Detekce časných patofyziologických změn dýchání u dětí s chronickým plicním onemocněním MUDr. Václav Koucký - dizertační práce Abstrakt Úvod: V současné době je k dispozici řada metod plicní funkční diagnostiky nespolupracujících dětí (iPFT) a morfologického hodnocení mikroskopických změn ve vzorcích endobronchiálních biopsií (EBB). V rámci výzkumu umožňují detekovat časné patofyziologické změny dýchání již u nejmenších dětí s chronickým respiračním onemocněním, resp. v riziku jeho vzniku. Jejich klinické využití však v této věkové kategorii není dosud plně akceptováno. Cílem předkládané práce je zhodnotit bezpečnost, proveditelnost a klinický význam těchto metod ve věkové kategorii do 2 let. Ve vybraných skupinách pacientů je též hodnocena funkce periferních chemoreceptorů a dále vzájemný vztah morfologických a funkčních změn dýchacího traktu. Metodika: Studijní soubor tvoří 55 dětí s cystickou fibrózou (CF), 35 s opakovanými obstrukčními epizodami (AB), 9 s vrozenou brániční kýlou, 7 s intersticiálním plicním onemocněním (chILD) a 3 s primární ciliární dyskinezí (PCD). U všech pacientů bylo provedeno vyšetření iPFT a zaznamenány relevantní anamnestické údaje. CF skupina byla podle věku rozdělena na CFmalí (do 6 měsíců věku) a CFvelcí (nad 6 měsíců). Vaclav Koucky - Ph.D. thesis Abstract Introduction: Currently, there are different methods for infant pulmonary function testing (iPFT) and morphological assessment of microscopic changes in endobronchial biopsy samples (EBB). In research setting, they allow detection of early pathophysiological changes of breathing in small children with chronic respiratory disease, respectively in risk of its development. Their clinical significance, however, is not fully acknowledged. The aim of this thesis is to evaluate the safety, feasibility and clinical significance of iPFT and EBB in infants younger than 2 years of age. In addition, the relationship between functional and morphological changes of respiratory tract and the function of peripheral chemoreceptors was studied in selected patients' subgroups. Methods: Fifty-five infants with cystic fibrosis (CF), 35 physician-confirmed recurrent wheezers (AB), 9 infants with congenital diaphragmatic hernia, 7 with interstitial lung disease (chILD) and 3 with primary ciliary dyskinesia (PCD) were enrolled.
