Využití PET/CT vyšetření při plánování radioterapie nemalobuněčných plicních karcinomů Úvod: V současné době je standardním vyšetřením při plánování radioterapie u pacientů s nemalobuněčnými plicními karcinomy (non-small cell lung cancer; NSCLC) kontrastní CT vyšetření. Recentními studiemi bylo prokázáno, že PET/CT vyšetření má vyšší senzitivitu a specificitu v diagnostice plicních karcinomů a vede tedy k vyšší přesnosti při konturaci cílových objemů u NSCLC. Cíl práce: Hlavním cílem disertační práce bylo zjistit vliv použití plánovacího PET/CT vyšetření při plánování radioterapie nemalobuněčných plicních karcinomů, a to zejména ve smyslu změny velikosti cílových objemů. Dalším cílem této práce bylo zjistit vliv změn velikostí cílových objemů na hodnoty TCP a NTCP a v neposlední řadě také vliv kombinovaného vyšetření na staging onemocnění a na léčebnou strategii. Materiál a metodika: V období mezi říjnem 2008 a listopadem 2013 bylo do studie zařazeno 42 pacientů s histologicky potvrzeným inoperabilním NSCLC, kteří byli indikováni k radikální radioterapii. Všichni pacienti podstoupili plánovací PET/CT vyšetření. Vytvořil jsem vždy dvě různé konturace cílových objemů (podle CT a podle PET/CT) a byly vytvořeny dva různé plány, každý s vlastním dávkově objemovým histogramem (dose-volume histogram;... The impact of PET/CT scanning in the radiotherapy planning of non-small cell lung cancer Background: In the current practice of radiotherapy planning and target volume determination in patients suffering from the non-small cell lung cancer (NSCLC), a contrast-enhanced CT is standard procedure. Contemporary studies have proven that PET/CT examination attains higher sensitivity and specificity in the diagnosis of lung cancer and also leads to higher accuracy in the process of target-volume delineation in NSCLC. Aim: To compare radiotherapy plans made according to CT and PET/CT examinations in the same patient and to investigate the impact of changes in target volume sizes on tumor control probability (TCP) and normal tissue complication probability (NTCP) and the impact of combined PET/CT examination on the staging of the disease and treatment strategy. Materials and Methods: Between October 2008 and November 2013, 42 patients suffering from pathologically proven inoperable locally advanced NSCLC that were referred to radical radiotherapy were involved in our study.
