint. Other equipment was used rarely according to specific situation. As the conclusion it can be said that methods and equipment of Rescue service Vysočina follows latest algorithms and guidelines. Yet crucial remains professional attitude, following evidence based medicine triads and personalized medicine.

The thesis describes Medical Rescue Service of South Bohemia Region as the provider of pre-hospital urgent care in the zone of emergency planning of Nuclear Power Plant Temelín. In case radiation emergency arises, its main urgent target is to save as many affected people as possible and to limit the effects on their physical disabilities. Medical intervention by the Medical Rescue Service of South Bohemia Region crews is provided to people with serious health problems or in immediate life danger who participate in rescue and emergency work directly in the epicentre of the radiation emergency of Nuclear Power Plant Temelín. It also provides pre-hospital urgent care to the inhabitants who have already been evacuated right in the zone of emergency planning, on the evacuation routes, on the decontamination points and based on the emergency call also in the receiving centre or emergency accommodation points. The main aim of the thesis is to identify and clarify basic principles of tasks completion and workload of outgoing Medical Rescue Service of South Bohemia Region crews in case of an emergency event occurrence - the radiation emergency at Nuclear Power Plant Temelín. The theoretical part describes the status of the Medical Rescue Service of South Bohemia Region within the Integrated Rescue System in the case of radiation emergency. Attention is also paid to material and technical equipment aimed at personal protection during the intervention in the contaminated environment. Content analysis of key executive documentation and legal regulations research have been made in this part whose synthesis leads to a comprehensive professional text predicting the procedure of urgent medical intervention made by the medical rescue service in the zone of emergency planning. The practical part includes the network analysis of transportation availability of individual decontamination points by the medical rescue service vehicles. Based on the findings, the index of alert has been calculated that reflects the quality and availability of pre-hospital urgent care to the evacuated inhabitants in the decontamination point as the key point of evacuation from the zone of emergency planning. The results proved readiness of the Medical Rescue Service of South Bohemia Region in the decontamination points to tasks completion and workload of medical provision of evacuation from the zone of emergency planning of Nuclear Power Plant Temelín. The thesis has become a concentrated overview as well as operating material for the paramedics working at Medical Rescue Service of South Bohemia Region who could deal with the uneasy task of intervention in the zone of emergency planning of Nuclear Power Plant Temelín. It also reflects the need of preparing a unitary documentation describing the procedure in radiation emergency.
Preparedness of physicians emergency Medical services of the South Bohemian Region for emergencies with a collective disability of persons

This master thesis is focused on educating physicians in mass casualty incidents. This paper aims to introduce the basics of mass casualty management, triage categories and tasks of the staff in mass casualty incidents. A questionnaire forms the research. Fifteen physicians working for Emergency Medical Services of South Bohemian Region responded to the questions about their theoretical knowledge about mass casualty management and their opinion about the quality of training courses at EMS of South Bohemian Region. There was a comparative analysis of theoretical knowledge and the real-life experience of the physicians at EMS of South Bohemian Region. The graphs with an evaluation of all the responses showed the results. This paper can make good use for further education on this topic at Emergency Preparedness Service of EMS of South Bohemian Region.
Ability of laical public providing first aid in acute condition from point of view of paramedics

This bachelor's thesis examines the issues of providing non-professional first aid in acute condition from paramedics'point of view. The bachelor's thesis is divided into a theoretical part and research part. At first, the theoretical part describes the emergency medical service, its rescue forces and types of ambulance crew in order to explain the connection between the first aid and pre-hospital emergency care. This part analyses the first aid not only from the theoretical and practical point of view, but also from a social psychological one. Then, the theoretical part also deals with anatomy, physiology, pathophysiology and the first aid of six selected acute conditions. The second part is focused on the analysis of data obtained from a research survey. The research survey was conducted by a qualitative survey using as main methods a semi-structured interview and direct observation during university practice. The semi-structured interview includes 33 questions and is answered by eight paramedics that serves in the South Bohemian Region. The aim of this thesis is to find out what the range of capabilities of general public is in providing the non-professional first aid, how effective the general public is in providing the non-professional first aid and whether people are willing to help. The obtained information led to answering all research questions and fulfilling the aim of the bachelor's thesis. The results of the research evaluate non-professional public as willing enough and with sufficient abilities to provide the first aid but on the other hand the effectiveness of performed actions is considerably insufficient. The main contribution of this bachelor's thesis is the rise of awareness about the issues of the first aid provided by laical public, which could contribute to the improvement of the first aid educational method and subsequently to the improvement of providing the non-professional first aid by general public in cases of acute conditions.
Cardiopulmonary resuscitation from the point of view of emergency medical assistance group

This thesis thoroughly explores the issues that come with cardiopulmonary resuscitation, which in pre-hospital care is handled by emergency medical assistance groups and of which paramedic staff are an inseparable part. These paramedics have a necessity for sufficient, in-depth knowledge and experience. Without the early and critical assistance that paramedics are able to provide, a patient could suffer from irreversible changes in health and are even at a risk of death. This bachelor's thesis is divided into a theoretical section and a research section, further exploring this incredibly important subject. The theoretical section explores the history of cardiopulmonary resuscitation. It also digs into the basic life support provided mainly by lay rescuers, in accordance with the guidelines of the European Resuscitation Council. Furthermore, this section also contains information about advanced life support, once again in accordance with the guidelines of the European Resuscitation Council, which is aimed primarily at health care professionals and particularly those who carry the title of paramedic. The research section on the other hand, is based on qualitative research based on interviews consisting of 13 questions. These questions are targeted towards a research group consisting of a group of 12 paramedics employed in the South Bohemian region. This section includes an analysis of the obtained data and their evaluation. The aim of this work is to map the awareness of paramedics about the latest suggested methods for cardiopulmonary resuscitation according to the guidelines of the European Resuscitation Council and to find out how paramedics view and handle cases of sudden circulatory arrest.
The role of paramedic in care on patient with acute stroke in pre-hospital care

The bachelor thesis describes the role of paramedics in care of a patient with an acute stroke in pre-hospital care. The thesis deals with the topic of stroke, its symptoms, its diagnosis, its medical treatment, as well as the role of paramedics on site of the incident. The theoretical part describes the anatomy of the cerebral arterial circle to simplify the understanding of what happens in case of stroke. Furthermore, this part focuses on the question of ischemic and haemorrhagic types of stroke. It describes the high-risk factors, symptoms, diagnosis, neurological examination and the treatment within the pre-hospital care by paramedics. The transfer of patients suffering from a stroke into medical facility and their follow-up healthcare are introduced. Last part deals with the prognosis of the illness and the prevention of a stroke. The practical part consists of a qualitative research based on personal interviews, comprising of 27 questions. The research group is formed by 10 paramedics who have been working at Emergency Medical Services in South Bohemian Region. The analysis of collected data and the evaluation of the data are included as well. The aim of the thesis was to map the knowledge and working procedures of paramedics within the crew of paramedic services at an acute stroke.
The role of paramedic in curing pacient with acute pulmonary embolism in pre-hospital care

The bachelor's thesis deals with the issue of pulmonary embolism, especially the role of the paramedic in this issue in pre-hospital care. The theoretical part briefly describes the anatomy and physiology of circulatory and respiratory system. Furthermore, tromboembolic disease, specifically deep vein thrombosis, and especially pulmonary embolism are characterized. The etiology, a clinical picture, diagnosis and subsequent treatment are mentioned in this thesis. Then the pre-hospital care and the definition of the field of paramedics are defined, including the professional procedure in pre-hospital care and paramedic's competences. The research part was performer on the basis of semi-structured and anonymously processed interviews with paramedic from the South Bohemian Region and the Vysočina Region. The data obtained while asking the medical rescuers were analyzed from the point of view of information on a given problem, including work procedures in the occupational crew and from the point of view of the patient's re-directing and/or admitting to other departments. Results of the obtained data led to the finding that paramedies have good awareness about pumonary embolism in terms of it's definition and treatment steps. As the most problematic matter seems to be the question of the right diagnosis. The right diagnosis and transport of the patient is an important step that allows early and correct initiation of treatment. The brochure which was created as part of this bachelor's thesis could be helpful in raising awareness in this regard.
Feasibility of advance directives in paramedic care

This diploma thesis deals with feasibility of advance directives in the practice of emergency medical services. It explores how two emergency medical services deal with rising public interest in this topic - The Emergency medical service of Central Bohemia Region and of Vysočina Region in Czech Republic. The goal of the thesis is to detect under what circumstances can the EMS (emergency medical services) play a functional role in the practice of patients end-of-life decisions and in situations containing advance directives. Another goal is to make a set of recommendations for the organizations which could potentially help with dealing with advance directives in pre-hospital care. The empirical part of the study is constructed using a case study approach, it analyses organizational tools, documentation, and internal legislation. It is also made using semi structured interviews with the employees of both organizations. All acquired information is then divided under several categories that are profitable for the case study. Nursing Care in Patient with Acute Myocardial Infarction

The aim of this bachelor thesis is to identify the characteristics of prehospital and hospital care of patients with acute myocardial infarction. The theoretical part of the thesis summerizes clinical symptoms, diagnostical procedures and treatment of such patients. There is mentioned the importance of preventive steps, also. The differences in prehospital and hospital nursing care of acutelly ill patient are examined in detail. The practical part of the thesis uses data of Pilsen Region Rescue Service and analyzes the incidence of acute myocardial infarction in this area. The experience of EMS workers in accessing patients in an unfavourable social situation

The diploma thesis deals with the experiences of emergency medical workers when going to patients in an unfavorable social situation. Medical rescue service according to Act No. 374/2011 Coll. on emergency medical services, should provide pre-hospital emergency care to patients in acute life-threatening conditions. Nevertheless, the crews of the ambulance service go to patients whose problems fall more into the social field. These patients do not primarily need medical intervention, yet rescuers in the field have to deal with these situations. The aim of the diploma thesis was to find out and compare the experience of rescuers when going to patients in an unfavorable social situation. Find out how rescuers experience and interpret these situations, what situations they encounter during these trips and how they understand the concept of an unfavorable social situation. The theoretical part of this work deals with the definition of basic concepts in the field of emergency medical services and social work. The empirical part is devoted to qualitative research. It contains a description of the research methodology, analysis of interviews with rescuers and their evaluation.
