Cervical spondylotic myelopathy: development of clinical symptoms and surgical management

Based on contemporary theoretical knowledge in this prospective study we outline the relationship between a chosen surgical approach (anterior or posterior approach) as it relates to the localization of spinal cord lesion (anterior or posterior spinal pathways) assessed by the use of evoked potentials (SEP, MEP) and the effect of this approach on the postoperative state of patients with cervical spondylotic myelopathy. Furthermore we evaluate clinical outcome of these patients according to several aspects of their MRI and X-ray findings. The study, from 2006 to 2010, comprised 65 patients with clinical signs of cervical myelopathy. These patients had been indicated for surgery, which subsequently was performed by using either the front (anterior - a) or back (posterior - p) approach. The patients were assessed using Nurick and mJOA scores before surgery, then at 12 months, and finally 24 months after surgery. In addition, they were preoperatively examined with a battery of evoked potentials (EP) - somatosensory evoked potential (SEP) and motor evoked potential (MEP) tests. Background Epidural fibrosis (EF) is defined as nonphysiological scar formation, usually at the site of neurosurgical access into the spinal canal, in intimate vicinity to and around the origin of the radicular sheath. From the very onset, EF behaves as a reparative inflammation causing, as a rule, symptoms of characteristic nature and clinical course (pain). Treatment of epidural fibrosis causing failed back surgery syndrome (FBSS) by neuromadulation technique is very expensive. Finding of suitable parameters for the indication of treatment is therefore very important. Aims The study is based on evidence of the importance of epidural fibrosis for the development of chronic pain. Research is also focused on the comparison of the range fibrosis and the effect of stimulation (spinal cord stimulation - SCS). The goal is to find a suitable selection factor for the indication of neuromodulation. Methods I. Sliding and Lifting Platform

This thesis describes a project of a mobile stage device that will be used for a concert tour of Lucie. The first part characterizes the given task and expounds description and function of the device itself. The second part focuses on the design concept of supporting frames. This is followed by design and calculation of all mechanisms, actuators and construction of individual moving parts of a sliding and lifting platform. The last part verifies stability of the whole construction in accordance with appropriate standards. The thesis also contains technical documentation, which is to be found in appendix.

Electromechanical alternative of linear hydraulic tractive unit

In this diploma thesis is accomplished description of existing condition of decorating linear tractive unit. Analysis of safety requirements for linear tractive unit is also accomplished. Eligible construction concept of frame of electromechanical linear tractive unit is chosen. Selection and calculation of ball screw and nut is made also with draft of its eligible support . Choose of eligible engine and clutch for gearing of engine torque from engine to the ball screw is done too. Further is designed eligible concept of gearing of the rotary motion of ball screw to straight motion of bottom pulley block, which now interacting with upper firm pulley block provides lifting of tensile rod with coulisse.
