Physiotherapeutic care of patient with diagnostics of the impingement syndrome

This thesis deals with the issue of physiotherapeutical care of patients diagnosed with impingement syndrome of arteculatio humeri. Impingement syndrome means an oppression or distress in the area of arteculatio humeri. That signifies an oppression of tendon m. supraspinatus in the subacromial area.It is clinically demonstrated by pain and functional limitation of movement and is caused by an overload of rotator cuff and its oppression with traumatic changes in the area of subacromial area.The most important information from kinesiology and biomechanics of arteculatio humeri are described in the theoretical part of the thesis. The possible causes of the development of impingement syndrome in different age ranges, individual stages and possible treatment methods are included too.For the research part of the thesis I used the method of qualitative research and investigative and therapeutic methods. In the kinesiological analysis I used the investigative methods according to Janda (muscle test and the test of abbreviated muscles), investigation of arthral free play according to Rychlíková, goniometric examination, visual inspection and palpation and examination of reflective changes according to Lewit. In the proposed procedures of physiotherapy the methods of soft tissues, postisometric muscle relaxation according to Lewit (including subsequent stretching), mobilization according to Rychlíková, PNF according to Kabat, respiratory physiotherapy and kryotherapy were used. I used interviews and observations to gather the data. The research file consisted of three patients who were recommended by orthopedic doctor for the surgical solution of impingement syndrome. The whole amount of time dedicated to the actual research was two months for the first two patients and three months for the third one. The patients were informed about the progress and goal of the therapy. The goal of this work was to compile the issues of impingement syndrome and describe the theoretical foundation of the area of kinesiology of arteculatio humeri.The second goal was to describe the physiotherapeutic possibilities and its practical efficiency in the research part. At the beginning of the research, the investigatory question was chosen: Which procedure of physiotherapy is efficient? The process and results rated and judged during the operational solution of impingement syndrome of arteculatio humeri were. The therapy of two of the patients was efficient. The edema, range of movement,muscular strength was modified and the respiratory stereotype and abduction stereotype was enhanced. The third patient, who was subjected to the revision of rotator cuff, did not reach the optimal results because of the short stretch time of therapy. The first six weeks of the therapy consisted of passive movements and in the next six weeks the active exercises were introduced to affect the muscular strength and interplay when moving. The results were also affected by a fall that resulted in a fracture in the Th12 area, treated in conservative way. The period of six weeks is too short to affect the quality of movement in full range in this case. Nevertheless, the clinical state was improved according to the output examination and the therapy was evaluated as efficient. The frequency of pain was lowered and the edema, range of movement in arteculatio humeri was adjusted, the muscular strength increased and the respiratory stereotype was modified. The results of research part showed that the methods used in the therapeutic part were effective and thus the goal of the research was fulfilled. The contents of this thesis are going to be available to the physiotherapeutic public. It can be beneficial in the clinical practice because it calls attention to the impingement syndrome and the possible procedures of physiotherapy.


Options of respiration physiotherapy in patients at a spinal unit

Bachleor thesis "Possibilities of the Respiratory Physiotherapy in Spinal Cord Unit Patients" deals with respiratory problems of the patient with spinal cord injury. It debates problems of the patient in general, introduces the classification of spinal cord injuries and also informs about a clinical manifestation of the injuries. It includes a comparsion of respiratory kinesiology in healthy man to a man with spinal cord injury. It also informs about common respiratory complications. The work mentions the assessment of the patient. It also shows the most frequent techniques of respiratory physiotherapy which either prevent or suppress respiratory complications. Care of ventilator-dependent patient with spinal cord injury in Czech republic

The practical part provides the first description of the Czech population of pentaplegics. Kineziotherapy in pregnancy and it´s effect on quality of life of pregnant women

This bachelor thesis deals with the topic how physical exercise can affect the life of a pregnant woman. It is divided into two parts, theoretical and practical. The theoretical part describes changes involved in gravidness, which are mainly changes of the respiratory system, the reproductive system, the cardiovascular system and last but not least changes in the psychical state of a pregnant woman. From the physiotherapeutic point of view there are described changes of the physical system caused by the increasing weight. There is also described pregnancy from the first trimester to the third one and also birth phases. One chapter is dedicated to particular kinesiotherapeutical methods like yoga for pregnant women, which is good for breath adjustment and strengthening of particular muscular sets. Another method is exercise with a gymnastic ball, which has a positive effect on the spinal column, especially on the lumbar area. The last method used is Feldenkrais' method, which is focused on relaxation and feeling the performing movement. All the methods are briefly described and particular exercises are listed in the list of enclosures. The last chapter deals with the quality of life. The bachelor thesis has one research question and two objectives. The first one is to chart possibilities of kinesiotherapy during pregnancy. The second aim is to propose and make an individual exercise plan for pregnant women. The question of research detects effects of kinesiotherapy on the quality of life in pregnancy. A qualitative research consisting of a checkup of three pregnant women was used and followed by working out patients' whole casuistries. All the patients were taught the exercise unit and they have been practising twice a week in eight weeks. During each therapy with the assistance of the author of this thesis patients were given a massage of both soles. Their os coccygeal were examined by palpation, ranges of motion in hip joints were measured by a goniometer. Respondents were also examined by inspection. Respondents answered 26 questions of the questionnaire about the quality of life in the beginning of the therapy as well as at the end of it. Anamneses of all three patients were taken in order to gain all the useful information. The practical part includes the description of the therapy duration in each one case. The research is finished by the kinesiologic analysis and by the outcome of the WHOQOL questionnaire. The research shows that kinesiology had a positive effect on all the respondents. Psychical and physical changes caused by pregnancy were eliminated and each one respondent would like to continue with the therapy. This bachelor thesis could be used as an educational and informative material for pregnant women, who do not have any experience with physical activity during pregnancy and for women, who would like to have more information. The thesis can be used as a study material for physiotherapy students and medical workers.

Physiotherapy in the pediatric rheumatology

Another major symptom is muscle weakness which primarily affects the muscles of shoulder and pelvic girdle. Children have problems with movement and breathing. It can lead to permanent imobility. This disease occurs rarely. Comprehensive rehabilitation isn't described in Czech literature. In my bachelor thesis I compared the Czech and foreign resources which refer to the rehabilitation of childrens with JDM. I describe methods of testing and possibilities of physiotherapy in various problems associated with this disease. Possibilities of physical therapy in females with muscular dysbalances of the type of the upper crossed syndrome

Upper-crossed syndrome is, according to Janda, described as a muscle imbalance in the upper part of the body, which significantly affects function of the cervical spine, the thoracic spine and the shoulder girdle. The non-physiological function and the position in these parts further influence, above all, respiratory stereotype - upper breathing - and body statics. Therapy is based on influencing the deep stabilizing system, followed by a therapy focused on the place of malfunction and the kinesiology count. Therapy of deep stabilization system complex infuences breath and postural function in their mutual coordination and it is program-controlled by CNS. Importance of close connection of breath and postural functions ensues from the fact, that respiratory muscles participate in internal stabilization with deep spinal muscles. Rehabilitation treatment at the spinal unit. Theoretical basis and practical experience

After spinal cord injury occur a lot of complication, with resulting from neurological picture. These can be central, peripheral or mix paresis in some cases even plegie. Often an autonomic nervous system can be demaged, causing complication practice in nearly all body systems. Rehabilitation attend belongs to basic care of paretic patients and is closely connected with basic with basic nursing methods from which the physiotherapy of these patients arise. This survey gives a summary of procedures and methods of physiotherapy , which help the patient to prevent and better respond to the complication treatment e.g. on respiratory and movement apparatus. Complex physiotherapeutic treatment concentrates mainly on prevention of complications. The physiotherapist must anticipate formation of complications and adapt the comlexity of the treatment.
