Lenka Krbalová Školitel: RNDr. Miloslav Macháček, Ph.D. Školitel specialista: MUDr. Jiří Soukup, Ph.D. Název rigorózní práce: Somatotropní adenomy hypofýzy u pacientů s akromegalií - stanovení míry exprese proteinů s potenciálním vlivem na terapeutickou odpověď Akromegalie je vzácné onemocnění, projevující se širokou paletou různých příznaků (zvětšením akrálních částí těla, makroglosií, neuropatií, hypertenzí, kardiomyopatií aj.), které jednak zkracují život, jednak ve svých důsledcích značně snižují jeho kvalitu. Je způsobena zvýšenou sekrecí růstového hormonu v dospělosti. Příčinou této zvýšené sekrece bývá nejčastěji somatotropní adenom hypofýzy. Léčba akromegalie vyžaduje ve většině případů komplexní přístup, jedná se o kombinaci chirurgického zásahu, radioterapie a farmakoterapie. Ve farmakoterapii se využívají především somatostatinová analoga (SSA). Jsou to látky, které po vazbě na somatostatinový receptor vykazují obdobné farmakodynamické účinky jako somatostatin. Jejich podání vede k poklesu sekreční aktivity tumoru a k redukci jeho objemu. Do první generace řadíme oktreotid a lanreotid (ty dnes představují nejrozšířenější medikamentózní léčbu akromegalie), do druhé pasireotid.... These symptoms significantly decrease quality of life and shorten life span. It is caused by increased secretion of growth hormone in adulthood. The cause of this increased secretion is most often a somatotroph adenoma. In most cases, the treatment of acromegaly requires a comprehensive approach. It is a combination of surgical intervention, radiotherapy and pharmacotherapy. Somatostatin analogues are mainly used in pharmacotherapy. These compounds have similar pharmacodynamic effects as somatostatin after binding to the somatostatin receptor. The administration of these compounds leads to a decrease in the secretory activity of the tumour and subsequently to the reduction of its volume. Octreotide and lanreotide belong...
