Klára Horáčková Disertační práce: Charakterizace hereditární komponenty u vysoce rizikových pacientek s gynekologickými tumory ABSTRAKT Nádory ovaria (OC) jsou celosvětově u žen zodpovědné za téměř 5 % všech úmrtí způsobených nádorovými onemocněními. Více než polovina OC je diagnostikována v pokročilých stádiích spojených s nepříznivou prognózou (s pětiletým přežitím kolem 50 %). Průměrný věk v době diagnózy se pohybuje okolo 65 let. OC je charakteristické velkým podílem hereditárních tumorů; germinální patogenní/pravděpodobně patogenní varianta (GPV) v nádorových predispozičních genech je identifikována u více než 20 % pacientek. Z těchto pacientek však můžeme vyčlenit specifickou podskupinu s diagnózou OC s časným nástupem (<30 let), u kterých nacházíme atypicky nízký podíl hereditárních forem se záchytem GPV pod 10 %, a tedy nejasnou příčinou vzniku tohoto onemocnění. Předložená disertační práce se zabývá komplexní genetickou analýzou u pacientek s OC s časným nástupem s využitím celoexomového sekvenování na úrovni jak DNA, tak i RNA doplněného o analýzu polygenního skóre rizika (PRS) a typizace lidských leukocytárních antigenů (HLA). Ve skupině 123 pacientek s diagnózou OC do 30 let věku jsme identifikovali pouze 6 (4,9 %) nosiček GPV ve známých OC predispozičních genech. Výsledky dalších... Mgr. Klára Horáčková Dissertation thesis: Characterisation of hereditary factors in high-risk patients with gynecologic cancers ABSTRACT Ovarian cancer (OC) accounts for less than 5% of all cancer-related deaths in women worldwide. More than half of OC patients are diagnosed at an advanced stages with an unfavorable prognosis (with a 5-year survival rate of around 50%). The mean age at diagnosis is around 65 years. Characteristics of OC include a high proportion of hereditary cancers - a germline pathogenic/likely pathogenic variant (GPV) in cancer predisposition genes is identified in more than 20% of cases. However, we can distinguish a specific subgroup of early- onset OC patients (<30 years), in whom an atypically low proportion of hereditary forms is identified with a GPV detection rate in established OC predisposition genes falling below 10%. Thus, the genetic predisposition to early-onset OC remains unclear. The hereby presented thesis focuses on a comprehensive genetic analysis of early-onset OC patients using whole-exome sequencing at both DNA and RNA levels, complemented by polygenic risk score (PRS) analysis and human leukocyte antigen (HLA) typing.
