Úvod: Značné množství pacientů trpících depresivní epizodou (unipolární nebo bipolární) nedosáhne úlevy ani při adekvátní farmakologické nebo psychoterapeutické léčbě. Ke zlepšení výsledků léčby je na místě hledání nových terapeutických přístupů nebo klinicky relevantních prediktorů odpovědi na antidepresivní terapii. Neinvazivní neurostimulační metody (NIBS), jako je repetitivní transkraniální magnetická stimulace (rTMS) nebo transkraniální stimulace stejnosměrným proudem (tDCS), jsou v posledních desetiletích předmětem klinického a výzkumného zájmu na poli léčby deprese. Údaje srovnávající vzájemně tyto dvě neurostimulační techniky jsou však relativně nedostatečné. Časná změna prefrontální theta kordance se zdá být slibným EEG prediktorem antidepresivní odpovědi. Cílem této práce je prozkoumat a porovnat účinnost rTMS a tDCS v akutní léčbě deprese a přinést další informace o klinické významnosti časné změny prefrontální kordance jako prediktoru odpovědi na antidepresivní léčbu. Metody: Ve studii 1 (n = 57) jsme porovnávali antidepresivní účinek 4týdenní terapie tDCS oproti farmakologické léčbě venlafaxinem (VNF) u pacientů trpících unipolární depresí. Non-invasive neurostimulation methods (NIBS) like repetitive transcranial magnetic stimulation (rTMS) or transcranial direct current stimulation (tDCS) are the subject on clinical and research field on the treatment of depression in past decades. There is paucity of data comparing these two NIBS techniques. Early change of prefrontal theta cordance seems to be a promising EEG predictor of antidepressant response. The aim of this thesis is to explore and compare the efficacy of rTMS and tDCS in the treatment of depression and add more information on the predictive value of prefrontal cordance change on antidepressant treatment. Methods: In Study 1 (n = 57), we compared the antidepressant effect of 4-week course of tDCS vs. pharmacological treatment by venlafaxine (VNF) in patients suffering from major depressive disorder.
