This bachelor's thesis is divided into theoretical part and practical part. In theoretical part there are defined social services and concepts related with them, there is also concretization of the target group, activities of the services according to the act mentioned above and basic description of the individual planning. In the practical part of this bachelor's thesis there was used the method of qualitative research. The data was collected using semi-structured interviews with eight social workers of the LTFCY in the Vysočina region. Main goal of this thesis was to find out what activities provided by the LTFCY are the most wanted by their clients, what institutions do they usually collaborate, and what difficulties do those social workers encounter with while creating the user's individual plan. The research questions were focused on activities of the LTFCY defined by the law and also on how the users learn about the LTFCY and on the difficulties that might the social worker meet while creating the individual plan for the client. Thanks to the interviews provided by the social workers working in the LTFCY it was possible to identify the pitfalls appearing in the individual planning with children and youth endangered by unfavorable social situation. Also, the finding of how the activities defined in the Social Services Act No. 108/2006 Coll. reflect in the goals set by the clients is very interesting. This thesis might be helpful to social workers working in the LTFCY environment and workers who create internal rules, methodology, and so on, for this kind of facilities. It could also be interesting for other people working with the same or similar target group.
The use of coordinated rehabilitation in a home with special regime
RATHOVÁ, Lucie
The aim of this diploma thesis is to identify options of how to use the systém of coordinated rehabilitation in a home with a special regime. To obtain information I used qualitative research methodology. Semi-standardized interviews served as the research tool. These interviews were carried out with 10 employees of a home with a special regime run by the City Institute of Social Services in Strakonice. The thesis is divided into a theoretical and a practical part. The theoretical part of the diploma thesis focuses on coordinated rehabilitation as a whole, further, it discusses the individual parts of coordinated rehabilitation. Other chapters describe the most frequent client diagnoses within the Home with a special regime, and finally the standards of social service quality are mentioned. The main research question: What are the options of using coordinated rehabilitation in a home with a special regime? Partial research question: Are employees informed about the options of coordinated rehabilitation? For the purposes of the interview I used questions focused on general identification data, questions focusing on the importance of coordinated rehabilitation, multidisciplinary team. Further, I asked about the individual components of coordinated rehabilitation, and finally, how individual planning is carried out. The result of this diploma thesis is a finding that individual components of coordinated rehabilitation are not fully mutually connected. It would be useful to provide employees with suitable courses to complete their missing knowledge. This diploma thesis might serve as a tool for improving care in the Home with a special regime, it might contribute to the Home with a special regime´s employer awareness of coordnated rehabilitiation, and connect it with practice.

Individual planning with people addicted to drugs in the contact center
FIALOVÁ, Barbora
The topic of this bachelorthesis is individual planning with people who are addicted to addictive substances. The main objective is to compare process of individual planning in different contact centers in the Vysočina region. The partial aim is to find out which theoretical starting points are being used in the contact centers for individual planning with addicted clients. This thesis could help widen public awareness of the contact centers and services they offer. It could also be interesting for social workers themselves, who might find some inspiration by the concept of individual planning in other contact centers. The theoretical part of the thesis is devoted, among other topics, to a description of a social worker, low-threshold facilities, contact centers and individual planning. This part also pays attention to addictive substances. The practical part of the thesis is elaborated through semi-structured interviews with six contact center workers, while these workers were selected by the method of deliberate selection. Questionnaires are structured in order to help answer the main research questions. This part also includes a comparison of individual planning with addicted clients amongworkers from different contact centers in the Vysočina Region. Based on the interviews that were processed, it was found that the number of clients per one worker differs significantly (sometimes up to twice). This is an important factor in terms of administration, which is described by almost all communication partners as an obstacle to individual planning. The research also showed that none of the communication partners uses a specific approach when working with clients. From my point of view, the surprising finding was mainly the evaluation of individual planning (mostly communication partners) as a burden while working with clients.
