Pharmacoeconomic Analysis of Direct Costs of Treatment of Migraine Disease

ABSTRACT Background: This study is primarily aimed at describing migraine illnesses, the existing patient care algorithm with this diagnosis and to estimate the direct costs of treatment of migraine from the point of view of the health care system in the Czech Republic. In the theoretical part, the basic concepts and procedures of health economics and the health technologies assessment are examined, especially the cost-of-illness of the analysis that is conducted in the practical part, namely just on the illness of migraine. Methods: The cost of drugs used to treat migraine is determined using FNHK (Fakultni nemocnice Hradec Kralove) prescription data. Further, using the data on health care (both ambulatory and inpatient care), these services are being appraised and this way the cost of treatment of a patient with the disease estimated from the point of view of the health system in the Czech Republic. Results: The average annual cost of treatment of one patient with episodic migraine was assessed at CZK 1,182 for outpatient care, CZK 786 for hospitalization and CZK 2,707 for medication. For chronic migraines, the average cost is higher and estimated at CZK 3,321 for outpatient care, CZK 2,745 for hospitalization and CZK 7,415 for medication. The popularity of alternative therapeutic method is increasing, in this case namely acupuncture, currently between 11-46% globally, 4-9,9% respectively. OBJECTIVE: The primary goal of this work is to evaluate if acupuncture has any effect on HRV in patients with migraine. Secondary goal is to find out which psychosocial factors may influance HRV parameter changes. METHODOLOGY: HRV measurment was performed before acupuncture and after 12 weeks of acupuncture therapy. At first, results were processed in KUBIOS software which is specially designed for the study of heart rate variability and the needed parameters. Physiotherapy of patients with migraine headaches simultaneously used changes in sleeping positions

This bachelor thesis deals with physiotherapy in patients with migraine headaches while using a change of a sleeping position. The first part describes functional anatomy of the cervical spine, clinical picture, types, phases, triggering factors and treatment of migraine followed by description of basic sleeping positions. The aim of the theoretical part of the thesis was to describe the clinical picture of migraine, types of migraine and its treatment, including the possibilities of physiotherapy of migraine pain. The practical part of the thesis, which was done in a form of qualitative research, can be found in the second part of the work. The examined group consists of four respondents - three patients, namely two women and one male are patients from rehabilitation center in Borovany and one woman is a patient from rehabilitation center in Žďár nad Sázavou. The semi-structured interview, observation and subjective perceptions of patients were used as the methods to collect the data. The main aim of this work was to map possibilities of physiotherapy treatment in patient with migraine headaches. Further objective was to assess the effect of physiotherapy with simultaneous change of the sleeping position. Eight therapies were completed by each patient. After the fourth session, i.e. exactly in the half of the whole therapy process, the planed change in the sleeping position was implemented. The results were assessed on the basis of kinesiology carried out before the start and after the end of the whole therapy. As patients had been aware of the severity of migraine pain, the current state was improved and the migraine occurrence frequency decreased in all cases. They all understood that doing exercise at home is crucial for positive results of therapies and decrease in number of migraine attacks. Not only exercising but also ergonomics of the movement in work environment influenced results of the therapy. No rapid changes occurred after the change of the sleeping position, which may be caused by just the short-term change in the sleeping position. However, it can be concluded that regular exercise improves function of the cervical spine and lowers the number of migraine incidence. The change in the sleeping position is considered only as an accompanying preventive factor.
Myofascial headaches causes and physiotherapy treatment option

This bachelor thesis is written in form of background research. The aim of this study was to summarize types of myofascial headaches, causes and treatment options in the field of physical therapy. This thesis focuses as well on comparsion of recent foreign scientific studies concerned with nonfarmacological treatment of headaches in general. First part of the thesis describes forms of headaches, mostly treatable with physical therapy. One chapter is dedicated to myofascial trigger points. In the other part physical therapy treatments are explained and compared individually, enumeration of physical therapy techniques, which are used and signifi cantly effect live in treatments of headaches as migraine, tension type headache and cervigenic headache. Psychological aspects of headache

This thesis focuses on the psychological aspects of headaches from different perspectives which can affect patient's life. The theoretical part is divided into several chapters. The first one is devoted to the characteristics of different types of headaches and serves as a theoretical-neurologic background for the following chapters focused on the psychological aspects as mentioned. Next chapters are a mainstay of the thesis that deal with the psychological aspects as possible symptoms of headaches; accompanying psychological phenomenas which patient may face while having a pain attack and also psychological consequences of this health problem. The last chapter is dedicated to the co-morbidity of psychiatric disorders with migraine. In the empirical part a research project exploring the correlation of personality traits and headaches is presented. Signs of the temporolimbic dysfunction in persons with headaches

The aim of this research is the occurrence of symptoms of the limbic dysfunction in patients suffering from headaches. By means of the questionnaire survey method was tested group of people with this diagnosis. The basic premise is the assumption that certain symptoms, specifically abnormalities in brain activity, occur more frequently in patients with this diagnosis. The theoretical part of this thesis concentrates on the function of the temporal lobe and the limbic system, a brief outline of the characteristics of temporo-limbic dysfunction and a summary overview of headaches. The research part is addressed to the evaluation questionnaire study and answers to the questions asked. Possibilities of physiotherapy for patients with severe headaches, specifically with migraines

This bachelor thesis deals with possibilities of physiotherapy for patiens with severe headaches, specifically with migraines. The migraines are categorizied as primary headaches, which means that it is not triggered by any other disease. It is a chronic paroxysmal disease. There are usually no symptoms between attacks. The migraine has been a very common disease recently, especially among women. Although there has been a lot of research done, causes are not completely clear. Each attack has various triggers, which are individual. I divided my thesis into two parts theoretical and special. I devoted the theoretical part to inform about the migraines and also about the anatomy of the parts of human body, which are related to the migraines. I summarize general information about migraines, migraine phases, migraine types and also triggers and the possible ways of treatment. In detail, I summarize possibilities of physiotherapy, which can be used in the treatment of migraines. I divided my findings into special techniques and special methods of physiotherapy, where we can find migraines as an indication. The special part includes goals, characteristic of respondents and methodology, in which I describe the exercise unit which I have created. The exercise unit is meant for patients with migraines as an autotherapy. I used the method of a qualitative research, which I summarize in three case interpretations. Each person has been diagnosed with migraines. From each person I took an anamnesis and performed an introductory examination. They have been introduced to the exercise unit. I added an individual therapy each session, which I chose based on the introductory examination. In the end of the therapy I performed a post-therapy examination. The research went on for three months at home. Respondents kept a record of intensity of each attack for three months before the therapy and during the therapy. After the therapy I evaluated the progress of each respondent. The research has shown that the physiotherapy has an effective influence on the migraines. However, longer therapy with the respondents would be needed to prove the real effect of the exercises. Each respondent has improved physical condition and the intensity of attacks has lowered. Based on the data I collected, I wasn´t able to conclude that the therapy would have a positive influence on the frequency of attacks. This bachelor thesis could help inform the population about the physiotherapy as a possibility of a treatment for patients with migraines. It can also be used by physiotherapists and students of physiotherapy for better orientation in this issue and it can be a pattern for how to work with patients with migraines.
Nutritional and particular food influence on migraine headaches. Case Study

Migraine affects approximately 12 % of the population in the Czech Republic, and up to 85 % of these patients stated that their migraine has some triggering factor. In order of the most common factors that cause migraine headaches. Skipping of a meal is on 3rd place; certain foods (e.g. chocolate, ice cream, cheese, tropical fruit and alcohol) are on 6th place. This study examines the influence of nutrition on migraine headaches. Research questions are whether the foods as a whole affect migraine headaches that have some foods greater effect on headaches than others and whether triggering factors from literature correspond to reality. According to findings may affect nutrition as a trigger factor as well as individual foods that have individual meanings for different respondents. The results and data from the literature on some points agreed in some parted probably due to the targeted elimination of certain triggering factors by respondents from their diet. Respondents were searching snowball sampling method. The research was conducted with forms to fill in the data retrospectively after respondent received a migraine attack and then use the forms for interview to help. Data analysis was carried out quantitatively and qualitatively the data has been written into the Excel-sheets and were evaluated as a whole with using the frequency of trigger factors. The results show that nutrition or individual foodstuff such factors play a role in triggering migraine attacks like literature presents. Thanks to the results the questions arise. What significance has oligoantigen (also called elimination) diet for nutritional therapists? And what results would be achieved for its extensive use? These questions are out of the scope of this work but it would be interesting to reflect on them more.
