Abstrakt: [cze][eng] Abstrakt Stále modernější prostředky ve zdravotnictví umožňují řešit léčbu zlomenin především operací. Avšak ne každý pacient je k operaci indikován. I dnes se setkáváme na standardních ošetřovacích jednotkách i jednotkách intenzivní péče s pacientem s extenzí. Proto sestry musí vědět, jak pečovat o tohoto pacienta a jak předcházet imobilizačnímu syndromu, jež u pacienta s extenzí hrozí, a může mu přivodit řadu závažných komplikací. Cíli této bakalářské práce bylo zjistit negativní faktory ovlivňující vznik imobilizačního syndromu u pacienta s extenzí, porovnat odlišnosti v používání pomůcek k prevenci imobilizačního syndromu u pacienta s extenzí na standardní ošetřovatelské jednotce a jednotce intenzivní péče a zjistit, zda jsou pacienti s extenzí seznámeni s možností vzniku imobilizačního syndromu. Byly stanoveny čtyři hypotézy. V hypotéze 1 se předpokládalo, že nevhodně zvolené polohování ovlivňuje vznik imobilizačního syndromu. Tato hypotéza se potvrdila, nevhodně zvolené polohování se nejčastěji podílí na vzniku imobilizačního syndromu. V hypotéze 2 se předpokládalo, že nedostatečná hygienická péče ovlivňuje vznik imobilizačního syndromu. Tato hypotéza se taktéž potvrdila, nedostatečná hygienická péče ovlivňuje vznik imobilizačního syndromu. V hypotéze 3 se předpokládalo, že na jednotce intenzivní péče využívají modernější prostředky k péči o pacienta s extenzí než na standardní ošetřovací jednotce. Tato hypotéza se rovněž potvrdila, na jednotce intenzivní péče využívají modernější prostředky k péči o pacienta s extenzí než na standardní ošetřovatelské jednotce. V hypotéze 4 bylo předpokládáno, že pacienti s extenzí nemají informace o možnosti vzniku imobilizačního syndromu. Tato hypotéza byla vyvrácena, pacienti s extenzí jsou informováni o možnosti vzniku imobilizačního syndromu. Jako výzkumný vzorek byly osloveny sestry na standardních odděleních a jednotkách intenzivní péče Nemocnice České Budějovice, a.s., Fakultní Nemocnice Olomouc a Vojenské Nemocnice Olomouc. Sběr dat byl proveden metodou kvantitativního výzkumu pomocí anonymních dotazníků. Celkem bylo rozdáno 110 dotazníků, z nichž mohlo být použito ke konečnému vyhodnocení 68 dotazníků. Zbylé dotazníky bylo nutno z šetření vyřadit z důvodu nedostatečného vyplnění sestrami. Tato práce by mohla přispět k doplnění edukačního materiálu pro pacienty s extenzí a k rozšíření znalostí sester v oblasti péče o tohoto pacienta. Abstract More and more up-to date equipment in the health-care sector makes the treatment of fractures possible, especially by means of surgery. However, not every patient is indicated for surgery. Even today, we can see a patient with the extension at the standard and intensit care units. Therefore, nurses must know how to take care of this patient and how to prevent the risk of immobilization syndrome in patient with extension that may cause many serious complications. The aims of this study were to identify the negative factors affecting a development of the immobilization syndrome in a patient with the extension, to compare the differences in the use of immobilization devices to prevent the immobilization syndrome in a patient with the extension at the standard and intensive care unit and to determine whether patients with extension are familiar with possibility of the immobilization syndrome. Four hypotheses have been defined. In the first hypothesis we assumed that inconveniently selected positioning affected the emergence of the immobilization syndrome. This hypothesis was confirmed. Inconveniently selected positioning is most often involved in the immobilization syndrome. In the second hypothesis we assumed that inconvenient hygienic care affected the emergence of the immobilization syndrome. This hypothesis was also confirmed. The insufficient hygienic care affects a development of the immobilization syndrome. In the third hypothesis we assumed that the intensive care unit used more up-to-date equipment to take care of a patinet with the extension compared to the standard unit. In the fourth hypothesis we assumed that patients with extension had no information about the possibility of the imobilization syndrome. This hypothesis was disproved. The patients with the extension are informed about the possibility of the immobilization syndrome. The nurses at the standard and intensive care units in Hospital České Budějovice, a.s. (PLC) and Military Hospital Olomouc were asked to be a research sample. Data were collected by the quantitative exploration method using anonymous questionnaires. In total 110 questionnaires were distributed of which 68 were used for the final evalution. The remainig questionnaires had to be withdrawn from the investigation because they were completed insufficiently by the nurses. This study could help to supplement the educational material for patients with the extension and to increase the knowledge of the nurses in the area of care of such patient.
