[cze][eng] V roce 2011 bylo vydáno Doporučení pro laboratorní diagnostiku funkčních a autoimunitních onemocnění štítné žlázy, jehož cílem je doporučit praktickým lékařům správný postup při výběru parametrů pro vyšetřování funkce štítné žlázy a najít tak způsob, jak ušetřit peníze na zbytečných vyšetřeních, které by nemocnice mohly využít v jiné oblasti. K ověření, zda lékaři dodržují tato doporučení, byl použit soubor pacientů VFN v Praze získaný za rok 2011, který byl statisticky vyhodnocen. Sledovanými parametry byly tyreotropin (TSH), volný trijódtyronin (FT3), celkový trijódtyronin (T3), volný tyroxin (FT4), celkový tyroxin (T4), protilátky proti tyreiodální peroxidáze (anti - TPO), protilátky proti tyreoglobulinu (anti - Tg), tyreoglobulin (Tg) a tyroxin vážící globulin (TBG), z nichž základní testem by měl být podle doporučení TSH a při jeho hodnotách mimo referenční meze pak má následovat test FT4, případně stanovení protilátek. Další parametry jsou pro prvotní vyšetření nadbytečné, některé se dnes již nepoužívají nebo by je měly vyšetřovat pouze kliniky specializované v oblasti endokrinologie. Bylo zjištěno, že mnoho lékařů tato zbytečná vyšetření stále objednává. Jen za jeden rok bylo provedeno při vyšetřování funkce štítné žlázy 13,8 % nadbytečných stanovení parametrů z celkového počtu 44 415 vyšetření, v...In 2011 Recommendation for laboratory diagnostics of functional and autoimmune thyroid diseases was published. Its target is to recommend correct method when choosing thyroid examination parameters to general practitians and thus find a way how to save money on useless examinations which then can be used by hospital in other area. To verify physicians adhere to these recommendations file of VFN patients in Prague for year 2011 was used and statistically evaluated. Watched parameters included thyrotropin (TSH), free triiodothyronine (FT3), total triiodothyronine (T3), free thyroxine (FT4), total thyroxine (T4), thyroid peroxidase antibodies (anti - TPO), thyroglobulin antibodies (anti - Tg), thyroglobulin (Tg) and thyroxine - binding globulin (TBG). Basic test according to published recommendation should include only TSH and if out of reference limits then it should be followed by FT4 test or antidote assessment. Other parameters are unnecessary for initial examination, some of them are not used anymore or should be examined only by clinics specialized in endocrinology. Based on the file records many physicians are still ordering unnecessary examination parameters.
