9 Souhrn V rámci této dizerta ní práce jsem se zabýval patofyziologií renálního postižení u pacient s chronickým onemocn ním ledvin - chronic kidney disease (CKD), definovaným dle Kidney Disease Improving Global Outcomes (KDIGO) jako p etrvávání renálního postižení i snížení funkce ledvin po dobu nejmén 3 m síc . Studie Mo ové koncentrace TGF beta 1 u d tí s hydronefrózou je v nována poškození ledvin u d tí s obstruk ní uropatií (OU), zam ili jsme se na identifikaci neinvazivního markeru transforming growth factoru (TGF) beta 1 u tohoto onemocn ní a na jeho korelaci s laboratorními známkami poškození renálního parenchymu. Cytokin TGF beta1 pat í mezi hlavní mediátory fibrogeneze u ledvinných onemocn ní. U pacient s hydronefrózou obstruk ního typu byla již v n kolika odborných pracích zjišt na zvýšená mo ová koncentrace TGF beta 1, tento nález ale jiní auto i nepotvrdili. V naší práci jsme jako první popsali korelace mo ových koncentrací TGF beta 1 s laboratorními markery postižení ledvin. Ve studii asné projevy aterosklerózy u d tí s chronickým onemocn ním ledvin jsme se zam ili na zkoumání následk CKD a zabývali jsme se jeho vlivem na vznik aterosklerózy. Zde jsme konstatovali, že pacienti v kone ném stádiu selhání ledvin jsou mnohonásobn více ohroženi kardiovaskulárními komplikacemi než zbytek populace. V... 11 English Summary The objective of my PhD. thesis was to examine the pathophysiology of renal damage in patients with chronic kidney disease, defined by Kidney Disease Improving Global Outcomes (KDIGO) as a persistent renal damage or lowering the kidney function at least for 3 months. In the study Urinary TGF beta 1 in children with hydronephrosis we dealt with renal damage in children with obstructive uropathy. We focused on the identification of non- invasive marker transforming growth factor (TGF) beta 1 of this disease and its correlation with laboratory indications of renal parenchymal damage. Cytokine TGF beta 1 belongs among the main mediators of fibrogenesis in kidney diseases. A higher urinary concentration of TGF beta1 in patients suffering from obstructive hydronephrosis has been found in several studies before, but it has not been confirmed by other authors. In our study we are the first to describe the correlation of urinary concentrations TGF beta1 with laboratory markers of kidney damage.
