[cze][eng] Hypertrofická kardiomyopatie (HKMP) je onemocnění myokardu, které vzniká v důsledku mutací v genech kódujících bílkoviny sarkomery srdečního svalu. Je charakteristická nejenom širokou genotypovou, ale i širokou fenotypovou variabilitou. Již od 90-tých let minulého století, kdy byla nalezena první mutace odpovídající za vznik HKMP, se úsilí velkého množství vědců soustředilo na zjištění klinických projevů HKMP v závislosti na mutaci specifického genu. V naší práci jsme jako první v České republice zjistili zastoupení mutací odpovědných za vznik HKMP v 4 nejčastěji postižených genech. Zjistili jsme, že rozložení mutací je poněkuď rozdílně oproti jiným populacím, což může mít významný praktický dopad. Při zkoumaní vztahu mezi přítomnosti mutace v specifickém genu a klinickým obrazem jsme nezjistili žádnou korelaci, která by na základě genetického vyšetření opravňovala predikovat morfologické projevy či klinický obraz u postiženého jedince. V naší studii, kdy jsme nemocným s neobstruktivní formou HKMP podávali blokátor AT-1 receptorů candesartan, jsme zjistili výrazný pozitivní efekt farmaka na velikost hypertrofie a funkci LKS a na klinické symptomy u pacientů s HKMP.Hypertrophic cardiomyopathy (HCM) is a hereditary disease, which is caused by mutations in genech coding for proteins of carcomere of heart muscle. Disease is characterised by wide genotypic and fenotypic heterogeneity. Since the 90-th of 20-th century, when the first mutation responsible for HCM was identified, many research was done in detecting the correlation between mutant gene and clinical picture of the affected patient. As the first in Czech republic, we determined the frequencies of 4 most common genes in HCM in czech patients. We figured out, that spectrum of mutations is somehow different from another populations. We didn't find any correlation between specific gene mutation and morfologic and clinical characteristics of affected subjects. In our study with AT-1 receptor blocker - candesartan, we demostrated positive effect of candesartan on level of hypertrophy and function of the left heart ventricle and clinical symptoms of the patients with HCM.
