Úvod Benzodiazepiny (BZD) jsou jednou z nevíce diskutovaných skupin léčiv potenciálně nevhodných pro starší pacienty. Kvůli snížené funkci eliminačních orgánů a zvýšené citlivosti cílových receptorů jsou senioři zvláště zranitelní k jejich nežádoucím účinkům a jsou ve zvýšeném riziku vážných polékových komplikací, jako jsou pády, ortostatická hypotenze, synkopy, deprese, snížení kognitivních funkcí a léková závislost. Senioři v zařízeních dlouhodobé péče jsou ještě rizikovější vůči nežádoucím lékovým událostem kvůli svému horšímu zdravotnímu stavu, časté polymorbiditě, polyfarmakoterapii a zvýšené geriatrické křehkosti. Omezení preskripce BZD seniorům je součástí snah o racionální geriatrickou farmakoterapii a lepší zdravotní stav a kvalitu života starších pacientů. Metodika Tato diplomová práce přináší aktuální výsledky týkající se používání BZD v českých léčebnách dlouhodobě nemocných, získané z dat sesbíraných v letech 2022 až 2023 jako součást projektu START/MED/093 pod názvem "Racionální geriatrická farmakoterapie, léková pochybení a dostupnost klinicko-farmaceutických služeb v dlouhodobé péči." Data byla sbírána ve třech lokalitách, a to v Hlavním městě Praha, Ústeckém kraji a Pardubickém kraji. Pro každého pacienta byl zaznamenán kompletní seznam užívaných léků včetně dávkování, lékové... Introduction Benzodiazepines (BZDs) are among the most discussed groups of medications potentially inappropriate in older patients. Due to reduced function of elimination organs and increased sensitivity of target receptor sites, seniors are particularly vulnerable to their side effects and are at risk of serious medication-related complications, such as falls, orthostatic hypotension, syncopes, depression, cognitive impairment, and drug dependence. Seniors in long-term care facilities are especially sensitive to adverse drug outcomes due to their poorer health status, polymorbidity, polypharmacy and increased geratric frailty. Restricting the prescription of BZDs in seniors is part of the effort towards rational geriatric pharmacotherapy and better health and quality of life in older patients. Methods This thesis provides current results on the use of BZDs in Czech long-term care facilities, using data collected in 2022-2023 as part of the START/MED/093 project called "Rational Geriatric Pharmacotherapy, Medication Errors, and Availability of Clinical-Pharmaceutical Services in Long-Term Care."
