Hodnocení intervencí klinického farmaceuta na oddělení nemocnice III Autor: Šárka Elbelová Vedoucí diplomové práce: PharmDr. Martin Doseděl, Ph.D. Katedra sociální a klinické farmacie, Farmaceutická fakulta v Hradci Králové, Univerzita Karlova Úvod a cíl: Vzhledem k rozvoji zdravotní péče je velmi důležité analyzovat přínos klinického farmaceuta a potenciál k dalšímu rozvoji lékového managementu. V teoretické části se práce věnuje problematice akceptace doporučení klinického farmaceuta lékaři a přináší souhrn poznatků čerstvých zahraničních studií na toto téma. Cílem praktické části diplomové práce bylo analyzovat intervence klinického farmaceuta, typy lékových problémů, se kterými se setkává, jejich závažnost a možná řešení a také jejich akceptaci lékaři. Metodika: Prospektivní, deskriptivní studie byla prováděna na ošetřovatelském úseku interního oddělení nemocnice v Olomouckém kraji během října až prosince v roce 2021. Do studie byli zařazeni nově přijatí pacienti, u kterých byla provedena revize farmakoterapie. Údaje o pacientech (věk, pohlaví, diagnózy, laboratorní parametry atd.) byly zaznamenány do webové databáze. Lékové problémy byly tříděny dle modifikované PCNE klasifikace V5.01. Výsledky: Do studie bylo zařazeno 66 pacientů, celkem bylo nalezeno 143 lékových problémů, což odpovídá 2,17... In the theoretical part, special focus is on the phenomenon of physicians' acceptance of recommendations made by clinical pharmacist. It also summarizes the recent studies on this topic. In the following analysis in practical part, the interventions of clinical pharmacist are put under scrutiny as well as the types of the drug-related problems, which are encountered by the clinical pharmacist, the seriousness of these problems and their possible solutions. Furthermore, the physicians' acceptance of recommendations made by clinical pharmacist is measured. Methods: A prospective, descriptive study was conducted in the Nursing Unit of the Department of the Internal Medicine in the Hospital in Olomouc region between October and December 2021.
