Kolorektální karcinom (CRC) je závažné onemocnění, jehož incidence v poslední době neustále narůstá. Pacienti, jejichž léčba je započata v raných stadiích nemoci mají vysokou šanci na vyléčení a kvalitní život. Se včasným odhalením nemoci, monitoringem stavu pacienta i vhodným určením diagnózy by mohla pomoci tzv. tekutá biopsie, při ní dochází k získání a analýze biomarkerů z různých tělních tekutin. Cílem této práce je izolace a analýza cirkulující nádorové DNA (ctDNA) z moči pacientů s pokročilým CRC, u kterých byla detekována ctDNA v plazmě. Pro analýzu je použita metoda založená na principu PCR s tvorbou heteroduplexů a následném rozdělení "wild type" a mutovaných fragmentů DNA pomocí denaturační kapilární elektroforézy. Na rozdíl od vzorků plazmy, přítomnost mutace KRAS ve vzorcích moči se nepotvrdila. Zavedený postup pro analýzu plazmy, snaha o nabohacení ctDNA tzv. přečištěním ani další testované izolační soupravy s využitím zvolené detekční metody neumožnily záchyt mutace KRAS v moči prostřednictvím ctDNA. Ačkoli se vyšetření ctDNA z moči obecně jeví jako nadějná metoda pro zlepšení úrovně léčby (kolorektálního) karcinomu, a ačkoli by její použití mohlo mít celou řadu výhod, je nutné této problematice věnovat ještě mnoho úsilí a času, než se stane rutinní záležitostí. Colorectal cancer (CRC) is a serious disease whose incidence has been steadily increasing. There is a high chance of cure and a good quality of life, if the treatment is started in the early stages of this disease. The so-called liquid biopsy, in which biomarkers from various body fluids are obtained and analyzed, could help with early detection of the disease, monitoring of the patient's condition and appropriate determination of the diagnosis. The aim of this study is to isolate and analyse circulating tumor DNA (ctDNA) from the urine of patients with advanced CRC. In these patients the ctDNA was detected in plasma. The method used for analysis is based on the principle of PCR with the formation of heteroduplexes, followed by the separation of "wild type" and mutated DNA fragments by denaturing capillary electrophoresis. Unlike plasma samples, the presence of KRAS mutations was not confirmed in urine samples. Neither the established procedure for plasma analysis, the attempt to enrich ctDNA by so-called purification, nor other tested isolation kits using the selected detection method allowed detection of the KRAS mutation in urine via ctDNA.
