Komplexní chromozomové aberace jsou popisovány přibližně u 20 % pacientů s myelodysplastickými syndromy (MDS) a jsou asociovány se špatnou prognózou. Mechanizmy a možné příčiny zodpovědné za vznik těchto aberací však nejsou zcela jasné. Byly popsány dva modely, a to rozpad jednotlivých chromozomů či jejich částí během tzv. buněčné krize (chromothripsis) a/nebo postupná kumulace chromozomových aberací v průběhu onemocnění (klonální vývoj). Kombinací cytogenomických metod jsme vyšetřili 61 vzorků kostní dřeně dospělých nemocných s MDS a komplexním karyotypem. Ve většině případů jsme nalezli nebalancované aberace spojené se ztrátami genetického materiálu. Do přestaveb nejčastěji vstupovaly chromozomy 5, 7 a 12. Klonální vývoj jsme detekovali u 26, chromothripsis u 12 a oba mechanizmy u 14 nemocných. Nejkratší celkové přežití měli pacienti s deletovaným chromozomem 5 zahrnutým do komplexních přestaveb. Příčina vzniku komplexních aberací neměla na přežití vliv. Cytogenomická analýza komplexních aberací umožňuje detekovat balancované i nebalancované změny a identifikovat významné procesy tumorogeneze jako je klonální vývoj a chromothripsis. Studium chromozomových aberací a identifikace mechanizmů, které hrají významnou roli v průběhu onemocnění, se tak výrazně podílí na správném stanovení diagnózy a... Complex chromosomal aberrations occurs are described in approximately 20 % of patients with myelodysplastic syndrome (MDS) and are associated with a poor prognosis. Nevertheless, the mechanism and possible causes responsible for the emergence of these aberrations are not fully understood. There are two models describing the emergence of these aberrations, namely shattering of single chromosomes or their parts during the so-called cellular crisis (chromothripsis) and/or progressive accumulation of chromosomal aberrations during the course of the disease (clonal evolution). Using combination of cytogenomic methods we examined 61 samples of bone marrow from adult patients with MDS and a complex karyotype. Unbalanced aberrations with loss of genetical material were found in most cases. Chromosomes 5, 7 and 12 were most frequently involved in rearrangements. Clonal development, chromothripsis and both mechanism was detected in 26, 12 and 14 patients, respectively. Patients with deleted chromosome 5 included in complex karyotype had the shortest overall survival. The cause of emergence of complex aberrations did not affect survival.
