Souhrn Úvod: Aortální stenóza je nejèastìjší a nejèastìji operovanou chlopenní vadou, jejíž medikamentózní léèba dosud není známa. Cílem naší práce bylo stanovit prediktory progrese kalcifikace aortální chlopnì a dále zjistit asociaci progrese aortální stenózy s léèbou bisfosfonáty. Metody: První èást tvoøí prospektivní sledování souboru konsekutivních pacientù s angiograficky prokázanou významnou koronární nemocí, kteøí byli rozdìleni do 3 skupin: pacienti s aortální sklerózou, s aortální stenózou (alespoò støednì závažná, AVA < 1cm/m2 nebo støední gradient 30mmHg) a pacienti s iniciálnì fyziologickým nálezem na aortální chlopni. Sledovali jsme klinický stav celého souboru a pacientù s aortální stenózou pøed a po náhradì chlopnì, pomocí MDCT jsme hodnotili progresi kalcifikace aortální chlopnì u pacientù s aortální sklerózou a iniciálnì normální aortální chlopní a identifikovali prediktory progrese kalcifikace metodou multivariantní analýzy. Druhou èást tvoøí pilotní retrospektivní studie pacientù s aortální stenózou a dvìma echokardiografiemi v intervalu >8 mìsícù. Pacienti byli rozdìleni do dvou skupin: léèených bisfosfonáty pro osteoporózu a neléèených. Srovnali jsme anualizovanou zmìnu gradientu mezi skupinami a identifikovali prediktory progrese aortální stenózy. Výsledky: 1. Do prospektivního... Background: Calcific aortic stenosis is the most frequent and most frequently operated valvular diasease. Currently, there is no known medical treatment to slow the progression of the disease. The aim of our study was first, to assess predictors of the progression of the aortic valve calcification, and second, to assess the association of the progression of aortic stenosis with the bisphosphonate treatment. Methods: Part I - prospective follow-up of consecutive patients with coronary artery disease associated with aortic stenosis (mean transvalvular aortic gradient of 30 mmHg or greater), aortic sclerosis or normal aortic valve were prospectively identified. Clinical, laboratory, echocardographic, angiographic and calcium score evaluation using multidetector computer tomography at baseline, and at the end ofthe study. Predictor of calcification were sought using multivariate regression. Part II - pilot retrospective study of patients with aortic stenosis and two echocardiographies >8 months apart. The patients were divided into those treated with bisphosphonates for osteoporosis and those not treated. We compared the annualized gradient change between the groups and identified predictors of AS progression.
