Analysis of the readiness of hospitals with an acute inpatient care in the Pardubice region in the event of a direct threat of CBRN substance to the Pardubice regional hospital.
KRATOCHVÍLOVÁ, Martina
This work focuses on finding state of preparedness of hospitals with acute inpatient care in the Pardubice region in the case of a direct threat to the Pardubice Regional Hospital, Inc. (hereinafter "PRH") by chemical, biological, radiological or nuclear agents (hereinafter referred to as "CBRN"). The thesis is based on a comparison between the trauma plans that have been established as an indicator of a preparedness of hospitals in case of mass disaster and the legal standard, i.e. Decree No. 101 of 2012 Sb. dealing with the details of the content of trauma plan of the one-day or inpatient medical care and process for its preparation and negotiation, as amended (hereinafter the "Decree 101 of 2012"). The theoretical work describes the applicable laws governing the conditions for the provision of health care, trauma plan content and procedures of the Emergency Service (ES) components in preparation for dealing with emergencies. The next part discusses the characteristics of CBRN agents. Furthermore, the work focuses on defining terrorism and CBRN terrorism. The chapter also includes a brief historical summary of the use of CBRN agents and their threats. The following section describes the Security System of the Czech Republic and its elements. It is crucial to mention its existence in relation to the prevention of emergencies and crisis situations and elimination of their consequences. This part also introduces a selection of documents, which have a direct relationship to the security policy of the country and to this thesis. It may be noted that most of these documents have been prepared, inter alia, on the basis of current CBRN threats. The penultimate section characterizes individual hospitals with acute inpatient care in the Pardubice region and includes also a risk analysis of this region. The analysis is based on the Emergency Plan of Pardubice region. I believe that the knowledge of the possible threats pointed out in the risk analysis is important in a preparation for an emergency response in the relation to an emergency medical care. Analysis of risks and threats, based on the Decree 101 of 2012, is also a part of the trauma hospital plans. The last part of the thesis describes the different types of trauma plans and certain procedures in the process of an activation of these plans. Even though, the theme of the work is focused more on trauma plans of hospitals, in cooperation with ambulance services and regional authorities in addressing the consequences of emergencies, I find it important to pay attention to a trauma plan for emergency medical services, and also the one, which is part of the emergency plan of the region. Based on the practical experience it was decided the readiness of hospitals with acute inpatient care in the Pardubice region in case of a threat PRH is sufficient. Subsequently, a relative comparison of interdependence trauma plans was performed in selected hospitals to evaluate their index of readiness by means of comparative analysis. The overall index IPi scales the state of preparedness of the hospitals. The input parameter?s values for the index evaluation were determined based on the contents of the trauma plans. The concluding chapter presents the results evaluated by the described method. This uncovered that the degree of readiness of the examined hospitals, based on the mentioned parameters, was insufficient in three cases. Only Chrudimska Hospital, Inc. achieved satisfactory results. Based on these results, the specific measures were recommended to be taken the level of preparedness of the hospitals. The findings and the results of this thesis are mainly practical. The most important benefit of this work is the suggested measures for the activation of trauma plans for hospitals with acute inpatient care in the Pardubice region.

GRECHOVÁ, Michaela
The topic of this thesis is Tactical attitudes to the person affected by abnormal events - psychological aspects; it deals with the psychological aid to persons affected by abnormal events. The theoretical part of the thesis focuses on defining the basic applied terminology and its target which is: ? Mapping the present methods of psychological aid during the timely care of persons affected by abnormal events in the Czech Republic. The practical part has two goals: ? Mapping the present state and the possible development of psychological aid provided in connection with abnormal events from the viewpoint of psychologists of individual components of the Integrated Rescue System (IZS). ? Mapping the needs, values and strategies of mastering the situation by psychologists of IZS components in view of abnormal event In connection with the research target, 6 research questions were formulated. The research part of the thesis is focused on psychologists of the main components of IZS. It tries to summarize their experience, opinions, standpoints, reservations, and also needs and strategies of mastering the situation in connection with providing psychological aid in the early stage after the abnormal event. The qualitative research is implemented in the thesis by the method of questionnaire investigation with 16 open questions. Data collection was done in the form of a phone talk with several respondents, questionnaire method is combined with the method of structured talk. The investigated sample consists of 11 respondents ? psychologists of the main IZS components. 7 respondents were firemen (HZS ? Fire Rescue Service), 3 policemen (CR Police) and one psychologist of ZZS (Medical Rescue Service). The answers of individual respondents were recorded and the complete transcriptions of answers are stated in Annexes to thesis. The acquired data were interpreted by the author in the way representing the answers to research questions. To achieve a well arranged result, this interpretation was summarized into several points from which the results of research follow which are discussed in more detail in the following chapter. It was found out in the answers to research questions: ? What drawbacks are seen by IZS psychologist in the present system of providing psychological aid to persons affected by MU (abnormal events), ? Which aspects of the psychological aid provided to persons affected by MU are considered for well-functioning by the psychologist ? What possibilities of future development are seen by IZS psychologists in the field of psychological aid to persons affected by MU ? According to IZS psychologists, by what factors are limited other targets and development of the psychological aid to persons affected by MU ? What values, needs and mastering strategies and sources are applied by psychologist in connection with interventions during and after abnormal events ? What methods of psychological aid are applied by the addressed IZS psychologists during the early care of persons affected by MU? The chapter Discussion contains a more detailed analysis of results. During the research performance, other not formulated questions occurred concerning not completely clear or not defined border between the psychological and psychosocial aid which the thesis tries to determine. It may be stated in the end that the research questions were answered in the practical part of thesis. The research questions were formulated in such a way that the answers on them represent the base for SWOT analysis of the system of providing psychological aid by IZS components. I believe for this reason, it may be utilized not only for study purposes.

The second chapter is devoted to disasters of the 21st century and is divided into two parts. The first part of the second chapter deals with disasters of the 21st century in the world, including the current situation in Japan and the other part introduces major disasters of the 21st century in our country. The third chapter of the theoretical section focuses on the characteristics of basic disasters that may occur in the Czech Republic. The fourth chapter informs us about the impact and consequences of extraordinary events and crisis situations in the Czech Republic and about their solutions. This chapter introduces us to the way the Czech Republic addresses social impacts of disasters, which are an important factor in determining the seriousness of disasters. The concept of social impacts of disasters includes not only the loss of life or property, but also reduced ability of self-support of people affected by disasters caused by closing down businesses and their decreased work ability. The last chapter of the theoretical section, chapter five, deals with the role and responsibilities of state authorities and local governments during extraordinary events and crisis situations. This thesis aims at specifying social impacts of disasters and to propose some solutions. The results of quantitative research using questionnaire survey showed that we can confirm working hypothesis no. one, i.e. that one of the major impacts of disasters are social impacts. The thesis also includes research conducted using the method of guided interviews thanks to which we can confirm the second working hypothesis, i.e. that dealing with the impact takes place mostly in the reconstruction period after the crisis situation. Knowledge obtained while working on this thesis may serve as a comprehensive analysis of the current state of the Czech Republic in terms of dealing with social impacts of disasters.
