A study looking at this issue reports that the most common errors occur when a nurse administers medicines in a healthcare facility. Aim: The aim of the thesis is to monitor medication errors of nurses during the preparation and administration of medication to patients and to analyse the prevalence of medication errors, determine their type, their relationship to other influencing factors, and their severity with respect to the patient's condition. Methods: Prospective observational study with direct observation method, conducted in a specialized medical rehabilitation facility. The study included two pre- and post- intervention phases in which nurses' medication and procedural errors were monitored, analysed and evaluated. As a consequence of this fact, an ever-increasing incidence of medication errors can be expected. Medication errors and the resulting adverse events can have adverse consequences for the patient and, consequently, for the entire healthcare system. The aim of this study was to analyse the whole process of drug administration by nurses in inpatient wards in a South Bohemian hospital. Methods: This thesis is part of an ongoing research project involving the Faculty of Pharmacy of Charles University. The first data collection took place in 2021, followed by interventions towards nurses, doctors and hospital management. The data collection for this work was similarly conducted using the direct observation method for three consecutive days from 2 to 5 May 2022. They can occur at several levels - prescription, drug preparation, dispensation, administration, and subsequent use of the drug by the patient. Nurses play an important role in the process of the drug administration in a hospital setting. (english) ANALYSIS OF DRUG-RELATED PROBLEMS IN HEALTHCARE FACILITY IV. Author: Vanesa Gajdošová Supervisor: PharmDr. Martin Doseděl, Ph.D. Introduction Drug-Related Problems (DRPs) occure in lots of clinical areas.1 They are defined as pharmacotherapy related events, which actually or prospectively interfere with a therapeutic purpose.2 Aims The goal of the teoretical part of the work is to become familiar with the problem of DRPs and analyze in detail the problems of benzodiazepines and atipsychotics. The practical part of the work is focused on obtaining and evaluating of DRPs in the specific healthcare facility. Methods The data was obtained in Hamzova odborná léčebna pro děti a dospělé (Luže-Košumberk) which is the important center of rehabilitation in the Czech Republic. The audit was taking place from February 13, 2017 to February 16, 2017. The data of 94 patients from the three departments of the rehabilitation center was analyzed. The all necessary information was obtained from the medical documentation. The data was collected using the special online application and finally was statistically evaluated by the descriptive statistic. The most occurred DRPs were discussed with physicians. Implementation of palliative care in hospital setting

This thesis deals with the implementation of palliative care in hospitals and perceives the topic in context of change management in health facilities. In the Czech Republic, this type of care is still at its beginning in hospitals and its implementation is a big challenge. The aim of the thesis is to find out what factors influence the implementation of palliative care in hospitals. In the first part of the text, the theoretical bases concerning palliative care and change management are outlined. The empirical part introduces a qualitative survey identifying barriers that had to be overcome during the implementation of palliative care in hospitals and factors that were instrumental in the implementation. Within the survey, 66 interviews with members of multidisciplinary teams from 17 hospitals were made. All the hospitals participated in the Together until the end programme of the Avast Foundation. The assessment of the survey results focuses on recommendations for hospitals that decide to extend or launch the palliative attitude in their premises and on challenges for the future of palliative care in the Czech Republic.
