Gene expression of mutant alleles and X inactivation pattern in patients with selected X-linked disorders

In comparison to men, the number of X-linked genes is doubled in women as they have two chromosomes X while men are hemizygotes for X-linked genes. This imbalance is compensated by X inactivation (XCI) process, also known as primary X-inactivation, occurring in the early stage of embryogenesis. X inactivation is a random process and females are mosaics of two cell populations. The ratio of expressed alleles in women can be random (50:50) or skewed (≥80:20). The skewed X inactivation may occur due to selection when one of the alleles is preferentially inactivated (secondary X inactivation). In this study XCI status in heterozygous females with various severity of phenotypic symptoms and traits in selected X linked inherited metabolic diseases is analysed, with the focus being Fabry disease - the deficiency of the enzyme alpha-galactosidase A encoded by GLA gene. Moreover, XCI in one family with X linked agammaglobulinemia is examined. Level of healthy lifestyle of paramedics

Currently the total number of medical emergency drawaways is increasing. Rescue workers often face stress and physical strain, which requires their mental and physical fitness. In recent years medical rescue workers have been drawing away in medical emergency regime, thus without a doctor, which carries higher level of responsibility and therefore the rescue worker faces higher stress level. This is a non-stop profession and therefore it is highly important to heed sufficient rest and relax, both physical and mental. The aim was to discover a medical rescue worker´s healthy lifestyle standard in the regi-on of South Bohemia, how a medical rescue worker copes with stress and what way he or she relaxes and makes use of his or her leisure time. A survey of a quantity method by means of questionnaires has been placed at work. These have been distributed to medical emergency rescue workers in the region of South Bohemia. The results of the survey show that a medical emergency rescue worker´s healthy lifestyle standard of the Medi-cal Emergency of South Bohemia cannot be unambigously identified. The survey also discovered that the rescuers use mainly addictives, sport and friends to cope with stress and mostly active relaxing such as tourism or sporting activities to relax. In practice the survey shows that the management of the Medical Emergency could consider placing teambuilding activities and other company events.
Dispensing medication to the ICU and ARU within the scope of nursing practice

v anglickém jazyce: This bachelor thesis focuses on the issue of application of infusion therapy in ICU and ARU. Infusions has become an integral part of daily routine and complex medical care of patients on the critical care unit. The objectives of the thesis included the evaluation of the level of knowledge of nurses in ICU and ARU, finding potential errors and pitfalls in the preparation of infusion and then identifying effective interventions to eliminate them in nursing practice. The theoretical part focuses on the characteristics of critical care unit and intensive demands that are placed on nursing staff. In other chapters are listed all the aspects related to infusion therapy. In the empirical part is included in the research methodology, data collection process and the final interpretation of the results of subsequent analysis and discussions. Problems of patients with long-term cardiac support

Bachelor thesis is divided into theoretical and empirical part. The theoretical part is devoted to heart failure, given that it is patients with end stage heart failure is a long-term cardiac support implemented. It is described surgical treatment of heart failure and various cardiac support. A separate section is devoted to long-term cardiac support-Heart Mate II. Conclusion The theoretical part is devoted to the specifics of treatment of patients with established long-term cardiac support. The empirical part of the paper maps out the problems of patients with established long- term cardiac support. The method used to implement the survey questionnaire. The studied group consisted of 21 patients with established long-term cardiac support in outpatient care. Reconnaissance survey conducted in the outpatient department of heart failure IKEM in Prague. The work shows that although the introduction of cardiac support for patients and mentally demanding affects their daily life, most of them feel improvements in your health and well-established cardiac support perceived as a chance to wait for a heart transplant.
The composition of the team at the intensive care and its optimalization.

The thesis is devoted to the issue of the number of nursing staff at the intensive care unit. The theoretical part deals with general characteristics of the team, which also specify the environment of nursing at the intensive care unit. The specification concerns with the representation of the individual nursing professions, their education and jobs skills. I try to map out legal and non - legal regulations determining or recommending the number and the composition of nursing staff at the intensive care and resuscitation units. The last chapter of the theoretical part is concerned with the issue of optimalization of the nursing team. The practical part is focused on analyzing data collected from the research on the topic mentioned above. To obtain the data I chose the method of quantitative research using a structure questionnaire. The aim of the practical part is to determine consistency or inconsistency between the legal and non - legal rules and their implementation in practice.
Theoretical questions of edition of Old Czech Prokopian legend: with a proposition of edition of "Život sv. Prokopa"

Thesis deals with the textological-editorial issue of Old Czech texts, to be resolved at The Old Czech Passional material, focusing on the legend of St. Procopius of Sazava. The theoretical part deals with basic textological questions: the choice of the original text of the edition, the choice of type and form of the edition etc. Part of theoretical training is also to evaluate the past editions of the Old Czech prosaic Prokopian legend and to set them in the context of development over the publication of works of Old Czech literature. The practical part applies the theoretical findings in the preparation of critical edition of Prokopian legend, taking into account the specific issues of The Old Czech Passional. Part of the proposed edition is a detailed editorial report containing description of the graphic and language of existing text sources (manuscripts and prints), principles of transcription, a list of corrections of the original text, a list of linguistic and factual explanatory notes and a list of different text version of variant text sources. Thesis is given a literary-historical performance Prokopian hagiography issue and is accompanied by a list of sources and editions of Prokopian legends and numerous secondary literature.
Anesthesiological diffíeulties arising in the surgical treatment of chronic pulmonary hypertension

Pulmonary hypertension is a serious syndrome with very unfavorable prognosis. It encompasses numerous diseases and the only one which is surgical ly treatable is chronic thromboembolic pulmonary hypertension. Pulmonary endarterectomy (PEA) is a curative method for select patients with surgically accessible thrombotic obstruction. It involves not only the operation itself, but also complicated diagnostics, preoperative preparation, and sophisticated postoperative intensive care. According to contemporary world reviews of professional literatuře, mortality rates following PEA, range from 5 to 24 percent. Better results with lower levels of mortality as well as morbidity are unambiguously proven in the centers with more experience where the highest numbers of such surgeries are undertaken. Good team work is an essential condition for a successíul PEA program and the anaesthetist is its indispensable member. Anaesthesia of patients with chronic right ventricle dysfunction undergoing cardiac surgery using extracorporeal circulation brings about several diffíeulties and we tried to deal with them in our work. This kind of operation was not available in the Czech Republic and the íirst goal of our work was to implement this surgical program in its complexity - including anaesthesia and intensive postoperative...
