Dysregulation of immune system in the patients with Common Variable Immunodeficiency

This thesis includes set of published experimental results, which were obtained at the Department of Immunology, Second Faculty of Medicine Charles University within the project focused on the basal and applied research of the Primary immunedeficiencies (PID), particularly Common variable immunodeficiency (CVID). The first, theoretical part, is divided into two sections. The first section is dedicated to the general aspects of Primary antibody deficiencies (PAD). The second section is focused on epidemiology, ethiopathogenesis, classification, clinical and therapeutical aspects of CVID. The main consideration is devoted particularly to non-infectious complications, which significantly contribute to morbidity and mortality of CVID patients. The second part consists of the set of publications describing specific mechanisms of immune system dysregulation and their clinical manifestation, which are briefly commented. The spectrum of issues resolved within the project covers following basic aspects: 1) characteristics of lung complications in CVID from the point of view of bronchial asthma, 2) characteristics of associated malignancies, 3) significance of genetic background for the specific treatment, 4) therapy of CVID focused on aspects of immunoglobulin substitution. Detection of early pathophysiological changes of breathing in children with chronic respiratory disease MD. Vaclav Koucky - Ph.D. thesis Abstract Introduction: Currently, there are different methods for infant pulmonary function testing (iPFT) and morphological assessment of microscopic changes in endobronchial biopsy samples (EBB). In research setting, they allow detection of early pathophysiological changes of breathing in small children with chronic respiratory disease, respectively in risk of its development. Their clinical significance, however, is not fully acknowledged. The aim of this thesis is to evaluate the safety, feasibility and clinical significance of iPFT and EBB in infants younger than 2 years of age. In addition, the relationship between functional and morphological changes of respiratory tract and the function of peripheral chemoreceptors was studied in selected patients' subgroups. Methods: Fifty-five infants with cystic fibrosis (CF), 35 physician-confirmed recurrent wheezers (AB), 9 infants with congenital diaphragmatic hernia, 7 with interstitial lung disease (chILD) and 3 with primary ciliary dyskinesia (PCD) were enrolled. The matters of patients foreknowledge about inhalant systems.

This bachelor thesis follows up the problems of awareness of patients about inhalation systems. It focuses primarily on who and how the patients are informed about the handeling with inhalation systems, how patients can handle with inhalation systems, which mistakes they are doing, and which inhalation system suits the patients best. It is about patients, who have bronchial asthma of they suffer from chronical obstructive pulmonary disease. The theoretical part deals with bronchial asthma and chronic obstructive pulmonary disease. It focuses on definitions of both asthma and chronic obstructive pulmonary disease. It also deals with their causes, symptoms, classification, diagnosis and treatment. In the next part, this bachelor thesis deals with are inhalation systems, types of inhalation systems, and their handling. At the end of the theoretical part is mentioned the role of the nurse in the education proces of the patient, within the proper handling of the inhalation system. The research part was processed by qualitative research. The data collection technique was semi-structured interview and observation. The interview was made up of 21 questions and the observation was written down in an observation sheet. The research group consisted of 11 patients, 4 of whom had chronical obstructive pulmonary disease and 7 patients had bronchial asthma. It came out from the research that the informations are forwarded to the patients by the doctors. Nurses are not very involved in forwarding informations to the patients, even though they have the necessary competencies to do so. Furthermore, the research had revealed that nurses spend only a few time with patients, in some cases just a few minutes. As part of a doctor's check, it became clear that nurses only control the patient's inhalation technique, only when patients are learning to handle with the inhalation system. Over time, they do not control the inhalation technique of the patients and therefore patients practice their inhalation technique wrong and make mistakes. It has emerged that the most widely used inhalation system is an aerosol dispenser, despite the fact that the powder inhaler is easier to handle. Furthermore,it became clear that patients were the most mistaken in aerosol dispensers. From the research came out, that the patients make the most mistakes, while using aerosol dispensers. The most common mistake are not holding the breath and not shaking the inhaler. With powder inhalers, patients made mistakes too. The fault was mainly in the exhalation through the inhaler, in the small force of the inhalation, and holding the inhalator in wrong position. Based on these results, there was made an helpful information brochure which will help all patients, who can not handle their inhalation system correctly.
Possibilities of sports activities in patients suffering from asthma and their effects on the course of disease.

Title: Possibilities of physical and sports activities for patients with asthma and their influence on the medical condition Objectives: The aim of the thesis is to discover the level of active lifestyle of asthmatic patients and their participation on physical activity and sport. Another aim is to find out whether asthmatic patients are limited in physical activity or sport because of the medical condition and how the physically active lifestyle influences the medical condition and whether it takes part on improving quality of life. Methods: A survey was used for data collection. A questionnaire was distributed to people working with asthmatic patients, to sports club and university students. The research group consists of 125 respondents. The obtained data was processed into tables and charts via Microsoft Office Word 2016, Microsoft Office Excel 2016 and SPSS program. A verification of hypotheses was carried out according to the results of the survey and the independence test - chi-squared test. Research results are in the form of tables and graphs summarized at the end of the thesis. Health-social aspects of patients with bronchial asthma

Current situation: Bronchial asthma is currently a civilization disease with a rapid increase in incidence and prevalence. The knowledge of its health, socioeconomic aspects can help us to predict, eliminate or support factors that will stabilize bronchial asthma. Subject: We have focused on the social situation of adult respondents with bronchial asthma associated with anti-asthmatic treatment. Objectives: According to the subject of research we have set partial objectives (to map out the current treatment of adult respondents with bronchial asthma, to map out the factors that cause the change of asthmatic problems from a health point of view, to map out the factors determining the change of asthmatic problems from a socio-economic point of view, to compare the importance of health and social factors within socially diverse groups of adult asthmatic patients, to identify, diagnose and monitor the use of social security for adult asthmatic patients in the Slovak Republic). Methodology: The following research methods were used - questionnaire, content analysis of documents, standardized asthma control test ACTTM. The obtained data were processed by qualitative and quantitative methods, fuzzy c-cluster analysis too. Research set: The set consisted of specifically selected 506 respondents aged 19 and over with a diagnosis of bronchial asthma in the pneumophysiologic ambulance of the Internal Clinic of the Faculty Hospital in Nitra from 1.5.2014 to 1.5.2015. Results: We have found that better socioeconomic conditions of respondent have positive impact on continuing with the treatment of bronchial asthma, with higher score in ACTTM. The stabilization of asthma getting worse by increased BMI. The cost of anti-asthmatic treatment was higher for obese respondents as well as any additional costs. We have confirmed that the socioeconomic situation of the respondent affects the healing costs of bronchial asthma. Treatment costs are positively reduced by lower age, higher education, higher income, psychical type of work. This shows in lower usage of hospitalization, sick leaves based on bronchial-asthma diagnoses. The stability of asthma is kept by older respondents by using spa treatments. It has been proven that the cost of treating bronchial asthma has increased due to several known allergens and risk factors. Because of impurities (dust, mites, moulds and feathers) there has been an increase in asthmatic complications. In addition, we experienced dyspnoea due to cold, humidity and seasonal changes in younger asthmatics who also were affected by air conditioning used mainly at work. With their knowledge and aimed elimination, asthmatic patients can achieve bronchial asthma stabilization as well as reduced treatment costs. Conclusion: Positive socioeconomic characteristics of asthma patients have a positive effect on their health status - control and stabilization of asthma.
Swimming training for children diagnosed with asthma bronchiale

The thesis deals with a swimming training of asthmatic children and its impact on this disease. The work is divided into two parts. In the theoretical part the focus is on the description of the bronchial asthma, causes o fit as well as on the symptoms, diagnosis and treatment. Furthermore, the importance of swimming for asthmatic children and how the water environment affects their health impairments are measured. The practical part is focused on swimming training of children who have been diagnosed with asthma. Various physiological parameters of children are being monitored during the training. These parameters include the frequency of breathing, heart rate, frequency of asthma attacks during swimming lessons from the first one to the last one. The last measured parameter is the of improvement or worsening of the conditions of swimmers (number of meters swimmed depending on time) - asthmatics, dependent on the swimming training. Acute and chronic airways disease in children: new bioindicators and molecular targets for therapy

The aim of this thesis was to contribute to the standardization and validation of expiratory flow parameters independent of NO testing, particularly alveolar concentration (CANO) and bronchial NO flux (JawNO) in children and adolescents with allergic respiratory diseases. The thesis was created in the time of increased interest of medicine professionals in non-invasive examination methods and targeted therapeutic influence of inflammation in small airways in asthmatics. After the standardization and validation of clinical benefits, the measurement of concentrations of nitric oxide (FENO) in the air exhaled through the mouth with the speed of 50 ml/s was validated for the evaluation of eosinophilic airway inflammation in patients with asthma and started to be widely used in clinical practice. Currently, there is an ongoing research focused on a new method for FENO measurement at several flow rates, which uses physiological models of NO transfer in exhaled air, allowing to quantify the amount of NO in an indirect way and to monitor the intensity of the inflammatory processes in distal and proximal airways non- invasively. Occurrence and Prevention of Asthma in Youth Division Cross-country Skiers

Title: Occurrence and Prevention of Asthma in Youth Division Cross-country Skiers Objectives: The aim of this thesis is to determine the incidence of asthma in youth division cross-country skiers and its prevention. Methods: This thesis will comprise the method of survey and content analysis of documents. Hypotheses: No.1 Assume that asthma will have lower prevalence in youth division cross- country skiers than in adult cross-country skiers. No.2 Assume that the majority of the surveyed cross-country skiing coaches met with asthmatic symptoms in their athletes. Results: We found that 12% of youth division athletes suffer from asthma and 24% from allergies. This thessis deals with the treatment of Asthma Bronchiale by using various physioterapeutic methods, especially respiratory physiotherapy. In Theoretical part there is given information about symptomatology of the disease, prevention, methods of examination, respiratory physiotherapy and devices like inhalers. The Empirical parts includes two case reports where the knowledge from theoretical part is applied. The main goal of this thessis is getting to know other methods of treatment than pharmacotherapy. And by this way influence the progress of the disease to improve quality of patients't live.
