The professional opinion of nurses regarding the Czech healthcare system

Bachelor thesis of "Opinion about health system nurses in the Czech Republic" addresses the current issues related to the changes in the Organization and management of health care in the Czech Republic. The aim of the work was to bring the view of nurses on the existing health care system. The sisters belong to the largest group of health care workers and their views on the health care system are important for the provision of quality health care. In the theoretical part of the thesis is devoted to the attention of the health care system and its constituents (the patient, provider, and the payer) and a description of their roles, the role of the sisters, the role of the patient, health care provider roles and role of payer of health care. The empirical part contains its own investigation and presentation of the results. One of the objectives was to also compare the results with the results of own investigations previously realized researches of Sociological Institute AV.
Postgraduate medical education in Czech Republic: Young doctors captured between formal and informal institutions

This master thesis deals with the field of postgraduate medical education. The main goal of this thesis is deeper understanding of problems in the educational system. By means of a qualitative research the thesis explores the problems and causes of current postgraduate educational system from the perspective of young physicians. The research is based on a series of semi-structured interviews with physicians who are currently passing through a phase of postgraduate medical education. Results of the survey show the most questionable fact, according to participants, is the lack of the educational leadership which is supposed to be secured by the assigned supervisor. Despite the formal rules, this role is often ensured merely formally or is missing at all. Participants also mentioned difficulties related to accomplishing of specialized practical training and required procedures. Both of them are frequently subordinated to informal rules. The main findings are based on revealing the causes of problems mentioned above. Regional variability of mortality level on death causes suggestible by health care

The aim of this thesis is to map regional variability in the intensity of mortality using methods avoidable mortality and a few selected characteristics of health care. The thesis describes the development of the concept of avoidable mortality, which is then used for the Czech Republic for the period 2006-2010 and to individual districts for the period 2006-2010. Furthermore, this thesis focuses on the relationship between expenditures of General Health Insurance Company and the level of mortality in the regions. For showing regional differences depending on the health care intensity of mortality from ischemic heart disease (both acute and chronic forms) was also selected, depending on the distance of specialized medical care. The last chapter focuses on National screening programs and cancer mortality, which are integrated in a comparison of the percentage of people examine in this program for districts. Czech Healthcare Policy after 2010 in the Context of Heger's Reform

The diploma thesis "Czech Healthcare Policy after 2010 in the Context of Heger's Reform", deals with the significant institutional change that was implemented in Czech healthcare law during the right-wing government, which claimed its reforms were necessary in order to balance the budget. The aim of this work is to analyze the development of Czech healthcare policy in the period 2010-2013 with respect to the case of Heger's reform. This thesis uses different methods of analyzing the political process, for example methods of institutional analysis, critical discourse analysis, and other methods. The main theoretical basis of this work consists of a social critique of the neoliberal approach to Czech health care policy, and of a study of healthcare systems, their functions and goals, and a study of health policy and its changes. Attention is paid especially to the context of human rights related to health and the international discourse. This thesis identifies the arguments of neoliberal ideology in the policy-making of Czech healthcare policy makers in the process of justifying reforms. Specialized education of general practitioner for children and adolescents in the Czech Republic

This dissertation" Specialized education in the field of practitioners for children and adolescents in the Czech Republic " focuses on the development of specialized postgraduate medical uducation in the context of social change in the Czech Republic after 1989 and their impact on staffing in the field of health care practitioners for children and adolescents . The first part describes the theoretical and factual basis , which is closely related to the issues . These concepts explained the functioning and development of the health system, providing primary care by practitioners for children and adolescents, impact of transformation of the labor market and human resources in health care and human rights in health care. Effectiveness of coeliac disease management in the Czech Republic

This work deals with effectiveness of coeliac disease management. It is a model case of chronic disease management. The analytical framework of this work is the multilevel Innovative Care for Chronic Conditions Framework. Crucial is the interaction between health care team that represents health care system, community partners and patients with their families. In the case of coeliac disease it is necessary to include also the role of representatives of market with gluten free products. The interaction of these participants takes place in wider political context, whose state influences this interaction fundamentaly and reflects so in final results - state of patients and quality of their live. Using the evaluation of the quality of life tool SF-36, it has been ascertained, that the quality of life of coealiac patients is lower in comparison with European standard of healthy population and that the causes of the problem have to be searched and systematically removed at all levels of the system of chronic health care. The findings validate the theoretic presumption, that the chronic health care is still considerably underestimated and that it does not reply to the needs of chronic ill patients, which reflects just on general quality of life.
Approach of the Czech Health Politics to Strategy Health 2020

The aim of the thesis is the assessment of developments in the approach towards the "European" strategy Health 2020 by Czech political players while using methods of critical discourse analysis and the assessment of the development of the Health 2020 agenda during the period 2010-2012 in the context of previous activities of the World Health Organization. The approach of the Czech health care political players to the strategy had been assessed on the basis of governmental programmatic statements since 1998, the National Reform Program of Czech Republic 2011 and interviews with participants in the Czech health policy and Czech health care. The approach of Czech health policy towards the Health 2020 strategy had been evaluated as extremely problematic. The course of creation and discussion of Health 2020 in 2012 had become a critical barrier for the Czech Ministry of Health. As a result, no wider awareness of this strategy exists in Czech health policy, neither does exist a Czech translation of this strategy. Lastly, the goals of Czech health policy do not reflect in a single point of the goals of the Health 2020 strategy.
Analysis of the health care systems in Spain and Portugal

The master thesis deals with the health care systems of Spain and Portugal. The theoretical part of the thesis describes the general types of health care systems and the way they compare. In the next part the health systems of both countries are introduced with a focus on the financing, provision of services and health care reforms. The practical part of the thesis aims to analyse the relation between health expenditures and selected indicators of quality of health care in Spain and Portugal. For this purpose is used the correlation coefficient and the cost effectiveness analysis, which provides a comparison of the two countries with selected OECD countries.
Comparative Analysis of Liberal and Mandatory Health Insurance Systems (specifically the USA and Czech Republic)

The thesis aims to critically compare liberal and mandatory health systems, and based on detected advantages and disadvantages of each, propose certain solutions and recommendations to optimize the health care system. During this task it also puts into context historical, legal and ethical backgrounds which strongly influence the system set up. Optimal adjustment of the health care system is an issue that worries most of the world's countries and this thesis, with it's unbiased evaluation, proposes suggestions on potential better functionality and effectiveness of health care. The theoretical part of the thesis deals with the major economical terms connected to health care and the typology of healthcare systems. In the following practical part, the US and the Czech healthcare systems are analyzed as the representatives of the liberal and the traditional mandatory health care systems by the international comparison method and the result shows positive experiences from the systems, applicable especially for the Czech Republic.
Comparison of health care in the Czech Republic and Germany

This thesis deals with comparing the two health systems, the differences between the quality of medical care in the Czech Republic and the Federal Republic of Germany, and more specifically with the individual physician access to the patient. The thesis explains in the theoretical part the basic concepts relating to the topic of health (the value of health and its determinants, lifestyle, prevention, illness, disease, health care, health care system and health policy) further discusses the Czech and German health care system and compares them basic structure, where the most fundamental difference is the difference of the German health system, which is divided into two levels, national and federal. The thesis also studies the role of the doctor and patient and the relationship between them. Secondary and comparative data analysis of the differences in health care in the Czech Republic and Germany showed that respondents are with care and physician access to the patient rather satisfied in Germany, although there are also drawbacks. Data obtained through semi-structured interviews show that views on health care varies according to the number of years spent in Germany (respondents who lived in Germany for example for 15 years, had with the local health care more experiences). Almost all interviewees seem to agree that the difference in health care in these countries is due to the differences in funds that are spent on health care. Annually put Germany on Health care about 11, 6% of GDP, of which 76.8% are public funds. On health care in the Czech Republic it is 7.5% of GDP. Participation in the payment of health care in the Czech Republic was 15.8%, Germany 23.5%. From the data obtained suggests that the quality of care affects the confidence in it and the need to treat the patient.
