Analýza lékových problémů ("drug-related problems") ve zdravotnickém zařízení VIII Autor: Kateřina Kraušnerová Vedoucí diplomové práce: PharmDr. Martin Doseděl, Ph.D. Katedra sociální a klinické farmacie, Farmaceutická fakulta v Hradci Králové, Univerzita Karlova Úvod: Lékový problém (DRP - drug-related problém) je dle Pharmaceutical Care Network Europe (PCNE) definovaný jako událost nebo okolnost související s podáním léčiva, která skutečně nebo potencionálně ovlivňuje zamýšlené terapeutické cíle. Farmaceuti bývají součástí multidisciplinárních týmů, kde napomáhají s rozhodováním ohledně terapie pacienta, monitoringu, mají důležitou roli v odhalování lékových problémů a přispívají tak ke zvyšování účinnosti a bezpečnosti farmakoterapie. Cíl: Cílem teoretické části práce bylo vytvoření přehledu lékových problémů vyskytujících se ve skupině léčiv antidepresiva, ATC N06A, a tyto problémy zpracovat dle PCNE V5.01 klasifikace. Cílem praktické části byla analýza lékových problémů ve zdravotnickém zařízení, jejich zařazení do kategorií modifikované klasifikace PCNE V5.01, zhodnocení jejich závažnosti a míry akceptace intervencí provedených klinickým farmaceutem. Metodika: Sběr dat probíhal v období od 7. února 2022 do 7. března 2022 v jedné nemocnici v České republice. Celkem byla data získána od 68... Analysis of drug-related problems in healthcare facility VIII Author: Kateřina Kraušnerová Supervisor of the thesis: PharmDr. Martin Doseděl, Ph.D. Department of Social and Clinical Pharmacy, Faculty of Pharmacy in Hradec Králové, Charles University Backgroung: Drug-related problem (DRP) is according to Pharmaceutical Care Network Europe (PCNE) pharmacotherapy related event or circumstance, which actually or potentionaly interfer with a terapeutic purpose. Pharmacists tend to be part of multidisciplinar teams, they help with decisions concerning therapy of the patient, monitoring, participating in the detection of DRPs and by that helping increase effect and safety of pharmacotherapy. Aim: The aim of the theoretical part of this thesis is to cteate summary of DRPs which occurs in antidepressant drug group, ATC N06A, and classify these problems using PCNE V5.01 classification. The aim of the practical part of this thesis is to detect DRPs in healthcare facility, classify these DRPs using PCNE V5.01 classification and asses their relevance and acceptance of interventions made by clinical pharmacist.
