Familiární adenomatózní polypóza (FAP) je dědičné autozomálně dominantní onemocnění způsobené zárodečnými mutacemi v tumor supresorovém genu APC. Projevuje se tvorbou stovek až tisíců kolorektálních adenomatózních polypů s vysokým rizikem malignizace v adenokarcinom tlustého střeva a/nebo konečníku. Cílem této práce je re-analyzovat soubor FAP suspektních probandů z let 1993-2004, u kterých se diagnózu v té době běžně používanými metodami molekulární diagnostiky nepodařilo potvrdit. Pomocí sekvenování nové generace (NGS) bylo na platformách Miseq a NextSeq (Illumina®) za využití NGS panelu CZECANCA verze 1.2 (Czech Cancer Panel for Clinical Application) osekvenováno celkem 78 vzorků DNA izolované z periferní krve probandů. Panel umožňuje vyšetřit exonové a exon-intronové oblasti 226 genů souvisejících s nádorovou predispozicí, přičemž nově zahrnuje také diagnosticky významnou oblast promotoru 1B genu APC. V rámci re-analýzy byla patogenní varianta v genu APC nalezena u 18 % pacientů, 11 % pacientů pak nese patogenní variantu v některém z dalších genů klinicky souvisejících s tvorbou kolorektálních polypů. Dalších 13 % probandů patří mezi nositele varianty s nejasným klinickým významem. NGS panel CZECANCA v.1.2 umožnil konfirmovat či přehodnotit suspektní diagnózu 22 probandů. Minimálně 5 variant... Familial adenomatous polyposis (FAP) is a condition caused by germline mutations in tumor suppressor gene APC, inherited in autosomal dominant manner. Patients with FAP develop hundreds to thousands of adenomatous colorectal polyps with extremely high risk of malignant reversal into adenocarcinoma of colon and/or rectum. The aim of this thesis is to re-analyze a cohort of highly suspected FAP probands from years 1993-2004 whose diagnosis previously failed to be confirmed by at that time commonly used methods of molecular diagnostics. Next generation sequencing on MiSeq and NextSeq platforms (Illumina®) was performed on 78 samples of probands' DNA, isolated from peripheral blood, using gene panel CZECANCA version 1.2 (Czech Cancer Panel for Clinical Application). The panel enables sequencing of exons and exon-intron junctions of 226 genes linked to hereditary cancer predispositions, newly also including the diagnostically important promoter 1B region of APC. Pathogenic variant in the APC gene was detected in 18 % of re-analyzed probands, 11 % of probands carry pathogenic variants in other genes associated with colorectal polyps.
