Úvod: Cirkulující nádorové buňky (CTC) a diseminované nádorové buňky (DTC) jsou zodpovědné za rozvoj metastazování nádorů. Detekce CTC z periferní krve (tzv. liquid biopsy) může přispět k diagnostice a volbě terapie u pacientů, u kterých není možné získat vzorky nádorové tkáně při chirurgickém zákroku. Obdobně monitorování efektu chirurgické léčby pomocí CTC a záchyt časné rekurence nádorového onemocnění představují další možnosti jejich využití. Cíle: Cílem práce bylo izolovat CTC/DTC z krve či peritoneální laváže pacientů s různými druhy solidních tumorů a následně se pokusit o jejich kultivaci in vitro a cytomorfologickou charakterizaci. Práce měla zodpovědět následující hypotézy: I. Vyskytují se CTC u různých druhů solidních tumorů? II. Odráží se ve výskytu CTC stadium nádorového onemocnění? III. Lze využít CTC k molekulární charakterizaci nádorového onemocnění? IV. Je možné vizualizovat CTC/DTC ve viabilním stavu? Metodika: CTC/DTC detekce proběhla celkem u 288 pacientů v průběhu chirurgické či systémové léčby: u 24 s karcinomem pankreatu, 165 s karcinomem prsu, 98 s kolorektálním karcinomem a u 1 s neuroendokrinním tumorem (NET) tenkého střeva. Pro obohacení CTC byl použit separační protokol založený na velikosti buněk. Po jednoduché filtraci krve přes polykarbonátovou membránu s póry byly... Introduction: Circulating tumor cells (CTC) and disseminated tumor cells (DTC) are responsible for the development of metastasis. Detection of CTC from peripheral blood (so- called "liquid biopsy") may contribute to the diagnosis and choice of therapy in patients in whom it is not possible to obtain tissue directly from the tumor. Monitoring the effect of surgical treatment and detecting early recurrence of cancer are other potential uses for CTC. Aim: The aim of our work was to isolate CTC/DTC from blood or peritoneal lavage of patients with various types of solid tumors and then try to cultivate them in vitro and to describe their cytomorphological characteristics. We targeted the following hypotheses: I. Do CTCs occur in different types of solid tumors? II. Does the presence of CTC reflect cancer stage? III. Is it possible to use CTC for molecular characterization of cancer? IV. Is it possible to visualize CTC/DTC in a viable status? Methods: CTC/DTC detection was performed in a total of 288 patients, including 24 patients with pancreatic cancer, 165 with breast cancer, 98 with colorectal cancer and 1 with a neuroendocrine tumor of the small intestine.
