Kardiovaskulární onemocnění jsou podle statistických údajů Světové zdravotnické organizace hlavní příčinou úmrtí dospělých v České republice, podíl zemřelých v jejich důsledku činil v roce 2013 až 47 %. Zahrnují úmrtí na ischemickou chorobu srdeční, cévní mozkovou příhodu a ischemickou chorobu dolních končetin. Epidemiologické studie ukazují v celosvětové populaci strmý vzestup incidence a prevalence cévních mozkových příhod, které jsou hlavní příčinou invalidizace pacientů středního a vyššího věku. U diabetiků 2. typu jsou tyto příhody z velké části kardioembolizační etiologie, často probíhají pod obrazem asymptomatických paroxysmů fibrilace síní. Cílem naší práce bylo porovnat výskyt rizikových faktorů a výskyt cévních mozkových příhod u diabetiků 2. typu sledovaných v ordinacích praktických lékařů oproti jejich výskytu u nemocných v dispenzární péči odborného centra. V obou centrech jsme zachytili stejný výskyt hypertenze, oproti výskytu dyslipdémie, který byl signifikantně vyšší ve sledovaném souboru. V tomto souboru převažovala u nemocných s diabetes mellitus 2. typu léčba perorálními antidiabetiky, léčba inhibitory angiotensin konvertujícího enzymu, a léčba statiny. According the World Health Organization are cardiovascular diseases the main cause of death among adults in the Czech Republic, in 2013 their share was nearly 47%. They include deaths from coronary heart disease, stroke and peripheral arterial disease. According to the results of epidemiological studies, the incidence and prevalence of cerebrovascular strokes in the global population is rapidly increasing, they are the main cause of disability in patients middle-aged and elderly. These strokes have very often cardioembolic cause by patients with type 2 diabetes mellitus, they often take place due to asymptomatic paroxysms of atrial fibrillation. The aim of our study was to compare the incidence of risk factors and the incidence of cerebrovascular strokes in patients with type 2 diabetes mellitus, between those, who are observed by general practitioner, to their incidence in patients in dispensary care in specialized centers. In both centers we found the same prevalence of hypertension, compared the prevalence of dyslipidemia, which was significantly higher in the observed group. The treatment of oral antidiabetic treatment in patient with diabetes mellitus type 2 was higher in this file too, as well as the treatment with angiotensin converting enzyme inhibitors and treatment with statins.
