Abstrakt Disertační práce pojednává o rozdílných metodických přístupech detekce různých genetických alterací u tří typů solidních nádorů: štítné žlázy, kolorekta a slinivky břišní. Moderní onkologický výzkum se na znalosti somatických genetických alterací v nádorovém ložisku zakládá. Tato znalost, krom toho, že vědcům poskytuje představu o příčině vzniku nádorů, má největší přínos v tom, že může významně ovlivnit léčbu konkrétního nádoru, respektive pacienta. Specifické genetické alterace dokážou totiž předpovědět prognózu pacienta nebo umožňují využít k terapii molekulárně cílenou léčbu, tzv. biologickou léčbu, nejčastěji ve formě inhibitorů onkoproteinů vzniklých expresí mutovaných genů. Další úrovní je pak využití konkrétních genetických alterací pro detekci tzv. volné nádorové DNA (ctDNA, z angl. circulating tumor DNA), která se vyskytuje v krvi onkologických pacientů. Analýza ctDNA v periferní krvi pacientů, označovaná také jako "tekutá biopsie", je aktuálně velmi atraktivním přístupem, který umožňuje neinvazivní sledování průběhu léčby pomocí stoupajících či klesajících hladin ctDNA, a to prakticky kdykoliv je potřeba. V této práci se zaměřuji pouze na nejdůležitější poznatky a výsledky získané během svého působení v onkologickém výzkumu solidních nádorů, které dokládám příslušnými... Abstract The dissertation discusses different methodological approaches for detection of various genetic alterations in three types of solid tumors: thyroid, colorectal and pancreatic. Modern oncological research is based on the knowledge of somatic genetic alterations in tumor. This knowledge, besides providing scientists with an idea of the cause of the formation of tumors, has the greatest benefit in that it can significantly influence the treatment of a particular tumor or patient. Specific genetic alterations can predict the prognosis of a patient or enable the use of molecularly targeted treatment, so-called biological treatment, most often in the form of inhibitors of oncoproteins produced by expression of mutated genes. The next level is the use of specific genetic alterations to monitor the so-called circulating tumor DNA (ctDNA) that occurs in the blood of oncological patients. The investigation of genetic alterations from the blood, also referred to as "liquid biopsy", is currently a very attractive approach that allows non-invasive monitoring of the course of treatment using rising or falling levels of present genetic alteration, virtually whenever needed.
