Case Study of Physiotherapeutic Treatment of a Patient after Total Knee Arthroplasty

Michaela Stupková Title: Case study of physiotherapy care for a patient after total knee arthroplasty Objectives: The aim of this thesis is to elaborate theoretical knowledge regarding total knee joint endoprosthesis and discuss its treatment from the prespective of physiotherapy. At the same time, elaborate the course of physiotherapy care for the patient with this diagnosis. Methods: The thesis was divided into a theoretical and a special part. The theoretical part is devoted to information and knowledge about the diagnosis of total knee arthroplasty. In a special part, a case report of physiotherapy care of a selected patient from concurrent professional practice at the Nemocnice Vršovice in Prague was recorded. In a special part, the initial kinesiology examination, the course of 9 therapeutic units with the patient, the final kinesiology examination and finally the effect of the therapy is evaluated. Results: The patient's physiotherapy care was without complications. During 9 therapies, the patient's condition improved. More detailed results of the therapy are written in the chapter entitled evaluation of the effect of the therapy. Title: Case study of physiotherapeutic care for a patient diagnosed with total knee arthroplasty. Aims: The aim of this bachelor thesis is the theoretical elaboration of osteoarthritis issues focusing on the knee joint and the indication for total knee arthroplasty, as well as the processing of a case study of physiotherapeutic care for a patient with this diagnosis and the evaluation of the therapy provided. Methods: The general part was elaborated through a review of theoretical foundations regarding osteoarthritis issues and the indication for total knee arthroplasty. Theoretical aspects covered in detail include the anatomy of the knee joint, definitions of osteoarthritis, its pathokinetic consequences for movement, clinical examination, osteoarthritis treatment, indications for total knee arthroplasty, as well as physiotherapeutic methods and procedures used for this diagnosis. The special part includes a case study of patient M.S. diagnosed with status post-implantation of total knee arthroplasty of the left knee joint. Methods: In the theoretical part of the thesis, I summarize the facts regarding the basic anatomy of the knee joint and its kinesiology. I present the topic of gonarthrosis, total knee arthroplasty and rehabilitation. A special part of the work includes a case study of a patient, which was carried out at the Beroun Rehabilitation Hospital between January and February 2024. At the beginning of the therapy, the patient's medical history was taken and an initial kinesiological examination was carried out, on the basis of which short-term and long-term rehabilitation goals were developed. The special part of the bachelor thesis is prepared in order to keep a case report of a patient after knee TEP implantation and to record the effects of selected physiotherapy care procedures. The theoretical part deals with a literature search to understand the structure and function of the knee joint from the perspective of anatomy, kinesiology and biomechanics. A case report of physiotherapeutic care of a patient after total knee arthroplasty

The bachelor thesis is focused on the issue of total knee arthroplasty (TKA). It is divided into two main parts: theoretical and methodical. The theoretical part deals with basic anatomical and kinesiological properties of the knee joint, the problem of knee osteoarthritis, and TKA as an effective method for addressing its final stage. The methodical part of the thesis is conducted in the form of a case study of a patient who underwent physiotherapeutic care after TKA implantation. During the physiotherapeutic intervention, methods and approaches corresponding to the knowledge acquired in the bachelor's degree program of physiotherapy were utilized. Thanks to the applied physiotherapeutic care, improvement in the functional status of the patient was observed in almost all monitored criteria.

Methods: The bachelor thesis consists of a theoretical and a special part. The theoretical part contains general information about the knee joint and the given diagnosis, as well as examination and physiotherapy methods and procedures used for the given diagnosis. The special part contains a case study of a patient after a subtotal rupture of the anterior cruciate ligament, which consists of an initial kinesiology analysis, an output kinesiology analysis and 8 therapeutic units. I processed the special part during continuous professional practice, which took place at the Beroun Rehabilitation Hospital. Results: If we compare the results of the entrance and exit examinations, we will see an improvement in most aspects. Title: Case study of Physiotherapy Treatment of a Patient after Implantation of Total Knee Joint Endoprosthesis. Objectives: The aim of this bachelor thesis is to elaborate a case study of physiotherapy treatment of a patient after implantation of total knee joint endoprosthesis and to develop theoretical basis for this diagnosis, applying them in therapy. Methods: This bachelor thesis is divided into two parts, general part and special part. General part describes problematics of gonarthrosis, indications for total knee joint endoprosthesis and and it's surgical procedure. Lastly, the general part is aimed at rehabilitation and special physiotherapeutetic procedures for patients with total knee joint endoprosthesis. Special part describes a case study of a patient with total knee joint endoprosthesis and is divided into individual therapies, initial examination and exit examination. The use of suspension systems for physiotherapy in patients after TEP of the knee joint

This bachelor thesis focuses on the utilization of suspension systems post total knee replacement. The aim of the thesis was to explore the possibilities of physiotherapy using suspension systems in patients after total knee replacement, propose an exercise unit within the Redcord system, and evaluate how exercising in this system influenced individuals' conditions. The thesis is divided into theoretical and practical parts. In the theoretical part, I summarize knowledge about the anatomy and kinesiology of the knee joint. Additionally, I describe osteoarthritis and its surgical solution - total knee replacement. Finally, in the theoretical part, I mention the possibilities of rehabilitation after total knee replacement and information about the Redcord suspension system. The practical part was conducted in the form of qualitative research using case studies. Two patients aged 55 and 70 after total knee replacement participated in the research. Each case study consists of a kinesiological analysis, including anamnesis, examination of scar, standing and walking, anthropometry, goniometry, muscle testing, and examination of the initial position in the Redcord system. It also included descriptions of six therapies and proposals for shor - term and long - term rehabilitation plans. An initial and final kinesiological analysis was conducted for each case study, which was subsequently compared. The results show that patients experienced increased muscle strength, improved range of motion, increased overal fitness, and strengthened deep stabilization system. Therefore, exercising in the Redcord system has a positive impact on the condition of patients after total knee replacement. This bachelor thesis can serve as a useful source of information for therapists or clients interested in utilizing Redcord for exercise.
Biomechanical studies of knee jointarthroplasty

The presented diploma thesis deals with stress-strain analysis of the intact human knee joint and the knee joint after reconstruction using fixation plate. Finite element method is used to determine the biomechanical response of the knee under loading with the FE software Ansys. The three-dimensional finite element model is consisted of bony structures (femur and tibia) which were created using computed tomography (CT) data, articular cartilages and both menisci. Model of the locking plate (LCP) was created based on producer's technical guide. Three models of the knee joint with LCP fixation plate are created for three different sizes of condylar fragment based on the intact model. On all variants is applied loading corresponding to maximum loading in a gait cycle. The results show that reconstruction of the knee joint with locking plate has no effect on changing in contact pressure on tibial plateau.
