Tato diplomová práce se zabývá vlivem vysokovýkonného laseru na měkké tkáně a pooperační jizvy v oblasti hlezenního kloubu. Práce je rozdělena na část teoretickou a praktickou. Teoretická část přináší shrnutí poznatků o traumatologii hlezenního kloubu a jeho operačním řešení. Dále je v této části práce popsané hojení ran, tvorba jizev, jejich vyšetření, hodnocení, rozdělení a léčba. A v neposlední řadě je uveden popis mechanismu laseru s jeho účinky a jeho využití v terapii. Hlavním cílem praktické části je zjistit vliv vysokovýkonného laseru na měkké tkáně a pooperační jizvu v oblasti hlezenního kloubu. Dílčím cílem je zhodnocení účinku vysokovýkonného laseru na snížení bolestivosti v ošetřované oblasti. Metodika: V rámci praktické části práce bylo otestováno 20 probandů (průměrný věk 42,3 let, SD ± 13). Intervenční skupina (n = 10) měla aplikovaný vysokovýkonný laser na oblast pooperační jizvy, kontrolní skupina (n = 10) tento druh léčby neměla. Pro hodnocení jizvy byla použita škála the Patient and Observer Scar Assessment Scale (POSAS) a pro zhodnocení bolestivosti byl využit tlakový algometr. Výsledky: Statisticky bylo prokázáno, že vysokovýkonný laser má na hladině významnosti α = 0,05 efekt na snížení bolestivosti (p = 0,0047). Dle výsledků škály POSAS hodnocené z pohledu pacienta byl... This thesis deals with the effect of High Intensity Laser on soft tissues and postsurgical scars in the ankle joint area. The thesis is divided into two parts, theoretical and practical. The theoretical part presents a summary about ankle joint traumatology and its surgical management. Further there is also information about wound healing, scar formation, their examination, assessment scales and treatments. Last but not least there is a description about the laser mechanism, effect and treatment use. The main focus point of the practical part is to discover the effect of the High Intensity Laser used on soft tissues and postsurgical scars in the ankle joint area. A sub-objective is to evaluate the high power laser effect on pain reduction in the treated area. Methods: There were 20 probands tested in the practical part (average age 42,3, SD ± 13). The intervation group (n = 10) has applied High Intensity Laser Therapy (HILT) on postsurgical scars. The control group (n = 10) haven't HILT. The Observer Scar Assessment Scale (POSAS) was used for scar evaluation and pressure algometer was used for pain evaluation.
