Akutní myeloidní leukémie (AML) je maligní hematologické onemocnění projevující se expanzí blastů v kostní dřeni a do periferní krve. Imunitní systém chrání tělo před leukémií odstraňováním transformovaných buněk. V AML jsou ale schopnosti imunitních buněk ovlivněny jak přímým kontaktem mezi leukemickými a efektorovými buňkami, tak cytokiny, metabolity a dalšími solubilními proteiny, které spolu s buňkami tvoří specifické mikroprostředí AML. Efekty cytokinů a dalších solubilních molekul v mikroprostředí AML nejsou zatím dostatečně popsány. Tato práce měla za cíl optimalizovat a zavést panel pro multiparametrickou průtokovou cytometrii pro měření buněčných populací a zavést multiparametrické testy hladin cytokinů, chemokinů a dalších solubilních proteinů v plazmě. Dalším cílem pak bylo pomocí těchto metod charakterizovat frekvence a funkční fenotyp buněčných populací a hladiny solubilních proteinů v mikroprostředí AML a popsat jejich vliv na závažnost onemocnění a celkové přežití pacientů. Pokusili jsme se také nalézt nové biomarkery imunitního úniku leukemických buněk. U pacientů jsme pozorovali supresivní mikroprostředí s aberantními hladinami solubilních receptorů a dalších proteinů. Toto mikroprostředí pak bylo odlišné v jednotlivých prognostických skupinách, do kterých byli pacienti rozděleni.... Acute myeloid leukemia (AML) is a malignant hematological disorder characterized by aberrant expansion of blasts in the bone marrow and peripheral blood. The immune system protects the body from leukemia by eliminating transformed cells. However, in AML, the abilities of immune cells are affected both by direct contact between leukemic cells and effector cells, as well as by cytokines, metabolites and other soluble proteins that, together with the cells, form the specific AML microenvironment. The effects of cytokines and other soluble molecules in the AML microenvironment are not sufficiently described yet. This thesis aimed to optimize and implement a multiparametric flow cytometry panel for the measurement of cell populations and to implement multiparametric assays for the analysis of cytokine levels, chemokines and other soluble proteins in plasma. The following goal was to use these methods to characterize the frequency and functional phenotype of cell populations and the levels of the soluble proteins and to describe their influence on disease severity and overall survival of the patients.
