SOUHRN Úvod: Lipozomální cytostatika vyu!ívají enkapsulaci ú"inné látky do lipozom# s cílem modifi- kovat její farmakokinetick$ a toxikologick$ profil ke zv$%ení benefitu lé"by. Jedním z nejv$- znamn&j%ích zástupc# t&chto lé"iv je pegylovan$ lipozomální doxorubicin (PLD). Ve srovnání s mate'skou látkou (doxorubicin, DOX) má ni!%í kardiotoxicitu a hematotoxicitu, av%ak vy%%í in- cidenci mukokutánní toxicity. Specifické farmakokinetické vlastnosti PLD umo!(ují odstran&ní frakce podané dávky metodou extrakorporální eliminace v dob&, kdy ji! byla cytostatikem dosta- te"n& saturována nádorová tká(, a tímto zp#sobem sní!it incidenci a záva!nost zejména muko- kutánní toxicity. Cíl: Tato diserta"ní práce m&la za cíl hodnocení ú"innosti a bezpe"nosti plazmafiltrace (PF) jako metody extrakorporální eliminace PLD s cílem zv$%ení benefitu cytostatické lé"by. Dále byl hodnocen vliv PF na toxicitu lé"by, farmakokinetika PLD, vliv laboratorních a klinick$ch faktor# na farmakokinetiku PLD a vliv PF na ú"innost protinádorové lé"by. Materiál a metody: Do studie bylo za'azeno celkem 16 pacientek s platina-rezistentním kar- cinomem ovaria lé"en$ch PLD v dávce 50 mg/m2 v jednohodinové i.v. infúzi ka!dé 4 t$dny, celkem 3-6 cykl#. PF byla zahájena za 44-46 hod po podání PLD. Odb&ry na stanovení plazma- tického PLD a volného DOX... Extracorporeal elimination of circulating pegylated liposomal doxorubicin to enhance the benefit of cytostatic therapy in ovarian cancer Background: Encapsulation of cytotoxic drugs into liposomes changes their pharmacokinetic and toxicologic properties in order to enhance the benefit of chemotherapy. Pegylated liposomal doxo- rubicin (PLD) is one of the most prominent drugs in this group. Despite its reduced cardio- and hematologic toxicity, it confers a higher risk of mucocutaneous toxicity in comparison to its parent drug (i.e. doxorubicin, DOX). The specific pharmacokinetic properties of PLD enable its partial elimination from plasma after reaching maximum concentration within the tumor tissue in order to reduce its toxicity. Aim: This doctoral thesis evaluates the efficacy and safety of plasmafiltration (PF) as a me- thod of extracorporeal PLD elimination, seeking to enhance its therapeutical benefit. The reduc- tion of PLD-related toxicity thanks to the application of PF was determined. Further, PLD pharmacokinetics and the factors affecting it, as well as the effect of PF on the outcome of cancer therapy, were also analyzed.
