Use of NIC, NOC classification of patients after spinal surgery

Generally, the main topic of the thesis is the use of NIC and NOC classification of patients after a spinal surgery. The theoretical part of the thesis deals with a trauma of the spine as well as a non-traumatic affection of the spine and its symptoms, diagnostics, and treatment. Further, it focuses on nursing care of patients before and after a spine surgery. Firstly, general terms of the nursing care, such as control of vital functions, are analysed. The next part is devoted to the specifics of nursing care with respect to every segment of the spine after surgery. The next chapter of the theoretical part of the thesis describes the nursing process with emphasis on every phase thereof, and related medical documentation with its use and rules. Moreover, the respective chapter deals with the nursing diagnoses NANDA, NIC and NOC classifications and Alliance 3N. The first goal of the thesis was to find out what knowledge nurses have regarding nursing care for patients after spinal surgery. My research has revealed that the majority of nurses are well-oriented in nursing care for patient after spinal surgery, despite some of nurses' lack of knowledge. The second goal set for this thesis was to determine what are the benefits of NIC, NOC classifications for providing nursing care for patients after spinal surgery. According to the results of my research, we can conclude that most nurses consider the classification of NIC and NOC in practice rather unhelpful. This piece of information also answers the established research question: Is NIC and NOC classification according to nurses beneficial in practice? Furthermore, the following two goals: to find out what NIC interventions nurses use by patients after spinal surgery. To find out what goals of NOC classification are typically used by nurses, are contained in Tables 3-10. The result of the research question, what do nurses think about the NIC and NOC classification, showed mostly negative opinions. For another research question: How satisfied are nurses with the created nursing documentation with respect to nursing care for patients after spinal surgery, we found out that most nurses are rather dissatisfied with the submitted nursing documentation. Furthermore, two hypotheses were established: H1: Knowledge of the principles of nursing care for patients after spinal surgery are affected by the experience, was rejected. H2: Knowledge of the principles of nursing care for patients after spinal surgery are affected by education, was also rejected. Despite the fact, that NIC and NOC classifications are not used in health care services in the Czech Republic, many nurses knew them. Because of the fact that many nurses have rather negative attitude with respect to the classifications, we can assume that if the classifications were adapted to the conditions of the Czech healthcare system and there would be certain changes such as simplification or shortening of content, nurses might have a more positive approach to the classifications. Therefore, NIC and NOC classification could be part of the nursing standards of practice, as they are verified tools for providing nursing care.
Nursing process in primary care

The main aim of this bachelor thesis was to research the way how non-medical healthcare professionals carry out the nursing process at the practitioner's office. To fulfill this aim, the following research question was set: 1) "How is the nursing process at the practitioner's office realized by non-medical healthcare professionals?" Three key goals were set. The key goal G2 was to find out, how are the individual phases of the nursing process carried out at the practitioner's office, with a related research question RQ2: "How do the individual phases of the nursing process at the practitioner's office work?". Other key goal, G3, aims to map out the documentation of the nursing process at the practitioner's office. The research question for this goal, RQ3, was: "How are the individual phases of the nursing process documented?". The last key goal, G4, focuses on the evaluation of further possibilities of the nursing process in the primary care. For this assessment serves the research question RQ4: "How can the nurses realize the nursing process in the primary care to a further extent?" The research part of the thesis was conducted in the form of a qualitative research with the method of half-structured interview. The interviews with the nurses were realized according to questions which were prepared in advance. The research set consists of six nurses, who work in a practitioner's office. The interviews were recorded as an audio file and then rewritten to a text form. The results of the research were processed by the coding method and the technique "pencil-paper". The results were mapped out into four categories and then into sub-categories. The nurses in a practitioner's office carry out the nursing process. All the individual phases of the nursing process are in fact executed, apart from its second phase (setting the nursing diagnosis), which serves as an orientation for the nurse, the decision about the extent of interest in a patient was in the competence of each nurse. The documentation of the individual phases of the nursing process is conducted based on the cooperation of the practitioner and the nurse. For the realization of an extended nursing process in the primary care are important factors such as having more time for a patient, having more experience with the method of the nursing process, also the ability to provide enough healthcare staff and last but not least also more focus on the nursing problems of the patient. This bachelor thesis enables a current view on the realization of the nursing process in the practitioner's office. The found results could be published and raise an interest of the professional public in a new point of view on the implementation of nursing process in the primary care.
Use of mesuring instruments in the intesive care unit and anesthesiology and resustitation department

An integral part of nursing documentation consists in assessment tools that allow for the analysis of patients' needs and planning of follow-up care. The Bachelor thesis delves into the problem area of using measuring tools, with a focus on the Anaesthesiology and Resuscitation Departments (ARD) and Intensive Care Units (ICU). The theoretical part introduces the most commonly used scales in the framework of intensive medicine: scales for evaluating consciousness, falls, nutrition, self-sufficiency, risk of occurrence of decubiti or assessment of pain. The objectives of the present theses are 1. To map out the use of measuring tools at Intensive Care Units and Anaesthesiology and Resuscitation Departments 2. To map out nurses' awareness as regards the use of measuring tools at Intensive Care Units and Anaesthesiology and Resuscitation Departments The following hypotheses were formulated in order to accomplish the above objectives: 1: Nurses' approach to the use of measuring tools differs depending on their education. 2: The spectre of measuring tools used differs depending on work sites. 3: Nurses' awareness of the use of measuring tools differs depending on work sites. The research was carried out by a quantitative method. In the scope of the quantitative research, a non-standardized questionnaire was used and forwarded to heath care workers at Anaesthesiology and Resuscitation Departments and Intensive Care Units. Results of the quantitative research show that most of the respondents perceive measuring scales as an asset for nursing. Differences between the use of measuring tools at ARD and ICU are negligible. Respondents' approaches to measuring tools do not differ subject to education; respondents' replies are very similar. The thesis provides a comprehensive view of the problem area of the use of measuring tools at Anaesthesiology and Resuscitation Departments and Intensive Care Units. The Bachelor thesis may be serve in practice as a study material, whereas, for convenience of the reader, a brochure depicting the basic measuring tools was prepared.
Organization of the care and nursing process at the stroke center

This bachelor thesis deals with the organization of care and the nursing process at the stroke centre. The bachelor thesis is divided into a theoretical and research part. The theoretical part deals mainly with stroke, patient care at the stroke centre and associations after stroke. The main goal of the theoretical part was to describe a stroke, its treatment and nursing care by a nurse in a patient with stroke. The research part of the bachelor thesis is processed using qualitative research, for which two goals were set and one research question for each goal. The first goal of the research survey mapped the organization of care at the stroke centre. The first research question is related to this goal and examined the course of the organization of care in the stroke centre. The second set goal of the research survey was to determine the course of the nursing process at the stroke centre. To achieve the second goal, a second research question was set asking about the way the nursing process takes place at the stroke centre. Data collection was performed using semi-structured interviews with nurses working at the stroke centre. The research group consisted of six nurses. The interviews were recorded and then literally transcribed. The obtained data were processed using the coding method, the "pencil and paper" technique. The results were then categorized and evaluated. The research showed that we qualify care at the stroke centre as intensive, focusing on patients after stroke. With regard to ischemic stroke, the patient is hospitalized at a stroke centre with ongoing thrombolysis, which has been applied by the nurse at the emergency department after CT scan. In the case of hemorrhagic stroke, the patient is treated conservatively at the stroke centre. At the same time, we learned how the nursing process takes place in a patient after stroke who is hospitalized at a stroke centre. The results of the survey are used to create a basis for a seminar for nurses working at the stroke centre, which will serve to deepen their knowledge.
Prevention of complications in the treatment of patients after stroke

The thesis deals with prevention and appearance of complications with the patients after a stroke. These patients are endangered by many complications, connected not only with the disease itself but also with their health before hospitalization. For the purposes of my thesis, I chose a qualitative method. It is a specific method which uses given problem in details and gives a complex perspective of all examined criteria. I collected the data with the help of example studies which I examined further on and compared it. The results of my research show that the most frequent complications endanger the chosen patients. Further on I deal with interventions with which I intentionally prevented the complications. In the last part I evaluate the results of preventive arrangements about how the respondents percieved my treatment care.
Benefits and complications of surgical treatment of pharmacoresistant epilepsy

The aim of this thesis was to evaluate the benefits and complications of surgical treatment of refractory epilepsy. The criterion for the selection of patients was undergoing preoperative long-term monitoring. Information were obtained from six patients after 2-3 years r of surgery using the narrative interview and questionnaire QOLIE 89, which evaluates the quality of life. The interview was analyzed using grounded theory while the information obtained from questionnaire were used only as supplement and comparison of the results. Overall evaluation of questionnaire QOLIE 89 was in standard in five of six patients which is against the results obtained in interviews. Comparing risks and complications with benefits of surgical treatment of refractory epilepsy in the observed group was evaluate as problematic. Nursing process of women after laparoscopically assisted vaginal hysterectomy in the working age.

This thesis deals with the nursing process in women with laparoscopically assisted vaginal hysterectomy. The thesis is divided into theoretical part and empirical part. The theoretical part is focused on the anatomy and physiology of the uterus and female reproductive organs. Next chapter is dedicated to hysterectomy and its performace realization. The individual subsections are focused on varions ways of operating and complications of hysterectomy. The last chapter of the theoretical part describes the process of care, which is divided into pre- and postoperative period. The empirical part includes detailed nursing history of a patient after laparoscopically assisted vaginal hysterectomy in the form of a nursing case study. In conclusion, there are established nursing diagnoses and their elaboration. The work is complemented with an educational plan, which describes how the patient should be educated.
Nursing Process for a Patient with Post-resuscitation Therapeutic Hypothermia after Acute Myocardial Infarction

The bachelor's thesis deals with acute cardiology, namely with casuistry of a patient after acute myocardial infarction with successful cardiopulmonary resuscitation in the field, artificial pulmonary ventilation and therapeutic hypothermia. The thesis wants to present the highly specialised and demanding nursing care of resuscitated patients from the nurse's point of view with the use of the most modern procedures and technology. The theoretical part of the thesis is divided to several chapters describing anatomy and physiology of the cardiac muscle, acute myocardial infarction, cardiopulmonary resuscitation and therapeutic hypothermia. The main focus of this part of the thesis is on the diagnosis of the acute coronary syndrom itself. The theoretical part describes in more detail therapeutic hypothermia and nursing care of patients after cardiopulmonary resuscitation. Very important chapter is also introduction of a patient on whom is concentrated the nursing care in the practical part of the thesis, and presentation of his hospitalization. The bachelor thesis "Psychosocial and Organizational Factors in Nursing in Selected Non- Governmental Bed Care Institutions" focuses on how nurses in non-state health care institutions perceive and evaluate the working environment and the conditions in which they provide nursing care. A positive assessment of the work environment has a significant impact on the quality and safety of the care provided. The work has a common structure composed of two parts, theoretical and empirical. The theoretical part provides a brief overview of the psychosocial and organizational factors that affect nursing work. Attention is paid not only to mental and physical burden, but also to organizational factors. The empirical part includes self-examination and presentation of results. The aim was to evaluate the results of our own investigation and to compare it with the results of previously conducted studies in the United States of America, Australia, Denmark and Portugal. Nursing care of morbidly obese patients

According available studies the number of obese people is growing worldwide, we can speak about so called pandemic obesity. Obese patients can be mostly found in hospitals and it is needed to react to this fact in medical and nursing care. Subject of this thesis is nursing process of morbidly obese patient. Thesis is formed into case study, which describes in detail nursing case of patient, who has undergone stomach palsy due to third degree of obesity. All attempts for conservative treatment were tried. Research was done on clinic specialized in obesity treatment. Information for both theoretical and practical section were based on czech and internatinonal literatures and researches. In practical section interview was done with staff of the clinic and patient of the clinic. Goal of the thesis is detailed description of nursing process according functional health models by Margaret Gordon, gathering most important facts in nursing care of obese patient on specialized clinic and suggestion how to use these facts even out of the clinic. By analyzing the nursing process I found out, that nurses of the clinic follow given standards.
